You are correct; Endermen can't pick up crafting tables (or most player-made blocks for that matter), so we're just going to call artistic liberty on that one :P

where did you download the model for the wood block that fell?

It's actually just a block that I made part of the map, so it's not a model that can be used outside of that map.
However, I think there are several Minecraft model packs available in garrysmod(dot)org, so you could try searching there.

I actually didn't: only the textures were imported from Minecraft. The map geometry itself was created by hand with the textures added onto the surfaces.

The game was officially released a few years before this video, but because of how the textures were extracted and set up for SFM the video certainly *looks* like it's based on a much earlier version of the game.
