That might be precisely what happened. Rakan's ult during that time has no initial cast delay as we have today. When he press ult that time, the ult activates instantly. Meiko could have seen the Rakan, but even then there is no ult YET. So when they saw the Twitch out of invis, Rakan REW'ed in the fight instantly locking Janna out of the fight, then the Silence of Cho and Shockwave of Ori finished their hopes.
