How Gap Certificate (Affidavit) is prepared/ drafted when taking admission in college etc

In Pakistan, when there is a gap exist of any student in studying either of one year or two or four years, he/she has to submit gap certificate stating therein the reason of such gap. Bhai mai ne 2017 mai matric kia ta pir 2020 mai 1st year k liye dakhla le liya leki mai ne inrollment form bharte wakt gap form nahi liya ta ab mai ne 1st year k peper de diye lekin enrollment na hone ki waja se mark sheet b ruki howi hai ab mai kia karo please gauid me
