The second actually means “fear” or to be very afraid of something. A gesture when you want to communicate to someone else you are afraid or scared of something, kind of like before an exam or before bungee jumping. The gesture for crowded is similar but both hands connecting with all fingers touching and pointing up.

The translation of the first hand gesture is just a little more "strong" that a simple "what are you talking about". If I have to find a corresponding expression in English, I think it could be translated as: "WHAT THE FUCK ARE YOU SAYING?". "What the fuck" in italian is "MA CHE CAZZO (literally: "but what cock") + first hand gesture". In order to make more impressive the gesture, you can also use both hands, which in Italy is very common.
