She (Haolan) is the wife of the Crown Prince i.e. 'Zichu' (if you mean the one with the sick son) Haolan chose to marry him instead of Lyu Buwie (her first real love) in an earlier episode - they have obviously been separated for some reason or another for quite some time? The other girl (I think) comes from a family where at least her father taught her medicine and remedies (she or someone like her called Yun) seems to be in most of the plots. Without a running reminder or cast list to identify each character before each episode (and no grey hairs even to show they are growing older) the looks of all the characters begin to merge and gets quite confusing.
