Director knows well and that's why all the other actors did fabulous job since no biased instructions were there. Only alam did a bad job because director can give a role to his niece but that won't make her a good actor and he won't do anything about it thanks to nepotism
The surgery didn't stopped her from giving fabulous expressions in the series amd it's music videos. She's not smiling because she don't want to. Tbh most of them have done small or big surgeries on their face. It's common in the industry
