True, that was really very mean of her and I really felt sorry for Rumpel in that scene. He was reluctant to accept the deal, perhaps even willing to die for a moment. But then he thought about his plan to return to Storybrook and surely there would be a chance to take revenge on Zelena. That's how I think...

But they’re are a lot people taking care of one patient, not just the nurse. So rumple would have someone coming into his room almost every half hour to an hour. Sometimes more often than that. But there are cameras in the halls of the units.
Galaxy Eyes she gives him the elixer so he can leave the hospital and get her happy ending. She knows that rumple doesn't break deals so she's confident that she will get what she wants
