No cause is given and the audience must simply wait for an explanation. So it starts with sorcery. Early in the series they even have a little joke with it. A bad guy says he's going to use his secret martial arts technique... When the hero calls him out for his bs because throwing lit dynamite isn't exactly 'real' martial arts, the villain simply responds with "whatever works"

My thought is that it would kill the person in ten seconds after removing his fingers, but in that ten seconds the person would mentally experience everything they ever regretted not doing in their lives before passing away quietly and happily.
That would be really toki like, sadly that concept was used in bleach to make someone suffer for what would be an eternity. The exact opposite of what you propose, but this is what make toki the best, he could also do that but chose mercy above all.

I mean yeah that's pretty much it. He created his own techniques and used a supposed art of assassination to heal others. He was called a genius for a reason.
I have always been a FOTNS fan and Toki is one of my favorite characters because of his backstory and technique. But nowadays when I see this I think of UZAMAKI and how people had the pleasureful grins while being contorted.
This is my second favorite scene besides Rei's introduction (a clip that TOEI content bots keep taking down). If you are new to HnK / Hokuto Shinken (i.e. the name the manga took after a volume or two) / Fist of the North Star, you should know that what most likely happens in this scene is that Toki the White-Haired Jesus fires his life energy (chi) at the acupuncture points in the bodies of the attacking guards. Because acupuncture is a bit of a placebo belief thing, the wildly exaggerated body effects are used instead. This makes sense considering that the fighting in the series is very Chinese in its philosophy and both acupuncture (based actual nerve clusters in the body) and the concept of life energy are old Chinese concepts. The Chinese wisdom as a basis gives the action a more mature tone (on top of the gore). Apparently both Hokuto Shinken and Nanto Seiken are fictive Chinese assassination arts. Rei reveals the nature of these powers shortly before fighting Raoh - firing life "energy" (chi) out of his body that "cuts sharper than the sharpest blade." This becomes to focus point in the later half of the HnK 2 when they start mixing that life energy stuff with their touki the fighting spirit manifesting as a colored battle aura to the point of being able to explode a dude into a pile of gore just by being close enough. Technically that makes it blood magic - the other meaning of the word "chi" is 'blood.' It seems that Hokuto Seiken the group of cutting styles Rei practiced (specifically Nanto Suicho Ken) is a simplified version of Hokuto Shinken done here because it does not focus on acupuncture points and instead just cuts things, possibly with minor added (psychological?) elemental effects (such Ryuga's cold feel).
Nanto Seiken's focus is on attacking an opponent with external force. Such as Shins spear-like fingers, and Reis slashing ones. Hokuto shinken attacks from within, turning the opponents own body against them. In this way they are opposite and balanced.
