I love the Mumei & Gura dynamic. In rare cases during natural childbirth, a bit of cartilage which is meant to bend to make an opening won't bend enough. This leads to the baby getting stuck and possibly starting to suffocate. A scalpel would take far too long to cut through something as tough as cartilage to save the baby, and the mother would also likely die if the operation took too long. The lack of proper anesthetic at the time would also make an attempt with something as weak as a scalpel incredibly painful for the mother. For this reason, there was a hand-spun chainsaw only slightly larger than a scalpel capable of more quickly cutting through cartilage too free the baby and allow the birthing process to continue with minimal risk of casualty. While the cartilage wouldn't heal, the incision could mostly heal with proper treatment. C-sections did exist at the time, but they were a last resort since they were usually fatal.
