No, they normally do not feel pain. Numbness, weird phantom pains and etc but not real pain. Chaos Dreadnought feel pain though, but thats on purpose to make them enraged.

Like, probably almost never. But Chaos Spacemarines have their own dark mechanicus and so can make their own Dreadnoughts. They use those Dreadnoughts as punishment for the veteran chaos marines who failed in battle. Although SOME Chaos Dreadnoughts are old enough to be with the traitors since Horus Heresy and treated differently than recently made ones. Lord Beross for example was a Dreadnought of the Iron Warriors and had a Chaos Lord status, or at least a chaos warlord status and led his own army of cultists and space marines.
