We know that they skipped 5 in exchange for a coin with the same denomination, at least since 1987. If these banknotes were really demonetised, then they are now using the New Zealand banknotes.

All 1992 + cook islands notes I have seen start with A, and are fairly common.

Yes, but depends on the coin, look up ebay sold items for a start.
