Dr. Gupta, thank you for your service to the community. I had a coronary angiogram yesterday because of ST depression at 170 bpm (Stress Test; 55 yo male) and night chest pains lying down. The angiogram revealed no blockages but a myocardial bridge on the mid LAD (1/2 way down and 10 mm long). EDP was normal. I've asked PCP for pulmonary referral. Why lower anterior left pain only lying down at night/bedtime and a function of lung volume (inhale/exhale)??? Thanks for this video.
Experts at the Cleveland Clinic say that IFR, not FFR, is the best way to evaluate a myocardial bridge. They also advise against use of stents to treat a bridge.
Stents are no longer advised for MB because the compression of the artery damages them. Bypass is not advised for MB anymore unless there is obstructive disease, this is because of competitive flow issues. Medication and unroofing are the two treatment options. Do you assess through diastole? Also, do you carry out unroofing surgery in Yorkshire?
For me the same story: recent heartattack, only myocardial Bridging is found during CAK. A lang time of angina pectoris because of vasospasm. But now a heartattack. Thank you for the clear information. It helps a to understand it clearly.

Microvascular coronairy spasms are confermed by coronairy fynctiontest. With that test they measured the flow in the coronaiys and also behind the bridging. That was okay. The symptoms are severe. With medecation it is manageble. Daily stressors give symptoms. So, how am I doing.... so, so. How about you, what is your story.

I finally know the reason for the sharp and severe chest pains I get once or twice a year🙏🏼 I was diagnosed ten years ago, while doing a stress test and after a heart scan, that I have a mild case of myocardial bridge. But I was not told that this causes severe chest pains which I suddenly get while resting and which last for 1-2 minutes . The last time it happened was one year ago so my new cardiologist had me undergo a coronary angiography where he inserted a scope through my wrist to my heart and I was able to see my heart and the coronary arteries on a screen. The cardiologist confirmed that my arteries are all clear and there are no blockages in them and he also confirmed that I have mild MB. But he couldn’t explain why I had that very severe chest pain because he explained my heart seemed ok...finally he said that it could be a heart burn or gas!!! He just didn’t believe that the pain I felt was from my heart?! Thank you again for explaining MB so clearly🙏🏼🙏🏼😊
Same with my diagnosis of MB during a heart cath to rule out blockage (none and they were chomping at the bit to put in a stent!) ...just had a second opinion today and was told it is heart burn. It was apparent to me that he just wants to push statins on me and never mind the MB. I have a third opinion with the doctor who did the heart cath and I am now ready for that appointment with lots of questions after reading a very comprehensive update on Wiki about MB. Cardiologists aren't schooled on this condition and they brush it off!

Interesting video thank you. I've recently discovered via a CT angiography that I have a long myocardial course in my LAD. Could this possibly have the same effect as a bridge? I have a stent proximal to the course after a silent MI caused by a 90% blockage. I've always lived a healthy lifestyle and my other main arteries are in good order.
I never had chest pain problems till recently like (2-3 weeks) back im 29 years old, active and always played full sized soccer, I complained of chest pain then got telemetry, ultrasound of heart, blood tests and finally CT of the heart with contrast the doc said my heart is perfectly healthy and 0% calcium buildup and mentioned nothing else, but I saw in the report that I had myocardial bridging of the mLAD didn't say the size or whatever.. What's the chances my chest pain is caused by that? I also have GERD symptoms constant farting and burping after eating or after long time of not eating. I have numbness of my left arm deep that runs into near my elbow area.. Thanks for any help and take care everyone!
Good info. but never should a stent be administered on myocardial bridge. Why? Can become very fragile and literally fracture (stent) and now it becomes life threatening. LIMA after unroofing is the best of both worlds when significant plaque occurs. That is left interior mammory artery from chest. This method provides a bypass much superior to the legs as they are pristine and bigger around in circumference.
Stents are proven to make the problem worse. Why is not surgery the first option? The bypass needs to be such that no blood goes through the old vein artery. Un-roofing is the only option. What are the risks of un roofing?

Hello, everyone. I’ve an myocardial bridge which is 15mm and complete. I’ve an CT Angiography. They said there is no blockage in vein and no meaningful narrow between systole and diastole. After that I’ve done Nuclear stress test. Nuclear stress test resulted, there is no problem with blood flow to my heart. But I’m still having symptoms. I’ve pressure on my left shoulder and there is a numbness on my left hand. Sometimes I feel, numbness on left side of my jaw. In a day life, when I stand and slow-walking, I feel painful pressure on my chest, 4-4.5 over 10. In addition to that, I feel dizzy while I’m standing. I’m a little fatigue in a day, can’t find energy to do exercises. I’m living in Istanbul/Turkey. I got contacted with one of the best cardiologist in Turkey and his name is Afksendiyos Kalangos. I showed my reports and he said not to worry about my myocardial bridge because reports show that, I’m not feeling these thing because of MB. But why I’m having these symptoms?

My blood pressure is high, on medication and recently found out I have a mb. When they did a stress test yrs ago,I had an infarction...again they did a nuclear 1 and just said it was abnormal. I get alot of heart palpitations/arrhythmias and my cardiologist says my myocardial bridge is benign and to just take my beta blockers but couldn’t there be a better way ?!? Maybe uninvasive surgery ?
My fathers family is now on the third generation with myocardial bridges that were not/ are not benign. Massive MI fatal/ cabg of 4-5 vessels without bridge repair/ and- I’m the only one who actually had the surgery to repair the bridge. As young as a 24 yr old cousin has died on meds ;and the majority of the first generation events were all fatal in their 40’s or early 50’s. We would be the family to study as literature is not abundant for this specific problem with myocardial bridges.
