What the hell, why are they just auto attacking

3.25 introduced a new skill archetype called "Retaliation Skills," - these skills require your character to block attacks. If you're not blocking then you're not casting retaliation skills. Goratha's league-starter is heavily invested in this mechanic and rotates between a "wave" like skill and a "fist" skill that strikes from above. The Bladestorm (blood) is a filler to do a little damage while waiting for the retaliation skills to be able to be activated/used.
I've done this Pirate fight twice, and I'll be honest with you, you can face-tank just about every attack the Pirate does, but this is HC (perma-death) so Goratha isn't going for risky block-activations to proc his retaliation skills (main-damage). The challenge to this fight was less the boss and more the arena; area denial, big attacks, wave related phases etc. It's not an overly difficult fight but it does take a while before you get better gear, weapons etc.

Can you still use totems to stop the exarch balls in their track?

ye, but they explode so it has to be at range

yep they explode on collision so dont stand close to your totems or you might get shotgunned
