What can cause vaginal bleeding after 20 weeks gestation, and what should I do about it?

  • If you are over 20 weeks gestation and you experience any vaginal bleeding, you should get checked out. If it's just a little bit of spotting, and your doctor's office is open, you can call them if you want, and see if they have additional advice for you, but generally speaking, they'll probably want to have you seen. That might mean in their office, or it might even mean going to Labor & Delivery. That is a good reason to get checked out. There are a few different things that can cause vaginal bleeding after 20 weeks. Once you get further along, like once you're full term, and you start having contractions, and going into labor, it's normal to have a little bit of spotting. We call it "bloody show". And the cervix is highly vascular, and once it starts to dilate or change a little bit, then it might bleed a little bit, and that's why we do see a little bit of bleeding during labor. That could be an explanation for the bleeding if you're contracting, but again, it's always a reason to get checked out so that we can rule out other complications that might be more serious. Again, because the cervix is highly vascular, other sources of irritation can cause a little bit of spotting or bleeding, so if you go to your doctor's appointments later on in pregnancy, like 37, 38, 39 weeks, and your checked, you might see a little bit of spotting that night or the next morning. And also, intercourse can cause it. But again, you should call your doctor, and they can ask you more specific questions, and decide if it needs to be investigated or not.
    More serious causes of bleeding include a placenta previa and a placental abruption. A placenta previa is where the placenta has attached over the cervix. And that's a problem, because, for one, the placenta is your baby's lifeline. A ton of blood is going there every time your heart beats, and that's what's feeding your baby oxygen and nutrients. And if your cervix starts to dilate at all, then you'll start to bleed, and that can be potentially life-threatening to you, and also affect your baby's well-being. A placenta previa is usually first diagnosed at 20 weeks when you have a routine ultrasound. And if the placenta looks like it's anywhere near or over the cervix, then you'll have repeat ultrasounds over the course of your pregnancy to see if it's "migrated" as the uterus grows. It doesn't actually grow legs and go up the uterine wall, but it's like words on a balloon, and the words can appear to be near the base of the balloon until you blow it up, and then as the balloon blows up, the words are pulled up, and that's like the placenta - it can actually pull away from the cervix and be fine. But sometimes it doesn't pull away, you still have a placenta previa, and that's a serious condition, and you need to be checked out as soon as possible in the hospital if you have any vaginal bleeding.
    A placenta abruption is where the placenta starts to prematurely detach from the uterine wall. Now this shouldn't happen until after the baby is born, because again, the placenta is how your baby gets oxygen and nutrients. So if it starts to come off prematurely, that means that the baby is not getting all the blood-flow that they should get, and you're losing blood. So if you go to the hospital and your bleeding, that's always something that we keep in the back of our minds - we wonder if it's an abruption.
    But in the end, no matter what the cause of bleeding is, our plan of care is always driven by "how is the mom doing?" and "how is that baby doing?" Sometimes we have mild cases of bleeding, and everyone's doing okay, so we can just manage it by putting the mom on bed-rest, especially if she's premature. It depends on other specifics going along with your circumstance as well. But sometimes, it's severe enough that we have to do an emergency C-section. So again, the bottom line is if you have any vaginal bleeding during pregnancy, get checked out. And after further investigation, your doctor will decide if treatment or intervention is necessary.     I'm scared . I flew out of state 24 weeks pregnant. I just went to doc 2 days ago. She was healthy . I'm so scared woke up I'm bleeding

    OK so at 20 weeks I have placenta previa and they've offered me a scan only at 36 weeks. I don't trust the NHS to be thorough; no one has asked me to refrain from weights training or intercourse but ignorance isn't exactly bliss....

    I had bleeding yesterday at 27 weeks 6 days. We haven’t had any intercourse since we found out we where pregnant. Dr, doesn’t really know what caused it. Something about the cervix and placenta not to close to each other that cause a space and that causes bleeding
    Today I’m spotting brown
    (old blood )
    I took the day off today and will probably rest it off.

    I’m confused so I’m 19 weeks 6 days pregnant and just went to the toilet and wiped and there was a tiny bit of blood I’m due to see my consultant next week
