Ive found with a hair dryer, it doesnt heat the pvc like the heat gun. The heat gun can heat it til its soft and folds like paper. The hair dry can only heat it up enough to relieve any stress on the pvc but not enough to easily shape it

Bee wax works very well for mouth piece, you can make it any size.

for the curve in the mouth bit wrap sand paper round a pipe and sand on it

Why not use a bottle to make it a bit larger?
I use a wine bottle to shape it. Mostly with the end to give it better resonance
