In more modern or renovated structures, that all happens "behind the scenes," so to speak, and the water is ultimately piped through the same faucet -- but in older, unrenovated structures, it's not at all uncommon to have separate hot and cold taps because of this, or even a cold water tap and a whole GAS BOX with its own shower head-like nozzle for the hot water. My old apartment in Miyagi had the gas box, as did the kitchen in the school where I taught in Ibaraki, though my apartment in Ibaraki and my current apartment in Tokyo have a more modern single faucet setup. The fact that the hot water is piped in from a separate water reservoir, however, is also why it's generally recommended that if you're drinking tap water in Japan, or heating water in a kettle, you use COLD water exclusively, since the hot water likely has more minerals in it.

That other tap is used for filling a glass with water. You then add some iodine solution to it and gargle.
