  • Episiotomy VS Natural Tear - What You NEED to Know!
    Are you afraid of tearing naturally or having an episiotomy cut for your upcoming birth? Which is better, episiotomy vs natural tear, if you get to choose? Will perineal massage prevent tearing at birth?
    Midwife Kira discusses what's an episiotomy and the difference between tearing naturally and an episiotomy cut. Did you know there are also a variety of ways to avoid tearing naturally and an episiotomy? For example, the idea of perineal massage in pregnancy to prevent tearing is actually a myth, however it does have its own unique benefits to some mothers. Also, alternative pushing positions and natural birth can prevent tearing. In this video, we will give you a visual on why alternative pushing positions will help, and how to prevent episiotomy.
    Don't forget to watch the episiotomy demonstration to get a visual of why, without good perineal support, a episiotomy may not be the best option for your vaginal birth. There are times where an episiotomy may be necessary, so stay tuned to find out!

    This video really appeals to me! My first was a hospital birth of a 8lbs baby. Epidural, episiotomy and tearing. My second baby in a birth centre with a midwife, an unmedicated waterbirth of a 11lbs 8ounces baby. No tearing ! I was so happy!

      My advice is put some water or witch hazel on a pad (just enough to make it barely damp) and throw it in the freezer. This acts as a natural icepack and can relieve pain. Also use a squeeze bottle, no wiping. Get some hemroid cream for your bum. Epsom salt baths are great for cleansing. (I tore naturally. This helped a lot)

      Kæla Brown It’s your body your choice but women do hemorrhage after birth (like I did). I lost 1500ml of blood, my friend lost 2000ml I thank G-d for modern medicine and the fact that I was in a hospital. I was pro home birth but my family convinced me to deliver in a hospital and I’m so glad I did.

      Our bodies can do this! Midwifes are experts at birth, but doctors are a jack of all trades. I feel much safer under a midwife’s supervision. Also, she will check in with you a few times after delivery to make sure everything is still good and you don’t require medical intervention. :)

    I like your approach to an episiotomy. That's exactly how I want it to go for me, no episiotomy unless to save the baby.

  • @NikkieRoxxx
    @NikkieRoxxx 3 роки тому +41

    overhere in the Netherlands, they dont do episiotomies. They let woman deliver on a special ' bench' with a whole in it, in sitting/squatting position. They let them tear, and repair the tear. The tear is how NATURE wants you to open if you need to. Cutting artificially with a blade thru the skin fibers, is painful, will heal painfully and is often seen as mutilation.

  • @afiiik1

    I had such ban flashbacks and basically most symptoms of PTSD after my first delivery because of the episiotomy that the midwife did. My daughter had a stellar apgar score so I doubt that she was in danger. The midwife was angry with me because I was "taking to long" "didn't push enough" she talked me into lying on my back, she didn't like that I refused pitocin....
    The episiotomy hurt so bad, she didn't even warn me, she didn't say a word after, she took my daughter away from me and only brought her all bundled up....
    I paid for a private midwife for my second child and it was the best decision I ever made ❤️

    I agree against the perennial message. I am soon to have my ninth baby, all of them natural. It was with my fifth pregnancy that I was told do do perennial messages to help the stretching. It made it worse. First of all, you are supposed to stretch your perenium until there is a slight sting. All that did was bring to mind the feelings of crowning all over again, and psychologically did me a great disservice when it came time to labour. Secondly, you’re Perinium isn’t meant to be stretched before birth. The hormones that are present at birth are what makes your undersides stretch to do what it does. I am now firmly convinced that trying to Prestretch your perinium without those hormones present is only breaking those muscle fibres down there and they have to repair themselves stronger from the abuse, thus actually hindering the stretching during birth. It was during my fifth labour that they told me I had a mild tearing. Have never done it since, and all my labours that I haven’t done it, there has been no tearing.
    I would like to include that I also tried to have many baths with oils, especially in the last trimester with olive oil, or avocado oil, which is good for skin elasticity. Or just rub oils directly on. And stay hydrated. I usually birth on my back, but tilted to my left side as much as I can.

    My sister was induced and had her baby in the hospital. Without asking or letting her know the doctor was preparing to do an episiotomy even though there was no indication that it would be necessary. She had also specifically put in her birth plan that she did not want one. She was lucky her personal midwife was there and caught it before it happened.

    The sad thing is that in out country (Croatia) they don’t let us give birth in any position except lying on our back... and the second thing is that they force you to have an episotomy..
    When i had my first baby, the most hurting thing was recovering afther the episotomy, i couldnt do NOTHING for 4 weeks..just lying on the bed.
    Now Im 8months pregnant and the only thing I’m thinking about is the episotomy 😞

      Tearing and the need for episiotomy is totally avoidable, if you have 'birth canal widened to 10cm diameter' prior to the start of birth. Oxford wrote up the method and the birth trainers Aniball and EPI-NO Delphine Plus achieve this also. Please view UA-cam video 'Opening Doors for Baby to 10cm' and 'Birth Explained for 16+' Thank you.

    Thank you I’m expecting my second child & have that fear of tearing or being cut in my neither region (like everyone) so this video was very informative & affirming. I only had a slight natural tear (that didn’t require any stitches) with my first - & I had an epidural & delivered her on my back - but I did have an excellent midwife who guided me so that I avoided a bad tear. As it’s a 17 year gap for my second I’m worried I could still tear if my baby is born too quickly or if I get a poorly experienced midwife/Dr this time round so I appreciate any tips I can get! I also ordered the epi-no (ani-bal device) to train & stretch the muscles in the vaginal region to avoid tearing also!

Then I learned that tears are normal and 90% of women will have some in labour. The great thing is that the blood flow to the vaginal area is so good that any cuts or tears heal up fast. @@meryem9664 Good luck! I think trying to relax about the whole thing will make it easier for your body to relax as well and do what it is supposed to do. That's what the nurses were telling me the whole time: "You're not relaxed! You need to relax so your vaginal muscles can relax as well" It was a whole thing. Then, in the end, they needed to vacuum my son out asap because his heart rate was getting dangerously low, and having assisted delivery with forceps or vacuum increases the chance of needing an emergency episiotomy. Visualize an easy birth :) I'll be sending you good vibes

Also that a clean cut (episiotomy ) heals quicker. Well, I have just been schooled😮. Now it makes sense how a tear would heal better. I had an episiotomy with my first baby who was 7 weeks premature. There must have been some reason the specialist did that, perhaps to get my 3lb 5oz baby out quickly. I had a lot of stitches. My baby was fine and grew rapidly in the incubator and I brought her home when she was 6 weeks old. The stitches were sore but I had salt and Dettol baths to help them heal. The outer stitches healed quite fast, but the inner ones took 5 months to dissolve. I could still feel them there until then, and the inner scar was painful and sensitive to the touch for a long time.

    Can you please reply back or anyone?
    What if your doctor does an episiotomy without your consent and also didnt even make sure you were numb and also the baby wasnt even in distress or stuck. (I have the doctor notes). I lost so much blood from the cut that I went to ER 4 days after having my baby and I was 1 point away from a blood transfusion. The doctor didnt even ask for permission or let me know that he was going to put his whole arm inside me and then proceeded to stitch me up without even making sure I was numb & I overheard him telling a nurse that I had an episiotomy and when i got discharged they had no instructions on how to care for it and when I saw another obgyn she told me how yo take care of it and to do sitz bath and it honestly has affected me so much it made my post partum depression worse and i also started seeing a counselor. What should I do?

      Look into hypnobirthing. Your only job during labour is to focus on your breathing to stay calm. Anything you do to help naturally increase oxytocin will help labour progress. Fear and stress can halt your labour and make it more painfull. Anything that helps you relax will help reduce pain and help you meet baby quicker. Be in a setting where you feel safe and comfortable, surrounded by people you trust, love and want with you. Have dark lighting it will help rise your melatonin levels, help relax you and baby when they arrive. Trust your instincts. You need to change your mindset on what pain is. Your contractions will never be stronger than you are. Think of how powerful they are, rather than painful. It is YOUR strength you will be feeling.

    Before i deliver my baby,i read that in usa have new guideline they dont practice episiotomy like before as natural cut heals faster than episiotomy..and like u said in yr video when they do episiotomy,it tend to rip more..so when im about to deliver,i heard midwife wanted to cut me so i told her NOOO...but she didnt respect my will,she asked me if i want to have big tear by preventing her to cut myself.i wanted to tell her how she know i would tear myself,although i knew episiotomy is same like 2nd degree tear,that time i couldnt argue with her as im in terrible pain,i couldnt talk much,just bearing the pain and out of sudden she cut me although i didnt give permission..
    My episiotomy only heal after 4 weeks..not 2 weeks like other people..and everyday in confinement i took picture of it to see the healing process..most of midvives think they are helping the mother by cutting their vagina during delivery but they didnt aware the episiotomy can give problem to the mother for her whole life...
    During confinement period,i always feel painful there and this make me get postpartum blues ..due to the pain i feel reluctant to breastfeed my newborn,i refused to eat bcoz i scared to pass motion if i eat..
    The worst part of this episiotomy thing,i noticed some inner flesh inside my vagina coming out...so when i have S.I with my husband,i bleed..i dont think its normal to have the inner flesh coming out but i dont know what should i do..my baby now 6 months old and it also has been 4 months i didnt serve my husband..this episiotomy thing really affecting myself,my emotion,and my sex life..

      Oh gosh, I am so so sorry to hear you had such a bad experience. Although episiotomies do take longer to heal, 6 months is not the norm. Please reach out to a local OB or midwife to inspect your laceration and see if you would require any care to repair and heal. Good luck to you 💕

    This is an excellent explanation and I wish I would of seen this before having kids. I delivered twins vaginally and had an episiotomy against my consent. Was told after the fact. Was not happy about this. With my 3rd baby I was against having another episiotomy and made that clear but was pressured by the doctor to have another one. Either it didn't heal properly or wasn't stitched well afterwards but now I have to go in for surgery to have the perineal muscle and skin fixed. Ladies do yourself a favor and do not agree to this. Better yet work with a midwife.
    A follow up question for you regarding birthing on your back, do you think this can affect whether or not you have a prolapse?

    • @sunnylife7203
      @sunnylife7203 3 роки тому +3

      @@seminrezayi2186 No, not yet. I have to wait for the surgery until I am 100% done having kids. The surgery is pretty straight forward, my OB-GYN told me they would just go in and close up the tissue structures that weren't closed properly after my episiotomy. Hospital births in my experience are not conducted in a normal manner. If I have any more babies I am going to use a midwife.

    • @seminrezayi2186
      @seminrezayi2186 3 роки тому +1

      @@sunnylife7203 oh okay thank you for replying. The reason I asked because the same thing happened to me. I had vaginal delivery 3 weeks ago and somehow my stitches fell off at the first week, it didn't resolve so muscles, skin didn't reattached. The doctor suggested surgery and also gave me two options, either I do surgery after my 40 days and then if whenever I want to become pregnant again I choose c-section OR I give birth to how many kids I want and do surgery one and for all. What I'm wondering is that if I do surgery now I can't do vaginal birth again? Or why it doesn't get restitched? If you have info 😀

    I always felt like birth flat on the back or sitting up made no sense to me physics wise. That it was almost going against the grain? I always felt like being as close to squatting and standing made more sense. It's so interesting you mention this because it's SO common for people to deliver on their back sitting slightly up. I suppose that's because many women get epidural too?

    THANK YOU for making this video. I'm a med student and have always abhorred the thought of doing an episiotomy, more so because in my country it is a pretty common practice. What you mention about the vagina's orientation is so true! Why do we keep trying to make moms give birth on their backs when they could be standing up with some support or maybe even on all fours. I'll look into any studies that address giving birth while in alternate positions, thanks!

    • @slee7991
      @slee7991 4 роки тому

      Late responce and not even a professional one but for anyone else scrolling.. -i dont think it would be an "even worse" tear than the first but my midwife informed me you are definitely more susceptible for that scar tissue in the first tear, to tear again:( im hopeful though. Maybe remember to use counter pressure on the next one, keep your mouth in an open position or panting instead of closed (ive seriously heard that helps since everythings connected) and apply a moisturizing oil (like vit e) to the perineum area for a few months prior to giving birth. Lol at least I've got nothing to lose to try all these things...

    • @CajunStork
      @CajunStork  2 роки тому

      Have you ever studied episiotomy vs vaginal lacerations in labor? Tearing in labor isn’t like a laceration elsewhere in your body. The vagina heals beautifully when jagged but with all the pressure on the perineal floor it easily gives with a straight wound. It’s not the same.

      The purpose of episitomities should ONLY be used in times of distress. As a mom myself, you’re welcome to cut me if it helps save or protect my baby! I am advocating only against the routine or unnecessary use of them for provider convenience. I’m sorry your baby was going through that :(
