It's a variation on drop C for the whole King Buzzo album, CGDgbe. Same for other melvins songs like Roman Bird Dog , Evil New War God, AMAZON etc.

Pretty sure it's the only C tuning Buzz uses, too; I don't think he's ever used normal drop C tuning.

I think the guy on the cover is a parody of Nick Cave. Looks like him. Got the cowboy hat mocking his rockabilly influence. Covered in pills and needles, mocking his drug use.
