I still adore the tone shift. Like the original was insane like a maniacal laugh and a crazy smile, this one is insane like a cold and unfeeling straight face in reaction to horrifying things probably being performed first hand.
Also it's really hot
Personally I feel the tone shift is a bit different for me I feel like that insane crazy person feeling was at the end while for most of normal novocaine feeling like a irritated person.

Personally I like to think about the story of this song like a therapist inadvertently causing her patient to have flashbacks to their abusive parent
"It's all your fault" and "its not that painful, so why the hell are you crying?" are some of the lines from the twisted version of the doctor in the patient's head
"don't be afraid" and "you don't deserve it" are lines from the actual therapist, trying to calm down her patient.
idk I just think that's one way of interpreting it. the tone kinda fits, right?

This is actually a BEAUTIFUL take on the instrumental to this song. Like the original sounds almost horror-like cause of the loud and busy instrumental with lots of background vocals *like at **2:13** in this you only hear one set of vocals, but in the original you could hear Hyperdontia playing* and ESPECIALLY at 3:17 when in the original I think it's the fastest and more powerful points, while in this version it's probably one of the slowests parts and gives off a more... clam yet insane vibe? xD I say insane because the lyrics are the same and those are creepy on their own. (and you can pay more attention to the lyrics in a slower and calmer remix like this)
Also as my own little side note. In the original I imagine the patient dies because the dentist tore off the lower jaw, probably having intentions to kill them. But in this one I imagine the dentist somewhat wanted the patient alive, but due to the blood loss tearing teeth out and cutting holes in their mouth, they bleed out. The ending kinda made me think of this I dunno why lol *Spaghetti's analisis over*
