In regards to the restaurant, it is referring to the 4 star and 5 star luxurious ones, as there are two kinds of women when it comes to restaurants. The first kind prefers to be taken to a 5 star restaurant on a first date, while the second kind knows when it is too soon for a 5 star restaurant. That's my take on it.
Restaurants are not 5-star or 4-star....those are hotel ratings....which are decided on many other factors than the restaurant(s) at the hotel.
Restaurants can be Michelin star rated...that goes up to 3 stars at the highest.

How the hell is that even up there? Is there really anyone who actually thinks going to a restaurant is too soon? If so when is it even ok to do it then, if not on the first date? What is that persons first date choice then? So many questions are in need of answering here.
its my understanding that many people go to something like a coffee shop where its a public place with no need to stay if you want to leave. at a restaurant you might have to wait for a bill or be mid meal when you decide it's not going to work. at something like a coffee shop you pay for your stuff first, it is also usually just a drink or a small snack item so you can walk whenever you feel like cutting the date short

I guess that is the number of people surveyed who only read part of the question. A surprisingly small number actually. It used to be that the shows producers selected the survey people...and like any survey the people get paid. So that SMALL of a number of people is probably because the people selected wrote the studio or applied for jobs there.
I did actually have a young lady tell me it was too soon to go to church though.

Yes but rare. Last time I saw it, the family made a bad decision to pass because it was the last question with only 3 answers. The were the defending family and the lost because the opposing family got all 3 answers and won. Even Steve told them at the end that you don't pass on the last question and with only three answers.
