    Can not thank you enough for explaining things so well. Hell broke loose with my left ACI pattern.Went to many chiropractors and physiotherapists that didn't have a clue. You saved me and hopefully many others from the unthinkable suffering! Bless your heart

      @@NealHallinan I can't seem to find any PRI therapist in Hong Kong.For years I got all the different diagnosis and kept getting worse. I am seeing a physio therapist weekly that helps a bit. What you explained here is a whole new level though and thank you again. If you do online consultation please sign me up!

      @@nikkinghk online is possible, but it's much more effective seeing someone in-person. I don't know if travel is possible for you, but there are numerous PRI people in Japan and they all speak English. They are listed on the PRI website.

    Now it all makes sense. Just today an chiropractor advices me to stretch it. But I told him that it makes it worse and more painful. And this information get to me in the right moment. Thank you man. You are doing precious job.

    I thought it was tight so i stretched it so hard that i stopped feeling the muscle. It was a huge mistake and it felt that the blood flow is blocked from my right leg! But its weird how you feel that its so tight and think stretching is the solution. I am doing the 4 techniques and they are extremely relieving, you should do them instead of stretching.

      You were probably feeling "tension" created by your autonomic nervous system and interpretating it as "tightness".

    Bro my right glute is gone i wasnt feeling it at all cuz of pelvis, now i reset it with ur instructions omg i can barely lift my right leg cuz its so weak , all the other muscles were rotating instead od him ong thank you thank you thank you

    Neal, thanks for this very informative video. A physical therapist I was seeing recently said one of the reasons for the lower back pain on my right side is a tight piriformis, and he identified the fact that my right foot "toes outwards" when I walk as indicative of this (Note: I've never had any sciatic nerve pain). But I've found that stretching this muscle can inflame my low back pain; in fact, correcting my gait by making sure by right big toe is pointing forward inflame it as well. Is all of this consistent (including the right foot toeing out) with what you cite in this video, i.e. that 1) the right piriformis is over-stretched, not tight, and 2) it's over-stretched because it's doing the work that isn't being done by a weak right glute max? Thanks again.

      The right foot turning out is usually because of the tibia externally rotating underneath the internally rotated femur. So you aren't pronating on the right. It is all consistent with what I discuss.

    Everything you just described is exactly what I’ve been feeling for 3 years! So many doctors, PT’s and chiropractors and none have looked at my pain from this perspective. But it makes so much sense!!! Even after all of this time and treatment, I still have no official diagnosis. They’ve been back and forth on piriformis syndrome multiple times. Some doctors saying it’s definitely that and others saying that piriformis syndrome doesn’t even exist. And everyone is always saying to stretch! Even though I tell them it seems to make it worse. Thank you so much for sharing your knowledge!!! I’m going to get to work on some of the beginner exercises today. Is there anything you feel is particularly important for me to focus on right away? What glute max strengthening exercises do you recommend? Again thank you!!

      Hello Shannon. Any glute max strengthening has to be done via the PRI left sidelying right glute max exercise on my website, otherwise your pelvis won't be in the correct position to work the glute the way it needs to be worked.

    Wow! Thanks for all the insights. First info I've found that talked about the hips in such a clear systematic way.
    If I sit up with my feet out in front of me in bed, my right hamstring and butt will fall asleep. I've always had a strange discrepency between sides and never knew what to do about it. Does this sound like more of what you're showing here or something else?
    My left shoulder also wings and clicks and does strange things no how much I try to correct it. I figured the whole problem was a chain.

      Thanks for the comment, J.A. It's relatively common that people will place more weight on their right side in a seated position than the left side. It's pretty predictable because of the orientation of our pelvis to the right. Maybe that's why they fall asleep. So any video I make is based on the assumption that you have a pelvis that is oriented to the right and thus your center of mass is also more to the right than the left. It all goes back to internal organ placement and the fact that our right diaphragm is bigger and stronger than the left diaphragm.

    But left glute is also weak but not as kuch omg i can feel my right hip flexor releasing, right glute burning just from a few lifts

      @@ivoursic7056 jesam ali nema nikakve veze sa vezbanjem jer ga je nemoguce ispraviti vezbom jer se uvek vraca. Mora da se otkloni inflamacija u telu i onda ce telo samo opustiti misice koje je blokiralo. Radio sam water fasting sto duze i cesce to bolje, tek posle nekolilo dana ces osetiti upalu koju imas u telu i takodje bilo bi dobro da uradis liver i galbladder flush. Kucaj na yt Andreas Moritz liver i galbladder flush.

    Hi Neal, I see you respond to a lot of the comments so, hoping to get a response. I've been dealing with chronic pain for 7 or 8 years. I'm still active and only 33. My symptoms are as follows: My right glute (maybe piriformis) is often in pain especially sitting, left ql is always engaged, burning sensation in right quad, right leg externally rotated, overdeveloped tfl on right.
    Any direction would be greatly appreciated, gone through 5 physios and 2 chiros. Willing to pay for a consultation.

    thanks for the video, it explains a lot of things, I just have one question how do you correct a 20 years old pelvic tilt of right side causing it to have 5cm different.

      It's a neuromuscular issue. So if you put someone in the correct position that turns "off" the overactive muscles and turn "on" the under-active muscles, things self correct. But you have to know what you are doing to make it all happen. It's not easy.

    Ive been doing L AIC exercises for 4 days. Felt good really. When i got into a train or bus. I find it extremely hard to balance. Is it some muscles that aren't firing before are finally starting too?

    My left side pulls backward due to posterior pelvic tilt. My physical therapist is trying to strengthen my glutes and leg muscles. I do knee to chest piriformis stretch. Is this a good exercise?

    What about the roles of glute medius and minimus? Are they not even more responsible for abduction and ex ro of the hip as opposed to maximus which is primarily an extensor?

      the glute medius and minimus are certainly important as well, but more for "swing phase" of gait. In regards to piriformis syndrome, most people have a right leg that likes to internally rotate as compensation to a patterned pelvis above it. As such, the right glute weakens significantly in "stance phase"

      There is a right glute max technique on this page, but it's hard to do correctly without instruction. But it's not just a right sided issue. If your left side isn't stable (which it likely isn't even though it may feel fine) your brain won't let you "unload" your right side bc the left side isn't sensed as stable.

      Yes, they are all just very painful symptoms of an underlying pattern. The pain can occur anywhere.

    So will squats and other exercises that work the glutes help with piriformis syndrome once it is the pelvis has been corrected and placed in a neutral position.

      Not necessarily. In order to use each glute efficiently, the pelvis has to be "biased" or "oriented" into the proper direction. Squatting won't bias the pelvis in either direction. I would use PRI to get rid of the piriformis syndrome because only PRI addresses the underlying "pattern" that is causing it.

    Can you please help me put my case into some sense? I had everything you mentioned above: right piriformis syndrome, right glute weakness, pain often on and off in the whole right side of lower back, and sciatica-like numbness on the right leg, once in awhile left shoulder pain. I went through all types of therapies out there but no one could help. Only this video makes everything connected and comes to sense. However, my question is that my chiropractor took a x-ray of my hip and told me my right hip rotated slight forward, and my right SI joint was the issue (of course he doesn't look at me as a whole connected being like your mentality). How to understand this?

      @@NealHallinan I shared the left AIC pattern info with my chiro, but of course he doesn’t understand that. He said I have PI (I assume it is posterior Inferior) of right hip. I am still trying to understand everything to find the root cause of my right hip pain

    When you say left AIC left pelvis is forward does that mean the left innominant bone is rotated anteriorly?

      Possibly. In the left AIC pattern, generally coupled with the right BC pattern, the left arm will be limited in shoulder flexion, moreso than the right. So if you lay down supine (facing up), and bring your shoulders into flexion, the right arm/hand will usually come closer to the floor than the left (but not always).

    NEAL, i have exact same issues explained on your channel. left hip rotated, right ql very tight, right piriformis agitated (cannot sit for long - standing as i'm typing). do you have anything on leg length discrepancy? my left is 9 mm shorter than right? Are there forces that have moved the left or right femur?

      Leg length discrepancy can cause serious issues. There is a great deal of debate on how serious discrepancies are, based on how great the difference is between the two legs. Sometimes people are born with true discrepancies, and sometimes they are a result of the position of the pelvis (like in the situation of a left AIC patterned pelvis). A 9 mm difference is significant and I have no doubt that it can cause pain (well I know it can be because I also have a discrepancy of greater than 6 mm). I'd assume you'd need a full length orthotic (not just a heel lift) under your left foot to even out your pelvis. Then you'd have to do some PRI exercises to restore proper position, movement, and strength of the pelvis.

      I am a patient that has done PRI training for 7 years with a physical therapist. Also i have an anatomical shorter left leg by 4mm as confirmed by standing limb length xrays I use a full foot lift equal to my 4mm on my left foot and I have been able to balance my pelvis. I follow Neal’s videos to keep refreshed in PRI info in order to help other patients too. There was no way I was fully successful with the PRI until after I determined my leg length discrepancy. I am no longer tight in right QL and my spine has rehabbed wonderfully all the way up to neck and shoulders.

      It might not be a piriformis syndrome. Mabe central canal stenosis of some herniated lumbar discs which contributes fom bilateral compression of both sided nerve. Make an mri of lumbar spine to exclude that.
