Having worked in elderly care, I’ve seen dementia in all its forms at every stage and more times than I can count.
It was incredibly difficult just being the carer but seeing what it does to families, arguing about whether or not someone should go into full time care, almost giving up on the person inside only to see a little flash of who they were which threw the family into confusion and turmoil, watching someone get angry and frustrated because they still think they’re 15 and their daughter is their mother, watching someone strong, confident and proud slowly crumble away, like every day one piece of their soul was being removed and they were becoming less and less the person they were…
    Absolute bastard of an illness

How aware are they of their condition? Are they able to grasp and understand it at first and as it progresses, they lose that ability? I want to know because I'm ending it if I ever get it (and there's a chance I will).

@@Dead_Again1313 Unfortunately, it’s impossible to give a direct answer.
Some are not told by families which, although protects the sufferer early in, starts to seriously derail their mental health. It’s one thing to be aware of their memory to be less reliable and it inevitably becomes a little joke between the sufferer and the family as both attempt to keep it light but it’s entirely different when the memories begin to fade completely
There are some who are aware of their diagnoses and, throughout their illness, are aware of the situation and are also aware of how severe it is and that it will inevitably get worse. It’s rare, though not impossible, for someone to be completely aware that though they don’t recognise the people around them, they know why they don’t remember. People with forms of frontal lobe epilepsy can relate to this better than anyone as, following a seizure (which, with FLE, doesn’t involve violate shakes but rather they stare off into space and, in the space of a few minutes, they feel like they’ve lived a life, often many lives, of many years and, on recovering from the seizure, they need to spend a couple of days reorganising their brain into which memories are genuine and which are false.
By far the most common, understandably, is the degeneration of the brain and forgetting the diagnosis. This is a particularly troubling path for the sufferer and the one most are aware of and, though your concerns are understandable, applying the logic you currently do is almost certain to be impossible when the time arrives because you will inevitably look for that ‘sweet spot’ where you’ve gotten as much time and out of life as you can but to still have the awareness to say ‘tonight’s the night…’. You then become reliant on familial assistance to end it all but that’s almost impossible too as they’ll still see ‘pieces of the person they’ve always known’ and want to hang on to that as long as possible. That I will then take the sufferer past the point of compression and understanding to a point where they can no longer give consent for someone to ‘end it themselves’ as it were and, from that point, anyone assisting the sufferer at this point would be considered a murderer and no matter how much sympathy the courts have for the family member be irrelevant as there are few countries that turn a blind eye to any form of euthanasia let alone completemy turn a blind eye to it.
As someone from the outside looking in to the situation, it does paint a very clear picture of how family are affected by the illness, a same way you can never fully understand what’s going on in the head of the sufferer. People will make jokes about their poor memory but that’s not even a ripple in the water.
For example: I was involved in a serious car crash in 2016. I’ve no memory of the accident as the brain tries to find the best way to adjust to what happened, but in the last few years I’ve noticed my already poor memory has deteriorated in the last, possibly due to 2-3 years, possibly due to medication I’m on, possibly due to a brain injury, it could be completely unrelated to anything at all. I’m very aware of my poor memory but that’s all it is: a poor memory. There are days where someone will try talk to me about, say, a film we watched last week and I’ll have absolutely zero memory, not even a hint of it. It has become a running joke and that may mask the severity of what’s going on, it may just be that I’m gonna be this way, it might stop if and when I stop taking the pain relief that I personally think is responsible or it could be more serious. I just don’t know. This is where everything can take a potentially more sinister turn and the part that could tear your mental health apart if it’s allowed to do so and a sufferer of something like anxiety could genuinely worry themselves to death.
There’s unfortunately not only know which way things are going to go when the sufferer is first diagnosed but, as the illness progresses it does inevitably at one point just snowball out of control very quickly. Again, there’s no way to know how or when this will happen.
There’s some ways in which not only a sufferer but their family can work together to bring back some of the personality, if only briefly:
Music - music is one thing that really stimulates the brain. The memory area of the brain sparks into life as the patient is taken back to a time and place of the past. After listening to music (I couldnt say but this may only apply to people who loved to listen), the stimulated brain is sparking up all over the place and the patient often becomes more lucid, often for days at a time
Smell - Olfactory sensors in the brain ‘behind’ the memory section. This means the little electrical pulses being sent sent to the nose must pass through the olfactory sensors and this triggers memories.
These 2 tips can effectively bring the person almost back into focus but it doesn’t last and, sadly, so far there’s nothing to suggest these moments are of any benefit to the patient. Families receive a great sense of hope and patients are shown to have significant blasts of dopamine, adrenaline and so on. Their mood is improved, they become more relaxed, more at peace. There’s nothing to say beyond this there is any significant benefits to the patient and it can give false hopes to a family that their relation is recovering but this is because it’s a very new practice and studies are ongoing. The brain usually takes around 82 actions in order to build neurons, so the hope is that by stimulating the brain, on some level at least the brain deterioration begins to slow or stop, or even that as some of the less significant parts of the brain that die off, a more significant one can be rebuilt.
Sorry these posts are like writing novels and that I’ve repeated myself, but unfortunately that’s dementia. It’s a complicated illness that slowly kills off the brain l, fools family members by allowing lucidity to come in to play and headfuck the families. The illness progresses at different rates for different people and, worst of all, the sufferer becomes unable to describe what they’re going for.
My biggest concern, though, I’ll put on a different message as you’re probably sick of reading by now

    • @GioMarron
      @GioMarron 2 місяці тому

The worst part for me, though, is an art I touched on before:
One thing that we, as a species, do is trust our senses completely. You are looking at screen just now, reading this. You’re doing it on a phone of PC and you know which because you can see it, touch it, smell it, hell you can even taste it if you like. You trust every one of your senses even though you know they’re not always reliable.
Imagine, one day, you waken up and sit for a minute or 2. You sit up, look around and… wait, this isn’t your house! What the hell is going on. A woman you think is about your age walks into the room.
      “Ah, you’re awake” she says
      “I’ll get you some breakfast”
“I’ll get it myself” you reply “but thank you”
The woman turns to look at you, confused, concentrating on your face
She says something you don’t quite catch and ask her to repeat it. She tilts her head slightly, strokes your cheek and asks if you slept well.
“Like a baby” you reply and you see the same confused look “What about you?”
She stunts away “I’ll just go get your tablets”
“What do I need tablets for? I’m not taking tablets that I don’t need or what they do”
“I’ll be just a sec” and off she goes. It’s only as she’s leaving you realise she’s dressed as a nurse
“Why is she dressed like that?” You ask to yourself
Shit! You were going to ask her about the room. You stand up to go ask and suddenly almost lose your balance. The nurse comes in and flies at you, grabbing you.
“Come on, Mr Smith. You know better than to walk by yourself without your frame”
      You chuckle
“Cheeky besom. I’m only a year older than you. And I look better for it” you joke
Once again she looks at you confused again
“Right, here are your tablets and a glass of water. C’mon. Down the hatch”
“What are they for?” You ask
“Mr Wilson, I’m going to need you to take your pills before I can feed you. I’ve other people to see after you, you know, and the bathing team will be along in 40 mins”
      “Who ARE you?” You demand
After a confusing set of back and forths, the woman in the nurses outfit leaves angrily after she made you breakfast
Half an hour later, there’s a knock at your door, and a guy and woman walk straight in
“Hi, Mr Smith. Bathing team” the guy says
“A what? Sorry, why did you just barge in here” you demand
“Mary will go start running your bath. Is that ok?” The guy asks. He’s tall and you think about 8 years younger than you “and I’ll get you out of these clothes and find you some fresh ones”
“Sorry, what the hell are you doing?”
You feel yourself getting so angry at these people bathing in and you start yelling until the guy and woman leave.
You sit angry and frustrated in your chair, flicking the tv channels.
The door swings open and an attractive young woman comes in the door
‘My wife’ you think to yourself. ‘She’ll help with what’s going on’
“Kerrie! I am so glad to see you! See the day I’ve had…”
“Ok, ok, ok! You need to slow down. What’s wrong? Heard you weren’t happy today”
“You wouldn’t believe the people that were in here today. Every one of them just walking in, track mud over our carpet…”
“Yes, they’re here to help you. I put it on your whiteboard”
      “Whiteboard?”
“Yes, whiteboard. We put notes on it so we don’t forget anything”
“Why would I forget?” you ask “I’m not some daft old coot” is the reply.
Your lovely wife, Kerrie, looks at you with her big pretty eyes and you suddenly feel soothed
“No-one is saying you are, dad, it’s just.. you know how you get sometimes”
“Look, Kerrie, I know I get a bit forgetful but I’m not stupid”
You see a sadness hit this beautiful young woman in front of you
      “Dad…” she says
Your heart instantly begins to shattered
“Mum… Kerrie, died 5 years ago”
Sadly, this is a conversation that happens way more than you’d think. In my job, I was told that I couldn’t lie to someone with dementia who asked about their wife. I’m supposed to look them in the face and say ‘your husband/wife of 50 years is dead’ and that person has to be ok with it because it was 3 years ago
I never told them because every time you tell someone something like that, their husband or their wife has just died, right there in front of them every time they’re told. It’s just cruel
Anyway, once again I’m sorry about the length but it’s not a conversation you can have over a few words. The whole illness is a contradiction, unbelievable, painful, nuanced and it’s so so sad
Anyone who is dealing with dementia in their family or even for themselves, please get help with this. Carers often fail to realise how hard the work is, the time and effort, the physical and mental strain.
There are support systems out there. Please, please, please use them. They’re there to help and trust me when I say you need it more than you think: so many people think they’re coping, when in fact they’re just increasing the stress levels and that stress builds until it snaps

I myself have memory issues due to an autoimmune disease, and it's really difficult some days. I also have a photographic memory, so the days where I can't recall something as I used to is akin to starting to feel like you're going insane. What's funny is...i don't actually like walking simulators. You cannot do anything, change anything, or really lose until the game wants you to. You just walk around getting jump scared clicking objects until you find the right one. Then your character either turns out to be a murderer and dies, or turns out to 😊be innocent--and dies anyway.
Yet...i have never failed to tune in when you play one. Usually hunched over my phone in my bed. And then I refuse to look into mirrors for three days. XD Good times.
