    I'm 74. I've read about PSA testing for nearly 30 yrs, and I keep finding in the literature of different sources it is inconclusive. I recently had that test done again ordered by the NP and it came back a little above the commonly accepted limit--EXCEPT that she did not take into consideration that the limit advances with age too! For my age, it was on the low side. Now I had a biopsy done 4 yrs ago and the result was benign even though mine is significantly enlarged. The UroDoc said many men live out to their 90s with an enlarged prostate and never develop cancer. Mine told me I was highly unlikely to develop cancer before I die being it is a slow developing type.
    Now for the bad part. My biopsy just happened to take place 2 months into the COVID scare. This became a huge curve ball for the Uro business I was going to. They did not have the freedom to cope with complications. Previously the UroDoc said I could have either local anesthesia or be put under. He talked like a local wasn't too bad an experience--some reasonable discomfort from having instruments should up the tail pipe. I was led to believe that local injection of Lidocaine would numb everything sufficiently. Well first they have to inject you with what feels like a needle meant for a horse, and that was nasty enough. But Lidocaine doesn't do a complete job of numbing. 12 perforations of the intestine into the prostate even with a local is only for those men who are bull moose with pain. I was on the edge of shock by the 4th stab. At that point they were not going to put me under and kept going. It was all I could do to keep from passing out. The ordeal took everything out of me and it was 45 minutes after they finished before I felt strong enough to stand up. And its no fun seeing thick raw blood flowing out the peen. The biopsy is worth getting done, but never take a local. It's not worth any small advantage. And always, do your homework and know everything you can before allowing any doctor to bamboozle you into procedures you don't need.

      Thanks…I needed to hear this! I just took the 4K score test…I’m at 13.5 intermediate….however I definitely do the general anesthesia if I choose to do a prostate biopsy.

      Dang sorry to read your nightmare. My older brothers experience no pain, just full bladder to get the laser treatments, small painless burning/killing of the cancer. Good Luck sir

      Same experience with biopsy 15 years ago. Worst experience ever!
      Plus caused additional problems afterwards like prostatitis, way lower ejaculate and issues peeing for several years.

    Two of my older brothers had prostrate cancer, younger bro had stage 4. After laser treatments he is 100% ok. Oldest brother is fine now also. Luckily!

      I know this probably sounds a bit inappropriate and please don’t take offence but just to support what was said in the video did you know if they mast or had sex regularly ? Because if they did the it would kind of disprove what was stated in the video about mast preventing prostrate issues ? Cheers

    "Ejaculating more than 21 times per month showed decreased risk... As per studies.
    Study didn't delve into type of sdxual activity that preceded the ejaculation.
    But basic understanding is cleaning out of the tubes may be associated with a reduced risk
    Basically follow safe sexual practices "

    I had prostate cancer, and the doctor said it was genetically induced. I was ejaculating several times a day, and in my case, it didn't matter. That's why I say get a blood test at least yearly to screen for prostate cancer. Trust me. I found it early and had a prostatectomy, recovered, and enjoyed life after all that. No problem with sex.

    My father was diagnosed with PC in the early 1990s and he reassured me that it was not a genetic threat to me. I was skeptical and of course I now have PC since 2022. He underwent radiation therapy and it was no fun for him and eventually he died from leukemia in 1997. My belief is there are so many harmful chemicals in our diet and drinking water that cancer is a much bigger threat to us than earlier generations, including micro and nano plastics. Yikes.

    I'm always amazed that people freak out over a digital prostate exam. It doesn't hurt at all and takes like 5 seconds.

      Because there are people with anxieties, ADHD, and Autism that can make it far more than a routine exam. Also lots of people with trauma getting triggered with this exam.

    My brother was just diagnosed with metastatic prostate cancer at age 58. After urinating blood and some blood clots he saw his urologist. His PSA was 55. After a cystoscopy his urologist said it didn’t look good. Insurance refused to pay for a PET scan. They did some other type of scan where they could see his bones and it has metastasized to his pelvis, hips, spine, shoulders, clavicle, and scapula. He’s waiting to have a biopsy to find out the type of cancer and aggressiveness. Why would insurance refuse the PET scan? Isn’t that the only way they’ll know if it has spread to his organs?
    Update: insurance has now approved the PET scan since his lower cost scan (still don’t know what that was) of his bones showed a lot of locations of cancer sites.

      @@ChessPuzzles2 he had a scan that showed where it was in his bones only. I’m not sure the type of scan that was. He’s getting a biopsy in a few days and will talk with oncologist more then to ask about his organs and lungs.

      Why deny a PET scan? Only one reason… MONEY. And if it has already spread, there is very little utility to a more expensive PET scan.

      Wow sorry to hear that . Mri can also seee. So he must have not been geting yearly physicals. Have you had your psa checked, you could be at risk. Get a physical asap

    If they can show the details of a baby's face on a ultrasound, why can't they use ultrasound to get a more complete image of the prostate?

      Cause the flow of billions of dollars for research would stop. I sometimes think there are cures for cancer and other diseases, but they don't put it out there cause billions of dollars would stop for research.

      Likely differentiability between the medium and the object being observed. Much easier to pick out the bone and cartilage in a face within a liquid medium than one soft tissue among many others

      MRI with contrast is already an established procedure.

      There are those types of MRI machines that can do that. But the exam-screenings is very expensive and insurances have to pre-approve it in most cases.

      The topic was about ultrasound being insufficient for the application, thus MRI was needed. The original question was about why ultrasound was insufficient if prenatal ultrasound could be so detailed. I don't understand why we're talking about MRI given that context...

    I enjoyed this video. I do not have cancer but I do know the fear of biopsies and the so called (standard course of care) with treatments and tests that are not really necessary. One topic that this podcast did not discuss is that most if not all insurance companies will not give you an MRI until you have a biopsy first. I paid out of pocket for my MRI. Nothing major was there and it is worth the expense. My last urologist was so high strung on biopsies, he didn't think the MRI was accurate. I found a new doctor.

      Watch out for the cancer industry, its big bucks for the surgeons, the radiation and chemo treatments. Its just as bad as the virus and vaccine industry, both are fake.

    at 71 had my lower grade localized prostate cancer treated with a HIFU focal treatment at Sloan Kettering NYC. . fortunate to have had early screening and catching it early. prostate MRI is very important. Back on active monitoring..

      Did HIFU work well for you? I am looking for a specialist for FLA or HIFU. Did you have to travel to NY or was NYU local? I was just diagnosed in Dec 2023, with G6 & G7.

      @@db_carguy4833 i suggest you go to Mayo clinic Jacksonville , i started out with them ,they do focal treatments there they will be able to do accurate diagnostics on you and suggest your best options. i went to sloan because i am in NY. as well. the HIFU was not a big deal, other than being catheterized for a week after. its only been 3 months since my proceedure future testing will tell how effective it was ,but i feel great and i am very confident in my decision and my doctors. i had to stop going to Mayo due to an unsurance problem ,but i am sure they are just as good as sloan. please feel free to contact me if i can help.

      @@db_carguy4833 i suggest you contact Mayo clinic Jacksonville, i started with them, they do focal treatments and they will be able to diagnose you accurately and present you with the very best treatment options. i am only 3 months from my procedure so future testing will tell how effective it was , but i am very confident in my decision and doctors at Sloan, please feel free to contact me . most importantly get associated with a good CANCER hospital. Mayo is top notch as well.

    Prostate cancer runs in my husband's family so we try to stay informed. So far, frequent ejaculation is encouraged and our Dr has coached us on digital exams (I'm an RN, MSN) so that I can check him between annual visits. Obviously my skills aren't equal to our Dr's but if there's any change in size or texture I'll feel it and procede accordingly.

    I had a very high resolution MRI of my prostate in a high profile W Coast hospital about 3yrs ago. I believe this MRI was featured on Dr Loeb’s Dr Radio program and.fortunately I showed negative for cancer. Plus I received a San Diego developed genetic test for prostate cancer with a result showing very low risk for prostate cancer. I fortunately have every day old man BPH

    "21 time a month". Try easily double that. Funny thing is, when I was seeing my urologist for prostatitis she knew my number and still said "do it more, the emptier it is the better". She even had it on a print out and underlined it while saying that.

    I started a low-carbs diet (no more than 2 carbs per meal and usually those with low sugar index like broccoli, carrots, and cauliflower) and it has been lowering my PSA and other risks. I do not eat processed meats, but I do eat meat (beef, chicken, goat, pork, turkey, and occasionally fish) and have been having good results. Since I went low-carbs, I lost 25 pounds. I also do not consume dairy products. I do eat chicken eggs.

      Yeah, I don't need to get nutrition advice from a urologist who obviously hasn't kept up with the recent research and have been influenced by vegan propaganda. I came here to listen to her knowledge of cancer.

    I learnt so many incredibly important facts about the prostrate and prostrate cancer from this segment. No surprise that my favorite foods can increase my risk of cancer. I’ve already taken onboard a healthy diet and life style is fundamental to a healthy life. However it would have been a happy dream if bacon wasn’t on the bad list 😂. I’m 65, screening since early 50s with a PSA gradually increasing with my age. So I was very comforted to learn that an MRI and not a biopsy is the most likely first step in investigating a jump in the PSA. Thank you for such an informative interview/discussion.

      I have a PSA of 7.5 and was rising over the past 6 years. I’m also 65 and recently I had a biopsy procedure. I wish I had informed myself earlier to have an MRI done first as the biopsy could’ve been focused on the location of the questionable lesions. Still, I’m going to another urologist to have an MRI done as my current urologist frowned upon having an MRI and instead recommended to monitor it and another biopsy in 1 year. Perhaps the younger urologist recommend an MRI first as my urologist is a doctor in his 70s. The 12 biopsies couldn’t have discovered the greater area of the prostate. Fortunately I have a Gleason 6 score which is a low risk cancer but I want to be sure that any other cancer lesions are found thru an MRI. I wish you well!

    59 old, my psa was 8.3, rectum checked - cleared, done MRI and results was 4/5 - high risks with few lesions. Proceed to fusion biopsy, result was just enlargement and inflammation. After all the anxiety, thank god.

    Many years ago I read an article stating that ejaculation reduced prostate cancer. In fact, I seem to remember it stated masturbation was good for this reason. The origin of the article was from New Zealand researchers.

      In general I feel doctors in the United States are more interested in seeing as many patients in one day as they possibly can. Also I feel they are good sales people for big pharmaceutical companies. Last I think many are clueless about treating prostate problems. I’m one to testify.

      @@Jack-2day I saw three urologist before seeing a fourth one who I had to almost beg to perform a cystoscopy to diagnose my problem. Well he did this to only discover I have an enlarged prostate. Which is why I pee and ejaculate with little force. Now why didn’t the first idiot do this?

    I m 70 now. 8 years back I was diagnosed with prostate enlargrment after scan. Family doctor referred me to a renowned Urologist who put me on medication and advised surgery within 3 months. He told other wise a emergency may come. To avoid surgery I changed to Ayurvedic treatment and fully under control. Though I was advised vrgrterian food, occasionally I take nonveg

    There's data on dairy and prostate cancer, as well as the meats. That got my attention as my father and uncle had prostate cancer. I've been vegan for 11 years and have 0.45 PSA at age 71!

      I’m 65 and went whole plant based a few years ago because a friend improved his time to run 100 km by 1.5 hrs by going vegan without extra training. I don’t know any carnivore ultra runners.

    I was told by a doctor that ejaculation raises your PSA . If that is so, how can 21 times a month be healthy.

    Soy has the most glyphosate? It’s not simple as eating more plants if their laced in carcinogens. I don’t think it’s as simple as no meat all plants.

    It's weird that there are also actual family doctors on UA-cam who wholeheartedly recommend a meat based diet because it's less processed, better for blood sugar balance, etc.
    So what's ultimately the best diet now? Does it depend if you want to rather care for your prostate or diabetes?

    Yes! frequent daily ejaculation can lower prostate cancer risk. I've been practicing this for years, and yep... no prostate cancer!

      I am 76 yr.old i frequent have an ejaculation. In short am sexually active without the help of sex enhancer. I noticed that in the month of oct.2023 i usually urinate about 6 t0 8 times a night. Nov.dec.jan, i began to doubts what is happening to my frequent urination. Feb.20 i had my biopsy, later nuclear bone scan because my psa is 121. I was given an infusion, then iinjection. Am under observation. Could i do sex with a partnet?

    Thank you for a very informative video. My PSA went up from 6.4 to 10.3 over a 3 month period. One issue is I have severe heart disease and have been on Plavix for 20 years. To do a biopsy I'm suppose to stop taking Plavix for seven days but my cardiologist is pretty adamant about not stopping the Plavix so a prostate MRI would seem to be a better option. I'll look into it.

    What factor does injury to the perineum and prostate play in a high PSA? I went from a PSA of 2.5 to 5, and then I injured the area during a bike ride, and PSA zoomed to 28.5! The injury may have led to a UTI, but a urine test was negative. Within 2 weeks PSA was down to 13, then 9 ten days later. Subsequent DRE had no indicators. Next is waiting a month and doing a new PSA and staying off a bike. Thoughts?

    a lot is Genetics & i say this because my wife´s uncle had prostate cancer & his son got it which sadly killed him because it wasn´t treated in time.
    He complained for a while that he had back pains & the Doctors never realised it was his prostate.
    My father in law who´s my wife´s uncles brother also had prostate cancer as did their father. so there´s no doubt about it being genetic.

      Genetics only in so far that certain genetic types are more sensitive to all this crap they are putting into the food, water, air. I am old enough to remember a time where all these things were quite rare. It was usually very old people, 70+. Don't hit me with "statistics" that have most likely been doctored. This is a modern era phenomenon.

    Hello Dr Malik I do enjoy this podcast I'm a prostate surgery survivor 14 yrs now but my way of life down the tubes with regrets from radical surgery. On if I knew what I know now I would have waited before making this decision 4 weeks after I was diagnosed with Gleason 7 cancer.

    Stop to very little SUGAR! Made a big difference for me. No longer on meds.
    1. I take Saw Palmetto, Zinc.
    2. Aggressively reduced sugar. Made the most difference.
    3. Lite exercise
    4. Ejaculate (almost) everyday.

      if you get a yearly physical and blood drawn you will know. a PSA under 2 is good. My urloogist has a 41 year old with it as weil as I PSA 9
