-A: one-liner: DDx (creschendo): "56yomale w./ PMHx of XXX presents with HPI; MAY be X, but we should also consider Y; or even say something doesn't make sense
-Plan: specifically: patient has agreed to ... drink x 3x/ week, go to gym 4x month - very specific
