First line treatment if Motor symptoms are affecting patient’s quality of life: LEVODOPA. It is usually combined with Carbidopa (decarboxylase inhibitor) as this prevents the peripheral metabolism of Levodopa to Dopamine outside the brain (improving CNS bioavailability) and hence can reduce side-effects. Dopamine agonist like Pramipexol, Ropinirole maybe started in younger patients. It may not be as effective as Levodopa but will have few side-effects. Selegiline (MAO-B) is often used to treat early disease and can provide mild symptom relief. COMT inhibitors (Tolcapone, Entacapone) which prevents breakdown of levodopa- are used in combination with Levodopa. Antimuscarinics (Procyclidine, Benzottopine, Benzhexol) are used, mainly for drug induced Parkinsonism. For some patients who do not response to all these measures, Deep Brain Stimulation (DBS) techniques have become the surgical procedure of choice. Sir but levodopa causes more damage to neurons in basal ganglia and it is going to be helpful only for max 3 yrs so... Isnt it better to start with dopamine agonists? And why dopamine agonists are not used in old paitents?

Is it right sir ? History - my father has been behind the jail for 4 years , he was allegedly made accused in a case ..and his pension funds gratituty is stopped by government..

donepezil and carbidopa-levodopa.. donepezil is known to alleviate non responsiveness to carbidopa.
