What is Ryzodeg and how can I use it?

    A new insulin for people with diabetes, combining quick acting Novorapid and long acting Tresiba (insulin degludec). This is a handy tool to help people better manage their diabetes and blood sugar.
    So I want to just talk a little bit about Ryzodeg. Now Ryzodeg is a new insulin that is being launched all over the world. And essentially Ryzodeg is a combination of a long acting insulin called insulin degludec and the brand name for that is Tresiba and that is mixed in with Novorapid.
    So it is a basal insulin a long slow insulin combined with a rapid.
    And traditionally it is being used in a very similar situation that we would have used something like Novomix.
    Twice daily insulin with lunch and dinner to get decent coverage throughout the day.
    But actually Ryzodeg is a lot more flexible and a lot smarter as an option moving forward than Novomix, which has been around for about 20 years.
    And the reason it is so much more useful particularly in the hands of someone who understands how to get the best out of it is because it has a very long acting basal component.
    So you can use it like this. You might choose to have it maybe twice a day or alternatively you can use the long acting insulin in here as your background.
    And then have a Novorapid dose with your breakfast, lunch and the Ryzodeg at dinner.
    Now let's say you are not going to eat very much dinner today because it is such a long acting insulin you can then exchange Ryzodeg and have it at breakfast. And if breakfast is your main carbohydrate meal you would have Novorapid at lunch and dinner. But again it is very long acting so maybe the day after you think well actually I'm not going to have a very big breakfast, I'm going to have a big lunch so you can literally move your Ryzodeg dose from meal to meal to meal depending on your carbohydrate intake and your lifestyle, critically your lifestyle.
    And maybe you think well actually, I don't eat very big meals, so the Novorapid component is too big for me. The amount of carbohydrate that I'm consuming is not enough for the 30% Novorapid in Ryzodeg.
    Well very simple, you just split the Ryzodeg in two and have Novorapid as your third meal. And again because it is so long acting you can swap and have Novorapid with breakfast, Ryzodeg at lunch and dinner.
    Or alternatively, if you don't have breakfast you could just have nothing for breakfast but Ryzodeg at lunch and dinner.
    The flexibility is really quite impressive in terms of trying to match your lifestyle and the ability to tweak the system to get the best results is great.
    So Ryzodeg is becoming more and more available.
    I think it is something worth being aware of because it maybe something that you are going to need to know about.
    The other thing I want to show you about Ryzodeg, very quickly, is it is in the new flex pens. So traditional pens you dial up, and when you dial up you can see the plunger moves out.
    With Ryzodeg, actually you dial and the plunger stays where it is. And it is a much gentler button compared to the traditional pens.

    Hi sir, is it safe when we combine ryzodeg and mitfornin?? its doctor suggestions if my.blood sugar high. im worry if can causes heart problem..thank you sir

    Easier yet, use Tresiba on its own, and if needed, a rapid acting on its own... The most flexibility anyone can get if injecting insulin. Pre-mixed insulin should have stayed in the 80’s and 90’s

      Totally correct but in some countries there is no Tresiba on its own so you have to be inventive.
      I also ran the type 1 ryzodeg trial and that went really well for people in the study.

      For the most part, pre-mixed insulin is just a way to keep people alive, but that’s about it since it gives no flexibility compared to having a Basal, bolus regime since you can then adapt the dose for meals and you also get enough basal dose without having to chase the rapid acting part of a mix

      @@ReyOfLight I am not sure I agree completely...total flexibility is always great but I have hundreds of type 1 and type 2 people with Hb A1c below 7 who are thriving on Ryzodeg not just surviving. Imagine you are on 10 basal and have 5 rapid at breakfast....why not use 15u of Ryzodeg and rapid at other meals....works a treat. The big challenge is those on 30 basal but only have 5 units of rapid. That's the bigger challenge.

    Be nice if u could get it straight without the rapid combined in Australia. Advanced country, backward when it comes to approving meds

      The Alter Alter Ego I completely agree. The reason is that the PBS in Australia want companies to price match with a comparable drug and for the long acting component (Tresiba), the comparator is Lantus. Lantus is a 20 year old insulin now so is much cheaper. The fact that Tresiba has a 50% reduction in hypos compared to Lantus is ignored by the PBS.

    Hi doc....i have been using ryzodeg in 22units for breakfast and dinner and using novorapid for lunch at 10units.....the result before i ate lunch my blood sugar became 13.3 mmol/L and 2 hrs after dinner my blood sugar stay in 13 mmol/L.....so i believe its very high and not normal...so do you think Ryzodeg is effective for me? or do i have the wrong way to test or dosage?

    Can i take ryzodec insulin two times a day in type 2 diabetes bcoz my fasting sugar is 180 above

      Yes you can take it 2 times per day. If sugars are high the dose may need increasing every 4 days until levels are better

      @@sherillspurgeon4000 yes you do as insulin works in the future and a low sugar is about the past.
      BUT....you will need to eat a little more get your sugars up.
      If low sugars are happening often you may need to reduce the dose a little yourself (5-10%)and then chat to your doctor to make adjustments as needed.
      Again this is general advice but based on good principle.

      Fast acting insulin Fiasp contains nicotinamide and some people get itchy from it
