Trigeminal neuralgia is a painful condition of the half face caused by compression of trigeminal nerve. The pain is associated with tingling numbness of the half face. Pain comes in cycles and is often unbearable. Trigeminal neuralgia causes are vascular loop, brain tumors and multiple sclerosis. Multiple sclerosis causes trigeminal neuralgia. Trigeminal nerve pain is severe and not relieved easily. Trigeminal neuralgia pain relief is by medications, injections and surgery. MVD procedure for trigeminal neuralgia is highly successful. Injection for trigeminal neuralgia can be useful. Medications for trigeminal neuralgia are to be taken lifelong. Trigeminal neuralgia and MS association is strong. Gabapentin for trigeminal neuralgia can be tried. Carbamazepine is the first line of therapy.
    What is trigeminal neuralgia
    Trigeminal neuralgia (TN) is a chronic painful condition where there will be excruciating pricking type of pain in the face, mostly on one side of the face. This is also called facial pain or jaw pain. Actually this involves, trigeminal nerve fifth cranial nerve which supplies sensation to our half face, hence the name. This is a neuralgic pain, means nerve compression pain hence it will be very severe and intolerable.
    The pain can be brought on by mild stimulation of the face, such as brushing your teeth, putting a make up or shaving. It’s often described as feeling like electric shocks or stabbing type of pain. Initially it will be short, mild type of pain, but over time they may experience longer, more frequent attacks of intense pain often unbearable.
    What are the causes for TN: Commonest cause of TN is due to one of the arteries inside the brain loops over the trigeminal nerve. Sometimes I also see a tumor that putting pressure on the nerve causing trigeminal neuralgia. Rarely, multiple sclerosis, a well known disease which damages the myelin sheath, causes trigeminal neuralgia.
    How is trigeminal neuralgia diagnosed? Diagnosis is by neurological examination.
    What is the treatment for trigeminal neuralgia
    First line of Treatment as always in my practice is medications. There are many medications used in TN. Commonest is some kind of anitconvulsant medications, same medications used for treatment of seizures.
    There are some excellent surgical procedures are available for patients with TN.
    1.Glycerol Injections:
    2. Stereotactic Radiosurgery: This is a scalpel less, pain less and blood less procedure even if it is called a surgery. Radiosurgery. Here I use highly advanced radiation platform called TrueBEam system to deliver high energy radiation beam precisely to the root of the TN ganglion in the skull base.
    3.Radiofrequency or RF Lesioning: I do this outpatient procedure under heavy sedation.
    4. Microvascular Decompression: MVD procedure is my favourite surgical procedure. In those patients who have a blood vessel compressing their trigeminal nerve, this surgery can be done.

      I have bilateral trigeminal neuralgia. R/side surgery in 1965 where 2nd &3rd branch were cut at base of T/nerve. No pain since than but also permanent numbness. In 1974 got it L/side. Since than on tegretol with spontaneous remission lasting for 2 to 3 years. Presently on tegretol pain under contro.

    The herb was called Scutellaria - Skullcap. Mayabe also ensure you have a magnesium supplement

    I have been suffering fron TN since Oct 2022, I experience using tegretol for the first few months but I stop bec of the effect of this med.My neurologist orescribed me Carbox tablet It helped me a lot,But when I came to canada my doc here changed my medicine to Gabapentin.Now I used to suffer the pain I feel that gabapentin does not help me a lot .I prefer carbox but it lower my sodium according to my doc .

    Hello !
    Is this nerve also responsible for a " freeze up"? When one drinks something too cold too fast we get a brain freeze and can have this painful feeling in face at severe cases ... it does subside of course in short time but is this trigeminal nerve apart of this " freeze up"?

    Systemic infection is a huge cause.
    Unfortunately, most doctors don’t do a good assessment of May systemic infections including Borrelia, Bartonella, Babesiosis, Q fever, etc. His 10 year old patient probably has an undiagnosed systemic infection.
    Also, most doctors don’t use MAIO drugs because they are simply unfamiliar with these group of drugs.
    My husband ( a dentist) did his research & found that Parnate would help.
    None of my doctors would prescribe it , so I had to see a psychiatrist.
    Since the very first dose, I have been 100% pain free. It has been over 7 years.
    Thank the Lord in heaven for my husband.
    One really must be their own patient advocate.
    The most my doctors gave me was gabapentin, which is probably what this doctor prescribes.
    He’s a fairly young doctor that also doesn’t know much about MAOI drugs.
    I got my ATN from Lyme disease.
    Even cured, my ATN never got better.
    My ATN was both sides 24/7.
    Not one doctor I saw ( out of 22 doctors) knew what to do.

      Don't go for Mvd its not easy thing , try gamma knife or other procedures and they can be repeated time to time

    Doc I have a trigeminal neuralgia, my problem is the nerve that was damage is in my gums at the right side where I got my teeth extracted. First, the doctor here prescribed me carbamazepine coz , I am really in pain eating, drinking, swallowing, and also in talking. Now I am taking gabapentin and it will be my 3rd week on monday and it has lessen the pain only i could not talk properly coz of pain, sometimes the pain is bearable and also in eating , sometimes less pain. but today I' am always in pain that I could hardly eat, as if the medicine has no more effect. I always swallow my food because my only teeth left is infront 4 down and 6 up. Please could you give me an advice. Thank you!
