  • Educational video describing the condition known as snapping scapula syndrome and its treatment methods.
    What is snapping scapula syndrome?
    Snapping scapula syndrome is a condition causing painful grinding, clicking or snapping of the shoulder blade.
    Snapping scapula occurs at the scapulothoracic joint and is different from other shoulder related problems.
    Over 1/3 of the shoulder movement occurs between the scapula and the thoracic cage.
    The shoulder blade serves as an anchor for the subscapularis and serratus anterior muscles. These muscles as well as the bursa provide for smooth movement of the shoulder.
    With muscle weakness or inactivity, the scapula may rub on the rib bones during movement.
    Snapping scapula can be caused by repetitive motions that lead to inflammation of the bursa or atrophy of the muscles under the scapula.
    When the scapula cannot easily glide along the chest wall, the bones rub together and produce characteristic noise sensation, painful clicking and grinding.
    Snapping scapula can be a secondary symptom to shoulder instability, such as dislocation or nerve disorders which may lead to atrophy and weakened muscles.
    The patient may develop scarring and fibrosis of the bursa, as well as painful scapular crepitus when moving the shoulder blade.
    Common causes of snapping scapula syndrome:
    •Weakness, wasting or atrophy of the scapular stabilizing muscles.
    •Bursitis
    •Osteochondroma
    •Rib or scapular fractures.
    Differential diagnosis of snapping scapula syndrome
    •Snapping noise may originate from other areas of the shoulder such as
    •Biceps tendon subluxation: a condition where the biceps tendon subluxes out of its groove in the humeral head and slips forward casuing a snapping sensation.
    •Labral tear: condition causing pain often deep in the shoulder that can be associated with a clicking sound during movement of the shoulder.
    Treatment of snapping scapula includes:
    Physiotherapy to improve stability and coordination of the scapular muscles.
    Injection of the bursa
    •arm is usually held in a position of extension, internal rotatin and adduction with the patient lying prone.
    •Needle should be placed under the scapular and avoid puncturing the pleura
    •May need chest x-ray
    •Needle inserted into the scapulothoracic joint under fluoroscopy with the aid of scapular “Y” view x-ray.
    •The contour of the scapula projects as the letter Y on the x-ray
    Surgery is rare for debridment of the bursa or removal of bony lump. Debridment is done either open or arthroscopically.

    Thank you so much for putting this video online. I have gone for a year with doctors and chiropractors telling me there is nothing in that area of my body that could be causing me ongoing pain. This is exactly my symptoms and it is sad that it takes a patient looking online to figure it out. But it is very kind of you to take your time to put this on here to help us overcome our stupid physicians.

      Hi I have been looking for this . I have the same symptoms as this . 4 years at chiropractor doctors nobody cares . Please can you tell me if you had a shoulder ortherpedic or a back orthopaedic. My doctor now doesn’t no who to send me to . Please answer I’m so desperate.

    I've never been diagnosed but after seeing this, I am convinced this is what I have. Very good explaination. I've told dr. after dr. about this and PT after PT and nope, nothing. Exercises. I've asked for Xray of scapulae. Nope. I had rotator cuff surgery, acromion decompression and much more on my L shoulder.almost 4 yrs ago. My advice, if you don't have stable scapular then, do not have surgery for rtc shoulder. My RTC is torn again (both sides) and not because anything I did. I am just riddled with arthritis. Was a waste to do the surgery as I am worse than I was before the surgery. Also, sleeping on your side makes things so much worse but, if you have a sleep disorder and can't lie on your back...Doomed. I hope no one here has to go through what I do. And, I live in Florida where the medical/ortho care is deplorable. If you are female and over 50, they don't care and especially if you have other serious medical issues they won't touch you.

    Thank you very much for this video. I have been plagued/diagnosed with this for over a decade now and have never found something as good as this to explain what is going on. Having surgery (out of state) for this next month and hope the directly and non direct symptoms of this get better. When this flares up at its worse, I have headaches that make me want to just give up.

      Oh my days I have just clicked (no pun intended) that my pain is in my shoulder but the snap or cracking is at the back and lower down. Headaches galore are unreal. So happy I came across this video!!!!

    Been 2 weeks since I had the surgery and I can finally stand up straight again and my shoulder isn't popping grinding or clicking anyone struggling with this get an arthroscopic bursectomy it's likely that it will fix your problem

    I have been suffering from this problem for 4 years. First I started in the left shoulder blade and now my right hurts too. I have kyphoscoliosis and that has probably proved the problem

    What an exceptionally well produced, straight forward and informative video. I have this condition as well and for three months I have been seeing an exercise physiologist and employing some specific exercises to help alleviate the discomfort. I can certainly feel the benefit but I'm still experiencing considerable pain and find it hard to raise my head without a snapping sensation. Has anybody else found exercise helps?

      @@elsoudani4028 yes I actually I did stretches and weight exercises , I used to make my dad pull my arms and stretch a lot. Even when it would pain I would still pull and one day it just made a sleek sound in shoulder n den it vanished. Just like dat. I would suggest to do stretches and feel the kinda of pain u hav is it bones nerves muscle try to understand what part is in pain then u would be able to understand what kind of exercise to do

    This helped me so much! I couldn’t put a name to what I was feeling. I got a spinal fusion a few years ago and haven’t been very mobile with my shoulders. I now have the worst pain in my scapula. Hopefully I can do some strengthening exercises

    Seriously the best informative videos, I've spent thousands on pt, doctors & ultrasounds, with no one able to tell me what's wrong, thank you doctor so much, can anyone recommend any physical therapy exercises specifically for this condition?

      There are different types of exercise depends on which muscle target you are aiming for.. Im currently doing PT, im doing alot of twisting exercises also doing SIS to massage the weak muscles..

      I get popping all over my upper back, neck and shoulders when I breath in or raise my shoulders. When I pop mucus shoots up my throat and I have to constantly clear my throat all day everyday can anyone relate

    So I spent 8 months with scapula pain the last 4 months of the 8 with snapping scapula I went to 1 of the best sports doctors in Europe in my opinion within 10minutes he knew what I need and he said not many people will see this problem in the career of sports medicine, the first exercise I was told to do is the standing y wall slide with elastic bands, the 2nd exercise is the sitting hitch hiker with elastic bands the 3rd exercise is a side lying rotation with a 1kg dumbell if you find to hard to start with a bottle of water do 3sets of each 2days on 1 day of aim for 20 sets of each, these exercises will strengthen up the whole scapula and help it to glide perfectly again I'm only day 2 into my physio and the progress is absolutely amazing I know I will be back to perfect if anything Better then I once was as that area was under developed after months of dickheads giving me wrong diagnosis, finally!!!! I hope many read this with the same problem and I'm sure it will be as good as help to yous as so far it is to me, really connect the mind to the muscle on these exercises and get the form rite the y wall slide it's about palms correct and the stretch at the end, the hitch hiker is about Not activating the lat etc using the back of the shoulder and the did flying rotation keep the arm straight with the shoulder tuck the elbow in tight place a folded towel under the arm for support

      @@andreyt6966 good mate I had a irritated long thoracic nerve so the muscle went completely limp it will never be perfect but I'm not even close to the pain I once had, you just have to except the muscle will never be 100% but the exercises worked wonders I'm not in pain anymore but same time It let's me know my backs not 100% but no pain that's the main thing lad, focus primarily on external rotation workouts that's the key, it will strengthen up the rear delt which will help with a weak serratus anterior 👌👍

      karl blake im glad u feel better m8 my story seems very similar something to do with nerve and muscle for sure even tho my scapula is restored to normal position the pain still visits me.I agree on them external rotation exercises they were like a gamechanger for me,I really raped that shoulder a bit so it was sore next day lol but less that annoyin pain for sure have to work everyday lol

    I'm in so much pain exactly in that stop!! It's like a constant stabbing!!! It can get so bad that's depressing!! No doctor has been able to diagnose....I'm at my wits end!!!

      Wow this is like the twilight zone ! It's very clear all of us here have had a Condition either acute or chronic that while not common as other scapular/shoulder injuries... was able to be tracked down by the likes of ourselves. Likely most if not all of those searching for this info aren't even associated with the medical field🥴🤦🏽🤷🏾‍♂️... the culmination of the true morbidity and absolute life altering pain this injury has caused is emotionally hard to do and I'm not considered to be the most expressing type of guy...I served 10 yrs in the Army & played intermural sports my whole life.
      Whats worse is the symptoms do NOT truly surface until just outside of the statute of limitations🤔🤔🤔🤔🤨😓 when they are progressed far beyond just snapping scapula . I literally tend to sacrifice my future health for a taste of pain relief from this crucible of a syndrome, even if by way of knee buckling, conscioussness, interrupting, audible cracking and popping.
      What I assume to be the culprit of my agony is my ribs,
      (namely the ones that align with the c7 -TI vertebrae, it took me almost 3 1/2 months of researching feverishly to come upon this... breakups bc my girl thought I was coo coo, repossession from being unable to work etc.😓)
      impeding the natural course of travel for my scapula along the thoracic wall..this is also accompanied with nerve damage-like sensations along the serratus anterior (frontside).
      Upon palpating my ribs where I feel pressure on left when attempting to inhale. I winced as I felt areas of intense tenderness accompanied with a hard osseous buildup layered over the point where the ribs meet the spine and sternum respectively.
      Several years after undergoing a partial shoulder replacement which was frankly done too late to well "save my shoulder" I was 32 ...3-4 yrs of complaining to the VA medical center calling in agonizing pain imploring them for intervention 4yrs later they changed what was left of my humeral head that was ruled as being "Profoundly advanced end stage bone-on- bone osteoarthritis" "labrum is obliterated". 😖😔🤷🏾‍♂️
      I get the procedure done anticipating great imptovement of use of my shoulder and major pain relief...instead I come out of surgery with a marked pressure when breathing in on my surgical (left) side lung and sharp pressure from opposing location on my back or thoracic wall.
      Ever since that surgery every symptom got progressively worse and I began to experience neuropathy, plexopathies, and radiculopathy all along the very muscles that have been atrophying since surgery before long the same grinding I had before surgery was back but with a vengeance now it pops all along the serratus intercostal "process".
      I won't even begin to try and describe the pain and debilitation from all of this I get too emotionally involved and tend to rant into oblivion night after night my gf stares me down as I stay awake frantically researching the scapular thoracic acromioclavicular glenoid rib and glenohumeral anatomy determined to find the source of my anguish.
      Now apart from (now) obvious and outright nerve damage ,and the addition of never before had neuroma on my scapular thoracic notch (painful to the touch). This SSS is indeed the other half of my woes.
      If I didn't know any better this page has a wealth of litigation on it as we speak! At the very least this should this be brought to the medical boards attention for a procedural revision. At least as to not let the horrible pathologies associated play out and compound said patients ailments?
      It's too bad I just found this exactly 1 month outside of statute of limitation for legal recourse, although it took me the patient essentially until now to hunt this wild syndrome down smh and do my due diligence to understand the anatomy and innervations along with historical pathology etc..
      I truly just figured out what's wrong with me! Yet the surgeon responsible is likely somewhere riding with his top down feeling the breeze 😪😒

    This video is really helpful, I think wrong body posture also causes this, my husband sleep on left side he has this problem on his left scapula and I sleep on right side and I have this problem in my right scapula

    A great Video. I have severe snapping in right shoulder, moving across to the left one now, think it was done by a combination of an ice skating fall. and a lot of computer use. Had it for 2 years and its just got worse and worse. My Doctor didn't even look at it for a year. Told me it was all in my head. Been told now it's being cause by Cervical Radiculopathy at c5 c6 c7. It's torn my back to pieces in the mean time, But I think we getting somewhere now, finally. Good Old NHS England.. Thanks Again. Appreciated.

      I was wondering if you found any effective treatment. I’ve been diagnosed with impingement and possible slap tear but I don’t think my symptoms align exactly with it but have a surgery scheduled really soon!

      @@stefangeorg5435 had nerve conduction study in my back. This came back fine. Think surgery looking most likely fix now. It's strange. You can see something is not right. It's clearly visible. But doctors did MRI and other tests. Can't tell me what's damaged as yet.

    @kaleidoscopekay occasionally it can occur from a nerve disorder
    if you have no winging ,bad wasting of muscles and no loss of function then you do not have a nerve injury
    if you have snapping then you have bursitis

    YES OMg i know i have this every time i roll my right shoulder back its a super loud snapping noise like bones grinding popping noise and it hurts so bad ive had this for years and i was a personal trainer for 3 years so i must have gotten this through training ughhh thanks for the info i need to go to a specialist i hope they can fix it ans i have a bad back btw im only 24 yrs old so im already an old lady lol

    I suffered a traumatic fall over a decade ago when I slipped on foam at a party and suffered a right shoulder dislocation, since the fall my shoulder has popped out the socket more than 20 times, with each time resulting iin extreme agonising pain up and down the right arm. However after 9 years of dislocations I was operated on and had keyhole surgery on the right arm to join the rotator cuff to prevent further dislocations. I now suffer from extreme discomfort and skeleton pain in the left side of my upper back and left scapula. The pain located here is a result of my shoulders being unlevel with the left side bearing the brunt of the dislocations on the right arm causing severe upper back pain. I am attempting various exteriorises now to allievete and rectify this.

    Could scoliosis of the spine cause snapping scapula? If it does, would a bursectomy still work fo stop the snapping and pain?

    then if there ius a lump remove it ,if bursa do bursectomy ,ct and MRI may help in showing the problem

    After a surgery to relief the impingement on my right shoulder my scapula started clicking. I think this surgery might had caused extreme instability to this area and shoulder overall... I discovered months later I have Ehlers Danlos Syndrome which can explain why everything is so loose after decompression but I'm afraid I might need to have a new surgery

    I began having this after giving birth over a year ago. I still have this problem. I couldn't understand how I could crack my back at any place besides my spine. I get super stiff back and I have to stretch or pull my back a certain way in order to pop my back at the scapula and then I feel relief.

    I had surgery on my left shoulder to repair my torn labrum and for shoulder instability. now i have shoulder instability and snapping scapula on my arm arm which is really starting to become a sharp pain. i really dont want to have surgery on both shoulders, what should i do when i see the doctor? also what are some good exercises for shoulder instability and snapping scapula? how do you feel about push ups and pull ups etc. ?

      @@stefangeorg5435 I don't remember when I initially hurt it, that was like 12 years ago, but I get a severe pain like a burning sensation on the bottom inside corner of my shoulder blade when I do certain repetitive tasks. I worked retail and folding pants on a table was the most painful thing, I'd get through about a dozen pairs before it'd start to hurt and just got worse from there. If I have my elbow bent 90 degrees and move my arm left and right repeatedly it starts to "burn" in my shoulder. It also pops and clicks a lot.

    wow, thanks for the video, finally I understand what could cause my 2 discomforting clicking noises. Sure my scapula grinds a lot but also I get another noise bicept or lablar related, It happend only if I lower my exended arm closer to my ear

    i think it is coming more from the scapula area and not my shoulder. only happens though when i do a full forward overhead rotation of my arm.

    well i ended up actullay having problems with my rotator cuff and had inflamation in my shoulder area. i went to a doctor and got physical therapy and anti inflamatory meds. i have healed now but the problem was overuse of my shoulder. so first you should see a doctor but i would also check about shoulder injury. if you have problems raising it or moving it, could be a problem similar to mine. but based on your comment your problem is different. Hope you get better though but it might take time.

    Nabil Ebraheim I have this problem and have had it for many years now but would really like to start working on a remedy, I definitely know that it has something to do with my loose shoulder or subluxation because I have dislocated it a few times at the gym, my question is, is there some way I can strengthen my actual shoulder ligaments to realign the shoulder so it doesn't grate on my rib cage? I am certain it is due to the dislocations because I never had snapping scapular before that.

      @@stefangeorg5435 Yes, unfortunately I still have it. I have just learnt to live with it, I don't really do much gym work so it doesn't really bother me as much as it used to.

      @@stefangeorg5435 I am guessing it has something to do with the subluxation, which means that the shoulder has literally popped out and back in again losening the ligaments around the shoulder causing the shoulder to not sit quite right in a relaxed position. So when I do a shoulder roll I can feel the shoulder blade rub against my rib. I don't think it's related to any tight traps, etc because it starting abruptly straight after my shoulder popped out and in again losening the joint.

    This has been happening for 5+ years whenever I roll my shoulders back. But I would say my shoulder muscles are weak because I do archery so I don’t know what up with me.

    When surgery is needed, would part of the scapula also need to be removed, along with the bursectomy? It seems like the scapula would scrap more against the chest wall, with the bursa gone, since it helps with the smooth movement of the scapula.

    When one scapula has been snapping for awhile, and the other side starts to snap, even after being in therapy for a long period of time. What does this mean? There is no osteochondrots. Could nerve damage be going on?

    The video said that Snapping Scapula can be a secondary symptom from a nerve disorder, leading to atrophy and weakness in the muscles. I am active, kayaking, also excercising in water and doing my work at home. Since my other Scapula is starting to snap, I just thought something else was going on.

    I have this the last 7months it developed after I work up 1 morning with an extremely stiff lower back it last 3days and 7months later if developed snapping scapula don't ask how, is there hope for somebody with this it seems like a life long injury

    Omfg. I think this is what happening to my shoulder. I had 2 MRIs done, one about a year ago where they didn't find anything unusual, but I had one again yesterday and this is *exactly* the feeling and pain under certain load. It's hard to explain. Let's say I'm wearing ruckstack, if I try to lift it and put it on my back, or if I hold something high esp on my left hand which is heavy, it causes sometimes fairly sever pain. I just want to sort it out :(

    My snapping seemed to have been caused my a thoracic rib that was out of place and hypermobile rubbing against the scapula. It would snap each time with deep breaths. Stretching and massages only provided temporary relief. Amazingly I have lived with it for many years not knowing what it was. I used a door frame, facing perpendicularly, leaned in and pushed very hard, just to stretch, like a side push up. I used a lot of force and heard something pop back in place, completely inadvertently. So many years of discomfort went away. It was sore for a few days afterwards. If you suspect snapping scapula rule out a slipping hypermobile rib that is rubbing on the scapula.

    I have a major rotator cuff injury (can't lift arm more than 30 degrees in any direction), and have just found this vid which describes my scapular which is dislodged. All the symptoms are there, fractured ribs, wasted muscle and, I suspect, atrophy, following a car accident. I've been following physio vids on YT, some of which have made it worse, some better, over 2 years. Doc out of the question right now. Should I continue with exercise? Or let my condition continue to deteriorate?

    I think I have this. Has anyone looked at stem cell treatment / prp for this? Not sure if its viable or not? I had an bike accident 3 years ago which resulted in a shoulder labral tear. I had pain for a couple of years but it has diminished over time, however due to slight shoulder instability my shoulder is pulled fwd causing the snapping scapula syndrome ive had over the last year. It is low level but chronic pain in my trapezius area. Any advice based on peoples experience is greatly appreciated as it is really affecting my life badly. Thankyou!

      @@stefangeorg5435 Hi Stefan - The pain actually seemed to dissipate on its own after a couple of years of the pain. I did have a steroid injection in the area a couple of years ago and the pain never really came back after that. I continued exercises that help provide stabilisation to this area and I think it may be a good reason why the pain disappeared.

    I was told years ago I had a "Snapping Scapula."
    I then was told a few years later that I had a "Snapping Hip."
    I am now being told I probably have something called Ehlers-Danlos Syndrome which is a connective tissue issue (genetic). My mom and my sister also match the EDS-H3 form as do I.
    I wonder if the all the snapping in my life is part of EDS? I won't know until a I get official genetic testing next May, 2017 (made my appointment 13 months in advance).

      I currently get Botoxed and it keeps my ligaments in place for my snapping scapula but the hip thing - I get steroid injections but I have osteoporosis now and can't do that for much longer.
      I used to end up at ER clinics with my head in a spasm - talk about painful. I now have the meds to help control them and when it gets bad, I take them and then wait with tears in my eyes and keep waiting until the drugs kick in. It's awful.

    I have had this for almost 3 years now. Very painful and annoying ALWAYS uncomfortable and moving my neck and back. I have tried everything and nothing worked. Physical Therapy, dry needling, ART, chiropractor, Gratson, LASER, other lasers, deep tissue massage, shots, SURGERY, yoga, and more I am just screwed for life with this horrible condition. =( =(

    i have a question. i play tennis and i serve a lot with my right arm. lately i have heard a sort of thump or crack noise coming from my shoulder/upper back area when i rotate my shoulder and arm all the way around and a minor noise when i serve. it is not painful but a bit uncomfortable. i was wondering if this would be SSS. i have a bit of shoulder muscle problems as well. it could have something to do with nerves or tendons.

      I saw a local physician who said It's not a serious condition and should go away by time and did not prescribe any exercise or something. But actually It has been about two years and I got no fading of the clicking sound, not even a little improvement that I really now doubt this physician's words and have to go to another physician ...
