Its implied that 4419 isn't truly sadistic but has a very twisted sense of empathy. All bodily modifications are made in such a way as to prevent the initial injury from ever happening again, like the man who had a heart attack and was given additional hearts. So from SCP 4419's point of view, its helping people.

I like to imagine that the Butcher's Chariot isn't actually malicious at all but simply horrifically incompetent. Guess the Foundation is really formidable themselves, we know MTFs are in really limited numbers since they are the best of the best , but aside from that Foundation might have regional containment specialists or security forces in huge numbers other than MTFs, sorta like immobile task forces rather than MOBILE* ones.
My head canon is that it isn't one universe, but actually an amalgamation of parallel universes which all share the same Foundation. If the Foundation could secretly (without most of its employing even knowing) recruit people from different universes, it could also explain how it gets so many D-Class personal or how the world hasn't ended. At the same time, it would make it easier to hide or contain Keter Class SCPs, if there are only a handful in each universe. Otherwise it would have been impossible to keep all SCPs secret, or the foundation would have been overwhelmed (or depleted from all the crazy breakouts and whatnot).

So you might be right that multiple universe have collided with each other, creating anomalous versions of things we find normal in the Baseline reality. Edit: Now that I think about it, it must be a real burden for some of the top researchers and O5 members to know the existence of most of the anomalies, the cognitohazards, the extradimensional horrors and the monsters which killed, consumed, mutilated, enslaved, tortured or drove many friends, family and coworkers to the point of madness. i’m taking classes to become a nurse and this SCP really fascinated me, especially the instances where the anomaly seemed to modify the patient’s bodies in a way to not have that same injury again you can’t get a scrape on your knee if your knees point upwards you won’t break your jaw if it’s pried open to your skull (new bone formation! jaws are usually separate bones, allowing them to open and close so if it doesn’t close it’s fused to the skull and such bones are harder to break) the fact that the couple now had none of their broken bones- i’m not exactly sure why fusing them together was the path this entity went down but maybe it’s because say person A had a dysfunctioning heart and person B had a collapsed lung, so they decided in order for them both to survive they had to share the organs the man who suffered a heart attack had plenty of new hearts in case one of them breaks again people in the fire seemed like they were watery, so fireproof not sure about the Private, seems like this was either revenge or he had all his vital organs taken out so that the only thing that could be penetrated with a bullet wound was the bottom layer of his skin (it said paste- fatty, most likely, and not elastic) which has no nerve cells and nothing but adipose tissue aka fat which won’t ruin anything for the organism if it gets hurt as its MEANT to be a barrier if sorts. i guess the eye that was left was meant to be there so he can look out for attackers? great SCP, love it update ig! i wrote a lil story on ao3 about why there’s two paramedics and not one :)
Maybe it’s not trying to be “evil”; what if it thinks it’s helping the victims, like it fused a bunch of hearts to the guy who had a heart attack and took out the broken bones of the ones who broke bones. To us the aftermath appears to be bad and evil but to an “alien” who doesn’t quite understand our physiology it might think it’s helping? What do y’all think?

I love the theory people are posting, the scp isn't evil and is genuinely trying to help us with its surgeries, but is just incredibly bad at what its doing, or doesn't realise that its methods are hurting us. And for the fusing of multiple people, they get in the same ambulance right, so it probably thinks its one creature that was broken into pieces and puts them together. Now that I think about it, that makes jt similar to the gas mask child in doctor who

The incident with the couple in a car crash returning fused by the backs and having broken bones permanently removed genuinely makes me think that SCP-4419 is trying its best to help prevent recurrence of the initial injury (e.g. being fused by the back to prevent both people from suffering the same injury and supporting each other, and preventing bones in these limbs from sustaining another fracture) ...But its understanding of how human anatomy works is virtually non-existent, which is why it's so scarring to both the public and victims Honestly the one with the soldier being turned to this thin paste and spread across his barracks was extremely unnerving. But I wonder if SCP-4419 thought that the soldier would die if he were to step outside in a war zone and/or be shot again
considering that the soldier lacked internal organs, they probably had a faint understanding that if those organs got hurt he’d die so they removed them?
The thing about this anomaly is that we're not really sure about what it's intentions are. Whether it's incredibly misguided in trying to help humans, due to having little to no knowledge of the human anatomy and having an alien psychology/mindset or that it actually is malicious.
What if we electric chair someone in its vicinity? Also what would happen if say perhaps we have the injured person hold a whole bunch of medical books and such....would the quality inprove....or.....
I think it's the perfect metaphor for people who are confident and insistent about helping others but have the inability to perceive that sometimes it really is best to allow someone else to be the one to do it.

After a seizure, I'm very out of it and physically weak so I probably wouldn't be able to resist or even protest very well even if I was awake and trying to say I didn't need an ambulance. I love the theory people are posting, the scp isn't evil and is genuinely trying to help us with its surgeries, but is just incredibly bad at what its doing, or doesn't realise that its methods are hurting us.
And for the fusing of multiple people, they get in the same ambulance right, so it probably thinks its one creature that was broken into pieces and puts them together.

The incident with the couple in a car crash returning fused by the backs and having broken bones permanently removed genuinely makes me think that SCP-4419 is trying its best to help prevent recurrence of the initial injury (e.g. being fused by the back to prevent both people from suffering the same injury and supporting each other, and preventing bones in these limbs from sustaining another fracture)
...But its understanding of how human anatomy works is virtually non-existent, which is why it's so scarring to both the public and victims
Honestly the one with the soldier being turned to this thin paste and spread across his barracks was extremely unnerving. But I wonder if SCP-4419 thought that the soldier would die if he were to step outside in a war zone and/or be shot again
considering that the soldier lacked internal organs, they probably had a faint understanding that if those organs got hurt he’d die so they removed them?
The thing about this anomaly is that we're not really sure about what it's intentions are. Whether it's incredibly misguided in trying to help humans, due to having little to no knowledge of the human anatomy and having an alien psychology/mindset or that it actually is malicious.

Object Class: Keter
Special Containment Procedures: Due to the expansive range of circumstances in which SCP-4419 manifestations occur, containment efforts are to focus on information control and post-manifestation cleanup rather than physical imprisonment.
Any witnesses to an SCP-4419 manifestation are to be dosed with a Class-B or Class-A amnestic as appropriate for their level of exposure. In cases where it is possible to restore the bodies of SCP-4419 victims to roughly standard human configurations, a cover story is to be established to explain any remaining damage as a result of their original injuries. In cases where this is not feasible, victims of SCP-4419 are to be brought into custody and, if possible, euthanized. A cover story is then to be established regarding the death of the victim due to their original injuries or conditions.
Description: SCP-4419 is a vehicle resembling an ambulance of varying make and model which will spontaneously appear in an area shortly before a medical emergency arises. The means by which SCP-4419 predicts these situations is currently unknown. Although the appearance of SCP-4419 differs from manifestation to manifestation, it will always resemble an ambulance appropriate for the local culture.
Upon the occurrence of the medical emergency, SCP-4419 will proceed directly to the injured individual, hereafter referred to as the victim. Two individuals of varying physical appearances in paramedic uniforms will then exit from the back of SCP-4419, secure the victim, and bring them back with them into SCP-4419. The individuals that emerge from SCP-4419 will behave as expected for a medical professional in the situation, but will repel any attempt by others to prevent them securing the victim via extreme physical force.
Once the victim has been secured within SCP-4419, it will leave the area at extreme speeds, disappearing the moment it is outside of observation.
Two to seven days later, the victim will be returned outside a local area, suffering from extreme and invasive bodily modifications. Although the majority of these alterations would logically result in the death of the victim, death will not occur in these cases unless the modifications are tampered with or otherwise undone. The specific nature of these modifications differs from case to case, although there does appear to be a level of correlation with the original medical emergency. (See Encounter Log 4419-1.)

23/11/1994 A man suffers from a broken jaw following a fight outside a bar. Victim returned with his jaw forced permanently open. In addition, a glass window was installed in the mouth to permit viewing of the heart, which had been relocated to the back of the throat. Due to the relocation of the heart, reversion of the body modifications was not possible, and the victim was euthanized.
19/06/1999 A homeless man suffers from a drug overdose. Victim returned to the same place he was originally taken from. The top of the victim's skull had been removed, and the brain crudely scooped out and placed into the victim's hands, which had been fused together in front of him. While being brought into custody, the victim's brain fell out of his hands and he instantly expired1.
29/01/2003 A wife and husband involved in a car crash suffer numerous broken bones and severe bleeding. Victims returned fused together by their backs. All bones that had been broken in the original accident were meticulously removed, resulting in the loss of use of some limbs. The victims were successfully separated, administered amnestics as appropriate and their modified limbs were amputated.
15/02/2006 An elderly man suffers from a heart attack. Victim was returned with eleven additional and non-functional hearts within his body stuffed between his existing organs. Expiration occurred when surgeons attempted to remove these additional hearts before agents could arrive on the scene. Surgeons and medical staff who had treated the victim were administered amnestics, and the body confiscated.
19/09/2008 A fire at a bar results in nineteen people suffering from severe burns. An additional seven people suffer from skull fractures and broken bones when they attempt to confront the individuals who emerge from SCP-4419, and are also taken as victims. It is believed existing injuries were exacerbated, and new injuries caused, by attempting to force twenty-six people into the limited space within SCP-4419. Victims returned to the local community center as a single watery mass which twitches and shivers when physical contact is applied. As no method of euthanizing the victims could be found, they are currently stored in a liquid tank at Site-31.
24/11/2014 A US Military private is shot while on patrol in Afghanistan. Due to the suspicious nature of SCP-4419's arrival and the forceful securing of the private, other soldiers on the patrol fire upon SCP-4419 as it leaves. Witnesses report seeing a viscous black fluid leaking from the resultant bullet-holes in SCP-4419's surface. Victim is found broken down into a thin paste and spread over the walls of their barracks the next day. The agents who initially secured the remains reported seeing a mostly intact eyeball dilate when they approached. Although what has been brought into storage is referred to for convenience's sake as 'remains', it is currently not known whether or not the victim has expired.

I find the scene that the girl sees her friend in the ambulance off bears a striking resemblance to a scene in Animal Farm when Boxer is taken away by the butchers. The title of the SCP, the scene's set, the true nature of the ones driving the vehicle, etc, is almost identical.
