    Thank you for the video. In a comatose patient with an intact brainstem, the eyes drift toward the irrigated ear, no response to stimulation indicates brainstem injury. It's important to mention that you should wait for 3 to 5 minutes for the 1st response to disappear before repeating on the opposite side.

    What happens if we instill cold water in both ears simulteneously when cortex and brain stem are intact?

    I guess...there is a mistake sir....when you instill cold water into Right ear,there is initial eye movement to ipsilateral side(Right) and then horizontal nystagmus to contralateral side(Left)....which goes by COWS....cold-nystagmus in opposite...But,in your video....it is saying opposite ....I apologize,if I am wrong...?

      That is only present in awake patient where the cortical activity is present.

      This reflex was triggered by sudden and drastic change in temperature
