Might I add that Saya is not dead in the real world according to the game itself. After all Riki used it to travel to the past in the final portions of Refrain. According to Kyosuke and hinted by the CG's and the EX OVA, the still near-death spirit of Aya was merely sent back in time to her childhood (during the time she first met Riki) and it's very briefly hinted that she does meets with Riki again after the events of the bus accident. Honestly, Saya/Aya is basically just a newer version of Ayu and Fuuko (wandering living spirits drawn to people they once knew who in the end get a second shot at life due to a miracle).
Probably because they are the odd ones out. And technically Masato is standing next to Riki (as per usual since Riki and Masato are pretty close friends), while Kurugaya is just staying to the side of all the girls.
