Appley Animator I think he did a good job bc you can't really add anything truly "new" to the game. To be fair I know nothing about modding but it seems you can't exactly just add entirely new enemies and bosses or mechanics to the game that weren't hidden in the game files to begin with. Can't just add in enemies with brand new attacks and animations and special effects with hitboxes and such. At that point it would just be easier and more worth it to make your own game.
Yeah modding it's ridiculously complicated and convoluted for Dark Souls. It takes a lot of effort and testing and time spent bugfixing just doing one simple thing like moving a bonfire. I hated seeing all the people putting down the mod during the streams... It's free content that must have taken hundreds, if not thousands of hours to make. Criticism is fair enough in any medium, but some of the things poeple were saying, even when Grim was in the chat too, were just disgusting -_- On another note, custom maps are kinda possible now, but it's still in its infancy. Once the newest tools are fleshed out expect to see total converions and whole new worlds for DS1, DS3, DeS and even BB (nobody can be bothered making the tools work for DS2, the engine is too different and... well you know why)

Note 2: It seems to me that the Black Knights appear in the world in response to you killing Jareel? So you need to re-explore everywhere after that change in the world state. Lobos either hasn't considered this or doesn't believe it, at least not that I've seen so far. If I'm right, that's where his confusion might be coming from.
My guess would be that the locked chests are like any other interactive objects in Souls, and there's a capacity on the programming end to put a requirement on each of those objects, like earlier when lobos needed stone gauntlets to pull a lever near the gargoyles. So you just put a requirement on the object to possess a particular item, or have something equipped, and there it is. Little surprised there wasn't an actual mimic boss though. He should've turned into another boss as a red phantom, then a third one, with an explosion at each death (each has a new HP Bar). And yes, completely RNG, just a random boss you've already beaten. Some people get Jareel and then Nito, some people get Pinwheel and Taurus Demon, who knows!
You're pretty much correct. Object interactions are controlled by object activations, which dictates how you can interact with things like doors, levers, chests, fog walls, etc, and what happens as a result of that. All object activations can have certain requirements for activation, such as having an item or having a particular special effect applied to your character. For example, one way doors work because you require a certain special effect on your character to be able to open the door, and that effect is applied by a box region on one side of the door. I believe that's what is happening with the heavy gauntlets needed to pull the lever, though I can't sure. Certain gauntlets in the game provide a special effect that is required for the object activation to succeed.
Seems like the modder intended the catacombs to be one of the first areas you visit, due to the easier enemy spawns and lack of skellies in the graveyard, huh. Wonder how this LP would have gone if you hadnt glitched to valley of drakes early lmao

It starts to lose it's touch after thousands of hours in Dark Souls 1 (I have about 1,150 hours, Lobos has over 10,000 hours).
Shiftinggers he's technically not blind, moss grew over his helmet. But he can't see still so the fuckin no scope is the best part of the whole game
