    Thank you. You literally saved my life. I used to wake up tired even after using cpap because of the stupid auto cpap. Now I set my own pressure range of 14 and it stays there for my entire sleep. Now I wake up fine. Love from India. 🇮🇳😊

      @@Freecpapadvice yes, I am not even kidding when I said you saved my life. Even the doctors are stupid to recommend the auto ramp feature! Lack of sleep leads to many problems, literally it can life threatening, either you die while driving or stroke, heart attack etc.

      @@louiscollinge1106 Yes you can, just press the home button and the other one at the same time for like 3 seconds. It'll open up the clinical menu.

    Thanks Jason! With your help, I seemed to have dialed in my pressure to the exact pressure that works for me. I'm on 9.6 cmH2O. I remember you saying something about 10 cmH2O can be kind of the average for most. I am on CPAP mode and my AHI was .74 last night. I was able to go up little by little in order to tolerate the higher pressure (I couldn't sleep with higher than 4 at one point). For me it was worth sticking with it and not giving up, but I had to really look into your videos and be really proactive. It's hard work, but I'm hoping for the pay off. If I had to do this with my prescriber, I don't think I could've gotten to this point because I had to go up so gradually to get used to the pressure. Thanks again!

    Just as an alternate viewpoint....Mine was set to 4 because I didn't think I could get used to a CPAP mask plus any significant pressure at all. He set it at 4, knowing that was not therapeutic, and once I got used to the whole idea, he upped the pressure to my needed values. It was very helpful to me to be able to sleep with the mask on at a low value so I could get "mask used." Then the increased pressure was added. I don't think I would have tolerated both the initial mask and the increased pressure all at the same time for the first week. That titration pressure made all the difference to me.     I hope we will be back to old gold days. In these corona viruses days i had time to check your videos. As The Great Gatsby said you made a wonderful videos to save lifes.
    Here is my history; ( 5 months ago)
    In my childhood, teenage and 20 s i was always a fit guy. After 30s Unfortunatelly i was a heavy alcohol drinker and my BMI was really on high side as obese. I was 1.94 kg and 125 kg . My neck thickness was really high and it was normal that i had a sleep apnea.
    I had to go to the doc because of the breathless waking ups at the middle of the night.
    My doc said me that 14 pressure is great after she checked my sleep study and gave me CPAP but i prefered APAP. Because we can adjust the settings as CPAP in APAP with the flexibility between the pressures.
    After that i prefered Respirox G1 because our government pays 210$ for all the CPAP , APAP machines. This product price is 250$ in my country. The company adjusted my pressure as 5-15 pressure as the docs permission.
    My journey started and my AHI scores always were below 5. But i felt always terrible. I began to search on the net if something that can help me on it. I found that '' Soft Cervical Collar'' may help about it. But i bought ''neck collar'' lol and it made my pressure increase during sleep and made me worse. I threw it away.
    I am side sleeper and mouth breather. I also went to
    otolaryngology professor if i can fix this problem with surgery. He checked me and told me that you don't need a surgery. You need to decrease your focking body fat as a normal BMI person. Go to a nutritionist. Than he said ' Please come to me again after you will make another sleep study'. I decreased my body fat and paid 150 $ to sleep study again. BOOOM my pressure settings were down to 6 with CPAP.(By the way right now In 5-15 pressures my AHI scores were 0.4-0.9). My otolaryngology professor said me that if you will make soft palate radiofrequency, it will decrease your AHIs %50. I said i will keep in my mind.
    After today i watched this video, I will adjust my settings and give information .

      @@juliano8093 Interesting question! In my case I always get too many events with low pressures, and as many events PLUS huge mouth leak when I raise the pressure...! Confusing!

    My machine started on just that, 4 and 20. After two years I found a way of adjusting the pressure, so I'm on 8 and 15 at the moment.
    Still wait and see how that works out. Keep on truckin'

    I'm new to CPAP (got it on Halloween) and noticed a few things immediately. It felt like I was breathing in water so I popped it into manual mode and dialed back the humidity. No more feeling like I'm becoming a fish.
    The second thing I noticed was a lot of pressure in my ears in the morning. I'm a professional sound guy in the film industry so I'm constantly aware of my ears foe 12+ hrs a day and with my CPAP, I feel lile I'm having my Eustacian tubes blown out with compressed air at night. I drop off to sleep really fast but sometimes I wake up and feel the mask leaking a bit but the CPAP sounding like a jet engine blasting air while my eyes are dry. I've been looking for a solution to this because that pressure... ouch to my ears. I'm guessing your point of dialing back the pressure a bit would help. I usually average .1-.5 events per hour according to MyAir so I'm guessing I could play a bit with the pressure to avoid feeling so much pressure behind my ears. This makes sense, I guess, because when I have a better seal, I tend to wake up with more pressure behind my ears.
    The last thing I observed is that since getting the CPAP, I'm needing more sleep. I used to do great with 4-5 hours a night unless working overnight when I'd need 6-7. The CPAP is making be groggy and brain dead when I used to wake up alert. Maybe that's because I'm sleeping deeper but now I'm needing more sleep and am averaging around 6 hours per night now but still wake up groggy. Yikes.
    Thank you for your videos. My sleep clinic was no help with my second issue. They just said "would you rather sleep better or have no pressure behind your ears?" Not helpful. I'm hoping this info helps me dial in my settings so I don't feel like my ears are being damaged from extreme pressure.

    My doctor set my Air Sense 10 Autoset to 4-20 and I couldn't use it at all. 4 was to low it felt like I couldn't breathe and it would ramp up to 18 later in the night and that would wake me up with popped ears and gas in my stomach. I had enough. I went into the set up menu and changed it to 7.0 to 9.5 and at that setting I could sleep and my AHI was at 0.0. Couldn't be a better setting for me. I don't understand why doctors do that. It may be the reason some stop using CPAP and just never tell their doctors. I could tell they didn't like me changing the settings, but when they saw how well it worked for me they left it alone. Not everyone should adjust their own settings, but at least talk to your doctor right away if it is too high or low. They need to fix it so one can use it, otherwise the benefits of therapy will never be known and all that one went through is for nothing.

      CMJ, I'm glad you were able to change your own pressure and it worked. I don't believe I can experiment with mine. I do believe if had run away pressure at times. If you correct the leak does the pressure settle back down to so called normal? This information on pressure points up the fact that comfortable well sealing mask is important.

      I am currently going through this, around 2:00 am the pressure is too high it wakes me up and I have to unhook the hose to release the pressure and then it resets, but you are right, wake up bloated. Have to talk to my doctor about this. Thanks for sharing, this helped.

    thank you for the videos! I wish I found you a few years ago. was mis diagnosed by a pulmonologist after my sleep studym got a dreamstation auto on my own (cheaper than using insurance) spent alot of time on a few cpap forums, using sleepyhead... finally got my pressure where I am happy and the numbers look good... 10 low and 14.5, maxing out at 13.5.. AHI below 3.5 average

    Thank you for this vid! I was started on cpap 8 yrs ago but kept swallowing air and that was too painful. That doc tried adjusting the settings but it didn't help so he said well, try an oral appliance. Of course, I couldn't afford that so I went 5 years with nothing til I couldn't take it anymore and went to a different sleep clinic that put me on bi level and that was great (no air swallowing). But I went a few weeks ago to get a new study done since I've gained weight and this doc was adamant about using apap. It works...sort of, but I find myself waking up alot gasping for air and am tired during the day. I go to follow up in a few weeks but your video was so helpful to understanding apap and what's going on. Thanks!

    thanks so much for this! Just saw this video for the first time today. I was diagnosed 4/2015 and was set 5-20 and just left there. My AHI's were 3-4, but I was still tired all the time. A few months later I hacked my machine and changed it to 9-20. Felt so much better and my AHI's now hover around 1 for the most part. I was worried I did the wrong thing but just went to to have my machine checked and the sleep tech ran a report and he said things looked great. I admitted I changed it and he said "well, it looks like you did the right thing." I have had sleepyhead for a while and can see why optimal pressure looks to be around 11, so thinking of changing it to 9-14. I didn't realize the max so high could be a problem too. Make sense?

    I just recieved a ResMed AirTouch F20. It has a foam ring sea. They call it memoryfoam. Almost no leaks - no nose sore. I had been using the Amara View. It doesn't go over the nose. It just seals your mouth and has a flap that goes under your nose. It works well - and seals fairly well. The thing that I don't like is the placement of the exhaust vents. The vents seem to blow too much air out and seems to blow at my eyes (annoying and noisy) Additionally - the ResMed Airtouch F20 seems like it is at a higher pressure due to the differences in the venting.

      I had the issue of air in my eyes, and modified my mask with a plastic shield duct taped over the defuser. I keeps the air out of my eyes, and is very quiet as well.

      Are you saying that you are having issues with the Airtouch F20 blowing into your eyes or the Amara View? The F20 has a vent port on the insert that connects the hose to the mask which intentionally emits expired air so that you do not re-breathe CO2. This feature is on every mask, but they all have different locations and sizes. If you are experiencing air blowing into your eyes and it isn't coming from this port, you have mask leak from sizing or fitting of the mask, and it can be resolved by getting a mask that fits you correctly and adjusting it.

    Im 30 was was diagnosed with mild apnea. They set me up with a ResMed AutoSet 10. The range they started me with was 5 to 15. 12 wakes me up and plugs up my nasal cavity and it burns. Even though my AHI got down to 0.5. I will be bringing it down to 10. Hopefully thats more comfortable because since starting, for a month so far my sleep has gotten worse.

    Hey Buddy, I changed the pressure, like you suggested, however, I can only get, 3.5 to maybe 5 hours of good sleep, oh yes, I have a brand new Resmed 10, and I take 200mgs of Trazadone, to be able to get the little bit of sleep that I do. Thank you , terry

    I have mild to moderate sleep apnea and I'm just starting my therapy. I'm about 10 days into it. I'm using a Dreamstation Auto CPAP machine. The doctor's office has prescribed a pressure range of 4-16 cm H20. I understand what you are saying. But, my machine seems to be learning and changing the starting pressure. For the first few days, it started at 4. After a few days of use, when I turn it on, the starting pressure went up to 5, and now it's up to 6.

      If you have a modem attached to it, it's likely that your DME or physician is monitoring you and increasing it remotely. Or your machine is dicked up.....one of the two for sure. 😉
      Call your DME to find out for sure.

    Hello Jason....very good information...thank y ou! But one question. My mistaken understanding how an Auto Cpap works was that when there is an event the machine INSTANTLY increases pressure to deal with that event. But....now looking at your presentation, am I correct in understanding that the increase in pressure is not in relation to one event but to a number of events over a perios of time? I am wondering if other people are travelling under the same mistaken notion? Many thanks for your answer.

    My father o2 lowest o2 is 60 with cpap ....without cpap its 76 % how is it possible ....last night still my father o2 drops to 85 % with cpap....his pressure - 9.2 to 10.2 ....his average pressure is 10.1 ...after seeing your video i raise my father pressure from 9.2 to 9.8 he sudden very uncomfortable and remove mask suddenly ahi is rise ...then i again lower down 9.2 then he sleeps peacefully...what should i do i am so tensed please reply

    Is it legal to sell home sleep study equipment to consumers, so they can get an AHI on their own whenever they want (just as the APAPs can give them) and experiment with different sleeping positions before getting a machine? It looks far cheaper than an APAP. Interestingly, the attitude seems to be that the consumer is much too dumb to interpret the results, and must therefore get a prescription and enlist the aid of a paid professional for every single night they want a study, when the thousands who already use OSCAR for free every night with their APAPs would tell you differently. BTW, I've found the Withings Sleep mat's breathing disturbances feature matches the AHI fairly closely, although they're probably not allowed to advertise it as doing such.

    I have the Resmed Airsense 10 Apap. Pressure 9.2-11.2 so I must have a stupid range. Sleepyhead shows 95% of the time my pressure is 9.85 up to 10.84. Any suggestions. I don't use the ramp

      No. Sounds like it's dialed in pretty tight. Have you looked at your data on SleepyHead? That will really give you some information about whether or not that range is appropriate for you.

    I was put on 4-14 during my titration. The Resmed technicians said this is fine, but I did not feel better at all. Maybe the lower range should be set higher? I don't have the machine right now as my titration is over and I'm waiting for insurance company to certify my treatment and pay for the machine.

    Hi, how fast should the cpap machine respond for raising pressure from a lack of breathing? Does the machine let you go 30 seconds or more before it raises air pressure. Also is there a way to check mechanically (with out the card) to know if the machine is giving air when pressure when needed from not breathing? Thanks

    yep, that was initial setting. 4 low apparently lots of events, waking up often and pressure would increase thru night. Blowing mask off my face.. so waking up from that. In 1 1/2 weeks, I went from very fatigued to scary exhausted. Pulmonologist changed me to 7 constant, which is what overnight sleep clinic had found seemed to resolve symptoms. After now one week at that level, I still feel crappy. Back to Dr. next week. Any suggestions?

    I'm thinking I may need to address this with my provider.
    Just under 2 years ago I was diagnosed with mild apnea (didn't have the classic 'tired' symptoms, but had started snoring loudly all the time instead of just when on my back).
    I've been on CPAP (5-20cmH2O) since then. I did ask them once if it should be narrowed and they said treatment was working and numbers looked good, so no. I'm just wondering if it could/should be better though.
    My next yearly should be in April, but I can set something up earlier and ask them again, just thinking that the answer will likely be the same.
    Guess I'm wondering if I should change the settings myself and see what happens. My avg pressure is around 6 and my 95% pressure is just under 8 (I think it's rarely gone over 10).

    I use a wide open Auto cpap, and most nights my AHI is below 2, often below 1. Most nights my 90% pressure is around 8cm, exactly where my titration study placed me. I admit that according to my cpap I don't experience large mask leaks, and my pressure is on the lower end, and so maybe that is why it works for me.

    So my machine is a res med airsense 10 an it's set on auto 5 to 20 like you said. But when I go into the clinic menu it says my average it's at is 7.5. should I change the minimum setting to like 7? So I don't have to wait like you said for the machine to get to 7 maybe few hrs later?

      I'm not a sleep tech, but it seems you could go 6 - 10 with no problems.

      Internet Privacy Advocate my doc still rewrote my prescription as 4-10. I did take the ramp off since I start with a low pressure. My AHI is 1 or less so I guess just taking the ramp off did well. In one month he wants to review my data and we will take it from there

    I'm a total newbee...Haven't had a sleep study either. I'm sure I have apnea so I bought the Philips Respironics and several Dreamwear pieces. I've increased my pressure to 12 (just to see if it would help) and sure enough because I'm a side/belly sleeper I get woke up from the air blasting me outta bed. I thought I musta died or something, the way all that air was blowin on me!

      dude - see a sleep specialist or get a sleep study (from someone like Jason ) - trial and error is not good for your health.

    Well I got some problem also...I'm using CPAP auto, my all stats are not bad, like AHI 0.3......but everyday I still feel so tired, that it ruin my life....and other think, my o2 saturation still drops to 82%.... How its possible if I'm using cpap? My apnea was tested, they found after sleep test about 7. Using cpap pressure from 8.5 to 13.5, maybe some advise?>

    Hope you can help, I began adjusting my pressure and for about a month I was doing fine at 7cm. Then I started to have the feeling of suffocation ouch!
    More adjusting and now at 9.8cm however recently I'm waking up with my mask off. I was diagnosed at a university hospital in Osaka with severe sleep apnea in 2018 and living in Japan I'm pretty much on my own as I was placed on the typical 4 to 20 my doctor is out in left field when it comes to sleep apnea. I stopped therapy for a year and restarted about five months ago. I'm using the FP Vitera and ResMed S10. Love my mask and don't want to get discouraged again. HELP ME if you can.

      It’s the range that needs to be tightened up much closer to your optimal pressure.

    Great stuff and well explained. I'd understood the issue/need for having the lower end close to your ideal setting so it didnt take too long to build up to it, but never really considered the leaking issues with having the upper end too high
    So your example for someone best around 12cm gave the range of 10-14... If you are going to use the APAP is that 4cm window around the ideal pressure the best to aim for? or will that also vary from person to person?

    Does anyone know if CPAP or BiPAP is more common - Which IS ? I've had three sleep studies - almost a year apart each time. We're going to try a BiPAP - but my Dr. says my data is all over the place and its the hard to figure out. ( Thats helpful ? ... )

      CPAP is more common. Typically CPAP is tried first (because its simpler and cheaper.) Most insurance companies want documentation that CPAP does not work or that you cannot tolerate it before they will allow you to try BiPAP. BiPAP is typically reserved for patients who need settings that are higher than CPAP allows, patients who have difficulty adjusting to CPAP, or patients with restrictive lung disease of some sort. There's no shame in trying it, and some people find it much more comfortable. Hope it makes things easier!

    That makes fantastic sense :D thank you,now I've just got to persuade my sleep technicians the same when I go back next month :/ I have managed to find out how to 'hack' my cpap so I can change the settings but am too scared to as I'm being monitored,I do find that when it hits 20 it's far too high and I get leaks :( my average is 15. Also someone pointed out the other day maybe the reason for my long term insomnia is due to my apnoea as it's my body being too afraid to sleep as it knows it wont be getting enough oxygen ...makes sense ? suppose it's a bit like people crash dieting to lose weight only to gain it because the body thinks it's being starved so it stores more body fat. The body is a pretty clever thing and without it where would we be lol :D thanks for this advice but should I tamper with my machine ? it's set between 4-20 and I am still feeling drained the next day but I fight it all day and don't nap,I'm getting I reckon between 4-6 hours a night if I'm lucky as it takes me ages to drop off,although that is getting better. Oh I've been on cpap a fortnight now :D

      +shyannedianne1962 May want to try lowering the upper end of your pressure. May be runaway leaks. Also increase increase the lower pressure. You're struggling from 4cmH20 to 15cmH20 (your average) Should help. Or use straight CPAP.

      +TheLankyLefty27 so this week I confessed to my sleep clinic that I've been tweaking and they had an idea I had as my average has been 10 !! they're not happy as I'm still having aps so they wanted me to turn it up to 12 which I have so I'll see how I get on,I'm now set on 6-12 also got a new mask which has helped as it feels so comfortable

      Just a quick update, so like I said they were not too happy at me messing with my settings but agreed that I should turn it up to 12, then when I went back again they said I could cause myself a collapsed lung if I mess with the settings :O surely if that was to happen it would be if I was to let it go to the max of 20 ??? so we agreed I'd try it at 18 .. not a chance as I managed 2 hours and ended up throwing the mask off and going to sleep without so I'm back down to 6-13 this time, which I'm still leaking from the mouth so tonight I'm going to try some tape on my mouth as I've just watched your 'mouth taping' video lol I shall be cutting a little slit just in case lol oh I've been singing your praises on a facebook group I'm in called CPAP Therapy,here's the link if others want to join (hope you don't mind) facebook.com/groups/cpaptherapy/

    I watched this video and found it interesting that you said that 4cm doesn’t resolve apnea issues. I started with the big range but then adjusted my pressure lower and lower and now have it set to a range of 4-4.5.
    So, am I not getting effective treatment? My AHI is typically around 2 at this setting. It rarely goes above 3 and sometimes goes down to under 1. I am using an Airfit F10 mask and it is comfortable at this setting, I’m just curious what you think it is doing for me. I had two sleep studies done; one came back with an AHI if 11 and one was 22 but I have often questioned the validity of those numbers.

    Long time CPAP user (15 years) at setting 14. Just (1 week ago) got the APAP with 4-20. After 5 nights I feel like a zombie... It's like I am not using it. I went back to my old machine last night, and it was fine. Thanks for this video. I will see if I can get MD just to set me back to 14. If not there are vidoes out there to do it myself. Great information. THANKS!

    Jason a simple question. What are you supposed to feel when using a cpap. I hear comments saying the pressure is intolerable. I just started using cpap. The pressure is quite comfortable when going to bed. The only problem is that during the night I gasp for air. I feel like I'm not getting enough air. My ranges are 05-15. Should I be feeling the air current stronger. I already know how to change the pressure on my resmed air10. But I'm not sure to what level. I've woken up in the morning feeling chest pains nausea dizziness. Please help!

      Wrote a long message and it didn't take!!! Some people that have a deviated septum or nasal polyps have a sensation that isn't pleasant. Once corrected through, they find CPAP comfortable. In your case you need to use OSCAR to see what your typical pressure is throughout the night and increase your starting pressure to that level.

      I have. It is a great feature that would eliminate most of this problem. It still isn't perfect though since APAPs will increase the pressure too much with some people. So the next nights 90th % pressure *could* potentially be too high. Nothing will be perfect for most people though. Most people don't even think about it. They just put it on. Those that actively engage with their treatment would be better served by just monitoring it themselves and set the parameters more appropriately for them.

    An AH HA MOMENT! I get this now, I was starting to have some issues and have been waking up to massive air leaks and not good sleep; especially frustrating because I'm tired again too frequently. I was still on the 5/20 pressure, then went to 10/20 and seemed good most nights, (i also thought that when I was moved up from 5 minimum to the range of 10-20, if 10 is good, having the ability to go to 20, must be great!) Now I know why I am waking up to loud leaks, big wind in my eyes, etc. I thought it was the mask and no matter which one I use, even my most tolerated, the Res Med 20 ,was leaking about two to four hours into the night, I would wake up, get up, drink water, use restroom, etc., then try again...Thank you, this just might be the problem, definately makes sense!! Very much appreciate this info!!

      I have a newer SleepyHead video I just recorded yesterday and should be an easy(ish) edit. Hopefully I'll get it up today. Point being, if you're not using SleepyHead yet I would really suggest that you do. You can use it to isolate the upper and lower pressure thresholds where things "go bad" for you. You may even decide that a static pressure is best for you. I'm finding that when I do data analysis for people this is what we find. A static pressure just eliminates so many areas where problems can arise.

I am just now seeing this reply, lol, I am a little computer challenged and half the time I find things by accident, even though I have my notification settings to alert me to new videos, replies, etc. (Everything "computer" seems to work smoothly after at least ten tries and I'm good and frustrated, lol!) I did slightly change the pressure on my machine, (Dreamstation), to lessen the gap between the low and high pressure; already, I am starting to have better nights once again, very few leaks or other disturbances. I'm keeping a sleep journal to keep track of everything and help with fine tuning the numbers. As I go, it may be that a single pressure works; I was wondering if that was a possibility and you already answered that question which I cannot thank you enough for all the information! Just so valuable; nothing like getting a good nights sleep and having access to the tools and great information! Thank you so much for your time, very appreciated!
