Thank you Dr., this very informative and well explained. I wish you would have a practice in South FL. Could you please tell me what would be better for melasma and brown spots? I am prone to freckles and large dark spots in my cheeks and nose. I am not sure if I should go with Erbium or Pico.

Remember you will have to have a series of treatments for whichever one you decide upon and honestly my decision would rest less on the device and more on the practice and the person designing your treatment plan, but if I had to choose one for this particular problem, I'd go Pico. But if you are deciding between aesthetician treatment protocol versus and MD, choose the MD.

Is erbium ablative or non-ablative? I'm confused. I have heard that it is both. Is there a setting on the laser that allows it to switch from ablative to non-ablative or are there simply different types of erbium lasers that offer ablative versus non-ablative. No one talks about this. Thanks

It is ablative in that most of the epidermis should peel or slough after your treatment and new cells take over, but you can alter the strength of the treatment. IPL is an example of a non ablative treatment.

Which one do you recommend for boxcar, icepick scars, and rolling scars, c02 laser or fotona erbium YAG laser ?

These would be addressed with a combo of treatments, usually punch elevation, dermabrasion, and CO2 laser skin resurfacing.

Does it work for ice pick scars

Ice pick scarring is a tough problem that requires a multi procedure approach. Punch elevations, dermabrasion and then laser is our approach. And we always let our patients know that it is for significant improvement and not full resolution.

Hey doc. I just got done with a pixel laser resurfacing.
I am pretty sure thats an erbium yag laser. They use microthermal treatment zone prinicpal.
Anyways because it was an erbium laser i am now thinking that its not as effective because it doesnt penetrate as deep. Is that true?
Also i would have chosen the c02 laser treatment but the medclinic that had it, things didnt workout between us.
So i went with the clinic that had the pixel laser using a microthermal zone treatment, and just because it doesnt penetrate as deep, in the back of my head, im telling myself it wont be or isnt as effective.
Is that true, or am i just over analysing things haha.
    Thank you

This is erbium which is less ablative than CO2. We often combine erbium with PixelRF which is needle delivered radio frequency energy. It takes a series of erbium to approximate the results of 1 CO2. I am unsure what laser you had as there are a lot of different names and brands used. They are all good, in the right hands, so having a provider that can guide you to your best results and understanding limitations of each is key.
