Yes when you get hit by a star meteors or don't defeat the Fake Millennium Star in under 3 minutes.

Yes. You can select it in the Mini-Game Room, and lose on purpose. There is no GAME OVER on the Story Mode version, just the one you select from the Mini-Game Room, and get hit once on purpose.
Edit: I forgot to tell you that you need to play Story Mode as any character, any difficulty you want, and what kind of Mini-Games, and beat the entire Story until you reach Stardust Battle, and then the Mini-Game Book will open. That's how you unlock this Mini-Game in the Mini-Game Library.
