The usual method of releasing the finished form from the mould tool is to reverse the air.. Just rig it so you can give a short 'Blow' into the vacuum area, it will release easier.. 14 years of Vac forming & injection moulding at Vauxhall Motors ;-) Steve
I design and program and machine thermoforming/vac/injection professionally. My advice for you is, start with a bondo material. Sand it reel fine. Drill vacuum hole along the tight detail line on the mold and along the contour of the part. Space the vacuum hole 2-3inches apart. Clear plastic forming, the mold will need to be polished nicely otherwise it will appear on the part. And don't forget to make a plug assist to force the plastic down into tight area.
Some suggestions - seal the concrete with some sealant first. Also put some screw holes in the bottom of the concrete piece so when it is done you can insert some screws to remove it more easily. Did you use any release agent? The sealing of the concrete would have made it release much more easily as well.
Next time try spraying a thin layer of cooking spray on the mold first. Also, it might have helped to embed some handles in the front and back of the bottom of the mold to make it easier to pull out.

you need to use a warm thick clear lube it makes getting the lexan off the mold almost effortless. Make shore your mold is smooth.

+Nick Doe his problem was he got wrap around whe the plastic formed around his mold and under it making release difficult
use compress air on the side flap to release it.
Just a heads up, a little compressed air would have made it easier to remove to mold from the original piece.

U need a spacer plate under your mold just a bit wider then the mold and it will come right out.
