Sick Kids usually don't really think of anyone but themselves as they are raised to be very self involved and self centered because of their medical condition. You're absolutely right. They severely abused and neglected their younger son, and thus have lost the right to be his "parents". And even if he wasn't taken away, can you imagine living with them after the older brother died? Knowing that they only had you so you could keep your sibling alive and not even loving you enough to consider your autonomy? It's already driven a wedge between them, and unless those "parents" make some EXTREME changes, they're not going to lose just one son, but both.

Because if they did, the parents could sue them for 'trampling on their parenting rights' as, until a certain age, parents can make medical choices for their child. The only way they could step in is if Gavin was saying no and asked the hospital staff for help because his parents tried to force him to. The older brother knows that he's sick and dying, and is also having to (literally) live with the fact his younger brother is suffering horribly for him; while the younger brother is in constant inner turmoil because he's so scared and is constantly being gaslighted by his parents to be a donor for his brother while his own life and rights are being shoved aside. Not to even mention how freaking YOUNG Gavin is, Chirst. There's a reason why minors are rarely donors for these sort of things.
In my eyes, these parents are no different than the ones who beat, neglect, and sell their children for their own benefit. They only care about themselves and their needs/feelings, no one elses.
If I were ever in the position of the older brother, and my sister the younger, I would choose to die or receive transplants from a appropriate donor, even if it meant it would be too late. What would be the point of living if someone I loved was suffering because of it?
