When I had anorexia due to being poor, I had a similar problem as MJ. People would see me, and buy me junk food like McDonalds. Then they would sit with me saying "all you have to do is chew, and swallow. It's easy." I would eat a couple bites of a burger. Then immediately feel sick. I really wanted to eat. People would stare at me, whispering to their friends. I felt bad a stranger had brought me a meal, and I was having trouble eating it. So I would force myself to eat. When I would finish the person would leave. I would go to the women's restroom to vomit due to eating to much. A few minutes later I would always immediately feel extremely tired to the point I couldn't walk. So I would sleep anywhere, even in the middle of the footpath. Later when I wakeup my hands, and feet would be cold even on hot days. I would be trembling all over. My heart racing when trying to sit up. Only warm water or decaf green tea made me feel better. Only once I went to hospital when my heart felt like it was skipping beats, and my vision was extremely blurry.
Today I'm much more better. Due to past experiences I know it's best not to give anorexics/starving people high glycemic foods, and not give junk food. Plain vegetables in a variety of colours are best. I also let the person eat the food when they want to.

No kidding. I really feel sorry for her, especially if she was in a country like America. You have no right to medical care there, and unless a charitable source stepped in, the authorities would have sent her to prison for it, despite the fact she was found and brought to the hospital through a concerned citizen's choice. Countries like America aren't big on helping those that can't afford it, leaning more towards punishment or shaming in one way or another as a solution for poverty; especially if the person isn't an American citizen.

Well, if regular citizens can't afford homes with the money they have, how do you expect the homeless to get housing?
I used to work in an apartment complex for the homeless, and it's sad to say, but a lot of homeless people simply don't want to improve their condition.
For housing, you need to work to get it. Workers aren't just going to build houses for free. They need to be paid.

@@DrakusLuthosit’s sad to say, but a lot of people will talk about homelessness and the issues that lead to it, without understanding the scope of the issue.
No one wants to leave behind the embers of their dreams. But as we live, surviving takes precedence. That’s one way to sum it up.
People in crisis may not automatically understand or know of the services available locally and to inform people in crisis about services requires trained individuals (trained in counseling, resource navigation, hospitality [as in anticipating someone needs as well as asking what they need- I can anticipate that a ‘warm referral’ will meet the needs of the person in crisis. But to assume they want or need my spare jacket is kind of rude and not very dignifying. It’s better to ask if it would be helpful to them)].
I’m studying how to implement an employment navigation system in a county with high rates of homelessness, and also actually doing the community based organizing work to implement such a program side by side housing first policies, I learned that the person who thinks someone doesn’t want help while in crisis, has both not been able to understand the crisis and has also given up. I took a contract position for a year to set that program up in a well known county/city.
People don’t change on the revelation of information, like “hey crack is bad for you should stop.” People change over a course of time where they’ve been able to safely look at what the ‘wrong’ thing is on their terms with the guidance of anyone trained in ‘motivational interviewing’ (an applauded counseling approach) and then they’re able to see clearly what they want instead. -all human function this way, motivational interviewing is an effective counseling approach for anyone struggling with making a change.
Forcing change often influences people to fight the change as well as triggers people ‘righting reflex-‘ the instinct to see ourselves as a good and decent person.
Supporting people through positive changes with learned helplessness and lived experience in homelessness and who may even also suffer from an addiction is not easy.
I can also tell you that just working in the places you’ve been a d having seen resistance to positive change, the people you meet do not make up all of the people experiencing homelessness. You’d be surprised how many people leave their tent at 5am to go work a temp construction job and how many people sleep at the police station to know they’re safe while they wait for updates on income, employment, services, benefits, a case manager, etc.
Please try to understand the community you operate amongst. ♥️

Also being disabled, and barley get any money from SSDI,,,So I lost two of my sons and my eldest daughter all to cancer in a manner of a few years, then my husband, and my home and all my belongings, had to leave my city (even tho didn’t want to) and now living in a tent.... with a hundred wild mice, who run a muck all over. I don’t even bother praying anymore, since every time I pray even more bad things happen. I just wish I could have a run of good luck and Some really good things happen....😞

ah, so this is why the recovery process for eating disorders such as anorexia should be carried out carefully as well. now it makes sense why you cannot immediately switch to a normal daily calorie intake after your body got used to eating like 300 kcal per day, and why your meal plans should always include a subtle but gradual increase. overfeeding yourself on cookies seems like a really insidous process as well, given how many you can eat until you feel full, unlike with many other foods that actually have nutritional value.

I actually work on unit for eating disorders in a psychiatric hospital! We’re very careful to avoid refeeding syndrome. We have a specialist dietician that plans out all of the meals for our patients and we start them off with smaller meals when they first come in and then work them up to larger meals. We also monitor their fluid intake and make sure it’s within range of them not being too dehydrated but also not ending up with hyponatremia. We use the nasal feeding tubes shown in the video on our more extreme patients too. It typically takes about 1-2 months before they reach their ideal weight and can be discharged.

Nonsense. The number of homeless, hungry people vastly outstrips the resources available to them. There are barriers to getting emergency housing assuming that there are even enough "beds" available. If she can get a job, awesome, but it will likely be low paying since she's unlikely to have been able to receive higher education and the fact that people are predisposed to pay women less or think women are more suited to lower paying jobs. Even if she did get a decent job, the odds are slim that it would actually be enough to get into permanent housing when rents are going up way faster than wages all over the country.
Don't be mad at this woman. Be mad at the people who buy fleets of sports cars to drive past people like her. Be mad at the politicians who take million dollar donations from those people, get reelected and give them a tax cut that does not "trickle down," and then cut spending to schools and affordable housing programs.

I don't think it is 'absolutely' ideal. As someone who knows the workings of ATP and insulin in detail, a sizable portion of the content in this video was redundant to me, which made it kind of a slog.
By no means do I think the pacing was wrong, less people study A-level biology than do after all, just 'absolutely' suggests it was right for everyone regardless of prior investment in biology.

    • @gl00merz
      @gl00merz 6 років тому +27

    • @justmonica9253
      @justmonica9253 6 років тому +13

...I didn't mean to offend. All I meant was the pacing was for people with little prior knowledge in biochemistry. As such, the pacing was off for people with more of that specific knowledge. ( as I perceive it, at least )
I was not trying to imply that people who don't know about these things are stupid or ignorant. No one knows everything. If you showed me a video in this style on geology I would think it was well-paced for me as well, since I have not studied geology. All I was disagreeing with was the use of the word 'absolute', which implies everyone would be satisfied with it.
I feel like if these comments are taken as feedback, I wouldn't want Chubbyemu to think it couldn't produce videos to cater for different levels of expertise. That is all.

I've had refeeding syndrome as a consequence of being given IV rehydration during an anorexia relapse. This was my experience in the hospital: I hadn't eaten or drunk well for many months, had not eaten anything at all for a few weeks. I woke up one night experiencing bluish fingers, severe chest tightness, pounding heartbeat/ringing in my ears, etc.
After the symptoms subsided I attended my local A&E, the nurses took bloods, did an ECG (abnormal) and started IV fluids (5% dextrose solution). Later I needed the bathroom, so they asked for a urine sample, they said it was very high in sugar and ketones. That's when they took more bloods and finger prick tests.
At first they thought I was diabetic. An endocrinologist, gastroenterologist and dietician were called down to A&E to look at my case. They said it was refeeding syndrome. My blood pH was acidic, my blood sugars were hyperglycaemic, I had low potassium, magnesium & phosphate (important) and a bunch of other deficiencies (these were less important). They also said I had a possible infection but were non-specific.
I began to feel very cold & confused, told everyone to "stop making such a huge fuss" and was wrapped up in lots of blankets. One of the doctors said I was confused because my brain had shrunk from the dehydration, so I just needed to trust that they knew best. I agreed.
It had only taken 3/4 a bag of 5% dextrose to trigger the RF syndrome. My treatment was similar to what is described in this video, but not exactly the same. I was immediately given IV: antibiotics, banana bags, magnesium, phosphate, and oral phosphate, magnesium & a multivitamin. A few hrs later I was fitted with an NG tube. I became hypoglycaemic which was corrected with orange juice. I was connected to a heart monitor, but a residence nurse took away the machine during the night because the beeps were annoying her lol.
My treatment protocol was under the MARSIPAN method and overseen by a team consisting of gastroenterologists, dietician, psychiatrist and psychologist. The purpose of the admission to the gastro ward was to stabilise my biochemistry and CBG's. They used my body weight/height to calculate how many ml of 24hrs continuous NG feed to give me per day, increasing it slowly. The type of NG feed I had changed every so often, at first it was the regular small bag, then a bigger bag, then a high protein one, then a concentrated one. No idea why, but I know it was important as they got super edgy about me having the right one.
For my oral diet, for the first week I was only allowed liquids like clear broth or jello. A very important part of my diet was to drink milk, as it is a great source of dietary phosphate. The dietician constructed a meal plan to address my body's specific needs. The meal plan changed daily, designed to slowly increase the amount of carbs I would eat everyday.
I received daily blood tests, IV: banana bags, saline (no dextrose), oral phosphate, magnesium & multivitamin supplements. Nurses checked my CBG's as part of their regular obs. For the first few days I still had high ketones, but that eventually went away. I thought I was over the worst of it after being in hospital for about a month, but got RFS again only 5 days after being discharged.
I was in the supermarket buying groceries, when I suddenly felt like I was going to pass out. I got to a bench and called my GP asking what to do, he called an ambulance, they did obs (abnormal). Took me to A&E where I was assessed & admitted to AMU ward due to abnormal blood work & hypoglycaemia.
The doctor overseeing my care on the AMU ward introduced himself, pinched my stomach and said "look at you, you have no belly fat!". I didn't receive any medical attention, I barely remember those 3 days as I drifted in and out of consciousness. A relative came to visit and was concerned, so called the gastro ward where I had been treated before to let them know I was in AMU.
On the third day, the doc put a Fortisip compact drink (300 kcal, 37.1g carbohydrate) in front of me, and said if I drank it he would allow me to go home. Later that day a call came through saying a bed had become free on the gastro ward and I was being sent there. I was confused as I thought I was finally being discharged. Turns out the Fortisip drink had given me RFS so I had to stay in hospital for further treatment. Same treatment protocol as mentioned before.
I ended up staying in hospital for an extra 6 weeks recovering. One time I went to the cafeteria downstairs and ate a 6" Subway chicken & bacon ranch sandwich, as I was managing regular meals, a few hrs later the phosphate and magnesium drips made a magical reappearance. It was frustrating but the doctors told me not to blame myself for this and to keep eating and they would correct the problem.
I thought this was worth mentioning, as it doesn't always take 40 cookies to trigger refeeding syndrome. Sometimes it can take very little when a person is chronically malnourished. This is why it is very important to monitor a person very closely for signs of RFS when reintroducing foods, as far as I'm aware, no one has been able to definitely say how many calories or grams of carbs it takes to trigger it.
There's also the issue of underfeeding when someone is underweight,, they can die of hypoglycaemia. I had hypoglycaemic episodes whilst in hospital too, so regular CBG checks and caloric adjustments were a must. It was a constant balancing act for my doctors.
That was just over a year ago. I am doing much better now and am forever grateful for all the doctors, psychologists, nurses, & HCA's on ward D7 who helped save my life and taught me to laugh & live again. Especially nurse Sarah for your hugs every time I was having a bad day, Dr Pither for your amazing expertise & knowledge, HCA Daniel for making Rick & Morty jokes during mealtimes, and my wonderful dietician Hannah who held my hand the very first day and said "I'm going to teach you life can be beautiful".

I really hope MJ and her two kids got help after this. Hopefully the hospital sent a social worker to talk to her before discharge to help set her up with some resources to help her and the kids get a stable, safe place to live and access to nutrition benefits.
I really hope she's doing better.
✨Please vote for social support programs✨
Things like sufficient nutritional benefits, housing benefits, family benefits for low income and/or homeless people and families, etc.
This level of extreme poverty and homelessness shouldn't be a problem in a first world country.
Rant over. Thank you for coming to my TED Talk. 😅

For any flack you may be getting, rest assured that you did scare me into changing my sedentary lifestyle and resolving to check in with my GP more often. Keep up the great work, it's a good motivator for us to value our health and be more mindful about these kinds of things. I'm off for an evening bike ride, thanks!

Young man, your presentation of medical issues is absolutely so appreciated you deserve
To have your own TV/cable/Netflix Shows a talent to explain these issues so the general
population can understand .Plus each video gives me more awareness of how issues
Can be more complicated than first appearances. So glad I found your channel
