Glad I found your post, our diagnosis are amazingly similar. I am a little frustrated because my oncologist decided to radiate the prostate bed even though I had a suspicious spot on 5th rib left and right after PSMA scan. I had very high Gleason and after radical prostatectomy no lymph nodes but some cancer in seminal vesicles. 2 years with zero PSA and 3 months ago PSA went to .15, month later .17 and second test .20. 37 radiation treatments of prostate bed and I just finished 4 of them. I am 60 years old, was diagnosed at 57 with no symptoms. PSA big jump during annual physical to 8 from 2, lump felt, biopsy, high gleasons, surgery. Thanks for sharing your journey.

God bless you Mark. I watched your videos prior to my DaVinci surgery in April 2022. Coming up on my 1 year anniversary of it. Thank you for sharing and I hope this treatment takes care of it for you. My father was diagnosed with metastatic prostate cancer in 2020 even though he had regular PSA checks, so he must have gotten an unusually faster growing cancer. I tell him that his diagnosis saved my life because I am 54. My post surgery pathology stated that it abutted the wall. So far no recurrence but will stay vigilant. Take care of yourself.

Mark, I'm so sorry that you're dealing with recurrence. I've been watching your series because I recently had a close call with prostate cancer. Based on an MRI, my doctor said it was 60% likely I had it. But then a biopsy that sampled 16 sites didn’t find anything. My father had prostate cancer. In fact, his was treated with proton beam therapy rather than surgery (at Loma Linda, when the treatment was very new). That was at least 15 years ago and he has not had a recurrence. He’s now almost 90. Given that both he and I began having BPH symptoms in our thirties, I assume I’ll eventually have prostate cancer and, unlike my dad, I plan to just have the darn thing removed. Your series has been super helpful in addressing the unknown and the fear that goes with it. Ironic that, just as I was finishing a binge watch of all your episodes, this final one pops up. Literally within 30 minutes of that moment. Was planning to write you anyway, but of course this news changes the content of my comment. Since I received so much hope from your cancer journey, I’m probably sharing some of your complex feelings around its recurrence. If it can happen to someone as optimistic and mindful as you, it can happen to anyone. Will continue to watch your channel for updates. Hello! I pray & hope that u don't have Prostate Cancer. Since you are very knowledge person in this matter. I wanted to ask you few questions that my father has been suffering from Prostate Cancer as well. He had surgery as first step of treatment & results was good after surgery. But few yrs Cancer reoccured. Then he was put under Androgen Therapy to shrink & control it. Finally, he undergone Conventional Radio Therapy since there wasn't Proton Radio Therapy in our country that time & also it wasn't affordable. After Conventional RT, his PSA has been decreasing for a couple of years. Hoping & praying that he's completely cured. So, my questions are whether there chances of me getting this disease. Currently, my age is 30. What measures/ tests should I undergone to keep a check on it. Also, How can an outsider take Proton Beam Radio Therapy in USA ?
@@contemplator1993 Hi Chethan. I'm not a doctor. I don't know enough for you to ask my opinion. I'll pass along some advice my father gave to me when I was your age. He said, "Every man past a certain age should be on a first name basis with a good urologist". I've followed that advice since I was in my mid-thirties.
I don't remember. Fortunately, it's looking more like it might not be cancer. My PSA came down in response to other treatment but not enough. So we're still figuring it out. @@Midnitexowboy
Was the Biopsy guided bye the MRI results (I think it's called Fusion Biopsy). I've just been through this. They took 51 samples, of which 8 were guided by the MRI and the others on a basic pattern. The MRI guided ones told a much worse story than the others that barely found anything.

One of the unique aspects of prostrate cancer is that it can be hit with curative treatment after a recurrence. And it is not unusual to do so. The stable and low PSA and singular site of spread tells me you'll be successful in eradicating this.
Hi Mark. Thx for doc your journey(s). You are an inspiration in your fight. I had RP surgery 8 weeks ago with 1 positive margin and first PSA of .02 at 4 weeks. I am preparing for possible SBRT down the road. I am surprised of your reocurrence at 7 yrs. I keep telling myself that so many people have bigger problems than me (very true of course). Mark, I've just received my Prostate cancer diagnosis on the 7th January 2023. I'm 61 have a PSA of 13.6 with a Gleason of 3+4. My Biopsy had 5 positive cores of 6 on the right hand side. I've spent a week trying to decide which treatment to have, RP or SBRT. I only binge watched your videos yesterday which made me decide on surgery. Mark. I am 7 weeks post prostate cancer surgery here in the UK and things to date have gone well. I have taken great interest and comfort watching your videos and have found them very informative and helpful on my own journey. This video popped up on my uTube feed today and it stopped me in my tracks. I felt quite emotional and shocked. In fact I stopped what I was doing and took myself to a quiet place away from my family to watch it. I have not even considered the risk that post surgery the cancer can return. Hi Mark, sorry to hear of your recurrence. My PSA remained undetectable for the first 4-years after my prostatectomy but is now starting to slowly rise. Therefore, I'm keeping a close eye on it every 3 months. I wish you a successful secondary treatment, and thank you for recording your experience. And I love your positive attitude. It gives us all hope in the event we face a recurrence.

2 1/2 tears ago I had advanced Prostate cancer, found 1 area outside that had it. Extensive radiation and lupron injections for 18 months. PSA has been steady at 0.014 for the whole time until today read 0.038. See Dr. next week, very worried. Thanks for video.
PSMA “PET” scan uses a radioactive dye to “light up” prostate cancer (head to toe). I had it recently and confirmed CA was confined to prostate. Bad news it showed an unrelated AROTIC ANEURYSM in my ascending (major) aorta. Actually that was good news because it was a ticking time bomb that could have killed me within a year or too. Had aortic repair/replacement surgery 5 weeks ago and am good to go. I start proton beam therapy he prostate CA in 2 weeks. So getting prostate CA literally saved my life (cardiac wise).
Wow I live in Knoxville. I was diagnosed late 2019 and had the robotic surgery at University of Tennessee Medical Center Feb 2020. Last PSA is demonstrating an increase and more than likely need the same treatment as you in the future. If I had know you were in the area I would have bought you lunch. Can I ask you a question , after surgery in 2020 did you have zero PSA ? you say you're PSA is increasing and you are likely going to need radiation treatment like Mark , have you had PSMA cat scan and are you being told treatment should be started sooner rather than later ?
@@mikescott6519 Great questions Mike. Post surgery psa was 0.02. The past year psa seems to have stabilized at 0.10. No treatment is presently planned. Surveillance is the plan to determine if psa continues to increase or remains stable. My understanding is the PSMA CT scan generally will not detect under a psa of 0.2. So it is a wait and see approach. Medical advances in prostate cancer diagnoses and treatments has been amazing the past three years.
There are quite a few RARP videos on UA-cam, but I must say, Mark, yours is the best. Not only super informative but the way you present the information is superb. By the way, was CyberKnife ever an option offered by your doctor? An reason why you chose Proton Therapy over CyberKnife?

I treated my cancer in 2017 with Proton Theropy in Hampton Va. No side effects or pain. ED: Can not help as that has been gone a while. Now in 2024 it is back. Proton people want nothing to do with me. Did not know all the after treatment rules. Seeing Dr. Freeze soon.🦇
Hope everything turned out Mark cuz I do watch all your videos but if I was you I'd probably would have done ... Nothing. I would have waited a little longer it's such a slow grower. Just saying Mark Hope everything is well Right now it's 2024 August. I'm about to take PHOTON radiation and I am going to REFUSE ADT treatments. The proton facility is ONLY 35 minutes away so it was a very very difficult decision. Between both facilities.

That's a bit rough, having it re-appear after getting chopped. I just got diagnosed last week. So now have to decide to chop early or try and delay the inevitable
Hi Mark Following your ct/pt PSMA scan. Did you then have a bone scan or MRI chest scan to confirm that there is prostate cancer in the bone.As I have recently been diagnosed with Gleason 9 prostate cancer had a ct/pt PSMA scan showed a bright spot on the rib I then had a bone scan which was clear.I’m now having this week MRI chest scan to further confirm if there is any prostate cancer on the bone. I have also just started 18 months of Hormone treatment. Thank you for sharing your prostate cancer journey. Oh just remembered before choosing SBRT radiation treatment did they do a DNA cheek swab to establish your toxicity level and your tolerance to SBRT vs lower dose more treatments This is done to minimise after effects.
An added query from me : 1. What was your Gleason score of the Prostate gland removed ( pathology report) . 2. Why didn't you go for SBRT radiation which is again for 5 days only and a recent trend - as I see from presentation of Dr Eugene Kwon of Mayo clinic Gopal India
I am interested in this also. My husband was just diagnosed with pc, gleeson score 6, psa 9.45 (or close). He was 100% ready to do the radical surgery after the diagnoses but 4 days later thinking about just doing observance. This scares me to death. His father passed at age 56 from prostate cancer. But his mother lived to a few days shy of her 93 birthday. He is an otherwise healthy man.
Considering Proton Radiation to treat my Prostate cancer instread of surgery. Only thing afraid of is the Lupron therapy side effects (hot flashes) , which takes away your testosterone which the prostate cancer feeds on. The Lupron weakens and shrinks the prostate cancer prior to Proton radiation.
Hey Mark, what do they do to appropriately protect your soft tissue behind the rib being irradiated? Is lung possibly falling into the radiation field a concern? I'd appreciate your input on this. Thank you for your post.

MARK! Thank You. I was just diagnosed by my Urologist with "Possible ReCurrent". 8 years ago at .2-.3 and recently hit 2.7 suddenly. I go in for my PSMA PET Scan this Wednesday. Thanks for the Proton insights... Did you have a Biopsy confirmation? Was Cryo considered....? What insurance covered this/ Medicare? So many questions I have. Thank you again!
Mark, the PSMA Pet Scan is certainly a game changer and you’re going to zap the now-identified PSMA-avid PC spot on your ribs. In my case (RP scheduled in three weeks) my doc used it as part of staging. While I’m not sure if it is sensitive enough to completely rule out any extra-capsular extension (ECE) that might lead to positive margins, that is the dream. It does apparently rule in/out other initial staging concerns such as distant spread, SVI, or lymph node involvement. My surgeon hasn’t raised the idea of salvage/adjuvant radiation following RP (immediately or down the road) I suppose that depends on post -RP pathology. You are so right regarding medical advances, etc. Best of luck, no doubt you’ll zap this site!

Hello Mark. I’ve recently been diagnosed with a Gleason 7 (3+4). I still can’t believe it as I have no symptoms. I’ve been asked to choose between surgery and radiotherapy. You have had surgery, and now doing radiotherapy. Knowing what you know now, would you have chosen radiotherapy instead of surgery back in 2015 ? Wish you all the best.
Proton radiation therapy was initially introduced mid 1990's at Loma Linda Chiledren's Hospital to treat brain cancer in children. Proton radiation is much different than conventional radiation. Please review online background comparing the several radiation treatments. Normally, proton radiation is done with small localized prostate cancer. Good luck..
JR, yes, you are correct. I am fortunate to have one of Loma Linda's specialists working on my treatment. While proton radiation is an exceptional form of radiation for spot-treating cancer, they have since developed a method to create a "cloud" of radiation in a broad area, such as the pelvic region and lymph nodes, to maximize coverage. Because of the Bernoulli effect of proton radiation (where the strength of the dose intensifies at the target location), this "cloud" will be at full strength, and the surrounding bones, muscles and tissue receive a considerably lower dose.
thank you thank you i just got diagnosed have to meet with urologist monday to see what needs to be done, I am very sad and scared ofcourse wondering should i do radiation if thats an option of just have it removed. Praying its nothing worse but this is bad enough for me to handle I dont no if I can do it. And how to I find the best place to do it. I stayed local, I'm 57 years old living in Rochester and I had my Prostate removed 1 1/2 yrs ago at U of R Strong . The cancer got outside of the prostate a little they told me and it was an aggressive form Gleason score of 4+4 (8) PSA was 8. In my case removal was the best option because it's harder to remove tissues if warranted after you've had radiation the doctors said. Turned out after prostate removal my PSA went from 8 to .08 (they'd like it around .02 or lower). PSA started climbing over the months .08, .12, .14, .19, .27, .32 and .44. I had CT scans, MRI, nuclear scan test, 2 pet scans during the PSA increasing levels. I did 38 sessions of radiation (wasn't too bad) and hormone therapy that consists of 3 shots total over 6 months (side effects I have so far hot flashes, no sex drive, itchy patches, swelling of my right foot). I just did a PSA blood draw today and it was .02! Whahoo! You may be lucky and only have to do the seed type radiation... Either way, hang in there. Your mental state has a big effect on your healing. I'm religious so my faith and family was my biggest help along with my sense of humor. I get my last hormone (lupron) shot in 2 days! Did your Psma scan show some new oligomer spots after these courses of proton beam radiation? You got radiated again one year later after this treatment with 40 days radiated !
Hi Mark, I had my prostate removed in Aug 2019 with a 9 Gleason score that was downgraded to 8 post surgery. 6 months later PSA was rising so 38 radiation treatments. Fast forward to Friday my PSA was .03. I did a follow up independent lab test and it yielded .22. My oncologist wants to wait 3 months and get another reading before discussing treatment options. What made you decide on Proton therapy, it seems like a no brainer but there are so many dang options available. What was your decision journey like?
Ken, Proton therapy is less damaging to the areas around the prostate bed, so is a preferred method of treatment. However, many insurance companies will only approve IMRT, which is the more traditional photon radiation (still very effective).
I wish you the best brother. I'm on day 25 of 39 for salvage IMRT having failed surgery two years ago. This disease is a bitch. The mainstream portrayal of it is completely inaccurate.
It's horrifically expensive, and insurance companies will often deny it, and suggest other type of radiation. But, you doctor can coax them into a approval, if they try hard enough.
Have you heard of melatonin for killing cancer? Or fenbendazole? I’ve heard nothing but great things about these 2 treatments for killing cancer! They both have a Facebook group! Amazing stuff! God bless and I’m praying for you!!❤
Hi Mark from Canada. I received my biopsy results yesterday Jan 19, 2023 and the results came back as mid grade prostate cancer. I have another meeting with my specialist in two weeks to discuss next steps! Did you have any pain in your ribs prior to the possible re-currence? I only ask as I have had some pain in my lower rib area for a long time. I was recently sent for a Nuclear Medicine bone scan which didn't show anything in the rib area, so I'm just hoping that they didn't miss reading anything.
Hai Mark I am 74 , had a Robotic prostatctony in 2018. My PSA is rising - but Ga PSMA PET CT is not able to identify . I will repeat the scan after 3 months I have been following your 2015 videos. The fact that the scan shows in rib and not in Prostate bed indicates Metastatis . Have u taken Harmone ( ADT) before the radiation . GOPAL INDIA Did
Hi Mark. I just had my robotic radical prostatectomy yesterday. But late last year I was watching your videos already when I learned I had this cancer. I am sorry to hear about the recurrence but am comforted by how you handle the situation. I asked my surgeon before I had my surgery as to what will be the next move if my PSA started increasing again, and he told me it will be hormonal therapy or anti-androgen treatment of that sort. I threw the question to him, why not radiation? He said NO. Did your oncologist discuss with you anything about hormonal treatment? I will include you in my prayers for healing.
So you sit on a possible reoccurrence a possible reoccurrence but not 100% reoccurrence OK man well I don’t know that I will just go and have proton therapy on a possible reoccurrence
Everyone is different and the types of treatment they choose to get or not should be respected. Mental health is a big part of cancer. Some women decide to remove the entire breast instead of a lumpectomy...personal choice.
Nothing wrong with proton therapy but, 1. It isn't new 2. It hasn't been proven to be superior to photon therapy either in terms of efficacy or lessened toxicity. In fact, some studies have shown IMRT to have less toxicity. The outcomes are the same. Proton therapy was one of those therapies that looked awesome on paper but real world clinical evidence has not supported that initial hypothesis that it would be a superior form of radiation.
Thank you so much for this comment of yours I really needed it right now! Because I am debating on photon or proton for my prostate cancer. I chose photon. The proton facility is only 40 minutes away versus 20 minutes. I just would rather like the convenience of being close. The winter months are coming in Florida is notorious for traffic. So I'd rather be closer to the treatment facilities. And I liked the oncologist and nurses and whole atmosphere a little better at the photon place in Boca Raton Florida. Very tough decision. But your comment really encourages me that I did choose correctly. And I certainly did very much research.
@@ricknowak4582 SpaceOAR is for primary treatment when one has a prostate. He is in the salvage phase so no. (He had surgery to remove his prostate as his first line treatment).
Come on Mark , when the Doctors removed your cancerous prostate they also left cancer cells in your body that obviously spread to another part of your body. . This is a common occurrence when dealing with cancer surgery. Just ask any oncologist it’s as simple as two plus two. Your recommending surgery yet all surgery has done was complicate your cancer diagnosis.
David, I have a lot of compassion for anyone who has to go through this process. The anger should be directed at the medical care received, and not at the proven medical science. This is why I tell everyone who is about to get treatment for prostate cancer to do their research and speak to multiple doctors for a consensus.

I had very high Gleason and after radical prostatectomy no lymph nodes but some cancer in seminal vesicles. 2 years with zero PSA and 3 months ago PSA went to .15, month later .17 and second test .20. 37 radiation treatments of prostate bed and I just finished 4 of them. I am 60 years old, was diagnosed at 57 with no symptoms. PSA big jump during annual physical to 8 from 2, lump felt, biopsy, high gleasons, surgery. Thanks for sharing your journey.

My father was diagnosed with metastatic prostate cancer in 2020 even though he had regular PSA checks, so he must have gotten an unusually faster growing cancer. I tell him that his diagnosis saved my life because I am 54. My post surgery pathology stated that it abutted the wall. So far no recurrence but will stay vigilant.

Mark, I'm so sorry that you're dealing with recurrence. I've been watching your series because I recently had a close call with prostate cancer. Based on an MRI, my doctor said it was 60% likely I had it. But then a biopsy that sampled 16 sites didn’t find anything.
My father had prostate cancer. In fact, his was treated with proton beam therapy rather than surgery (at Loma Linda, when the treatment was very new). That was at least 15 years ago and he has not had a recurrence. He’s now almost 90.
Given that both he and I began having BPH symptoms in our thirties, I assume I’ll eventually have prostate cancer and, unlike my dad, I plan to just have the darn thing removed. Your series has been super helpful in addressing the unknown and the fear that goes with it. Ironic that, just as I was finishing a binge watch of all your episodes, this final one pops up. Literally within 30 minutes of that moment. Was planning to write you anyway, but of course this news changes the content of my comment.
Since I received so much hope from your cancer journey, I’m probably sharing some of your complex feelings around its recurrence. If it can happen to someone as optimistic and mindful as you, it can happen to anyone.

Hello! I pray & hope that u don't have Prostate Cancer. Since you are very knowledge person in this matter. I wanted to ask you few questions that my father has been suffering from Prostate Cancer as well. He had surgery as first step of treatment & results was good after surgery. But few yrs Cancer reoccured. Then he was put under Androgen Therapy to shrink & control it. Finally, he undergone Conventional Radio Therapy since there wasn't Proton Radio Therapy in our country that time & also it wasn't affordable. After Conventional RT, his PSA has been decreasing for a couple of years. Hoping & praying that he's completely cured.
So, my questions are whether there chances of me getting this disease. Currently, my age is 30. What measures/ tests should I undergone to keep a check on it. Also, How can an outsider take Proton Beam Radio Therapy in USA ?
@@contemplator1993 Hi Chethan. I'm not a doctor. I don't know enough for you to ask my opinion. I'll pass along some advice my father gave to me when I was your age. He said, "Every man past a certain age should be on a first name basis with a good urologist". I've followed that advice since I was in my mid-thirties.
What was your PSA initially
I don't remember. Fortunately, it's looking more like it might not be cancer. My PSA came down in response to other treatment but not enough. So we're still figuring it out.

Was the Biopsy guided bye the MRI results (I think it's called Fusion Biopsy). I've just been through this. They took 51 samples, of which 8 were guided by the MRI and the others on a basic pattern. The MRI guided ones told a much worse story than the others that barely found anything.

It is crap that it can come back after surgery but good to know what can be done if this reoccurs. Big thanks to you for your insights.
One of the unique aspects of prostrate cancer is that it can be hit with curative treatment after a recurrence. And it is not unusual to do so.
The stable and low PSA and singular site of spread tells me you'll be successful in eradicating this.
It is unfortunately very common as I can attest to it personally. 40% of men have a recurrence after surgery.

Don, did you get a score after surgery of 0.02 or 0.20. It makes a huge difference.

I have taken great interest and comfort watching your videos and have found them very informative and helpful on my own journey. In fact I stopped what I was doing and took myself to a quiet place away from my family to watch it. I have not even considered the risk that post surgery the cancer can return. 2 1/2 tears ago I had advanced Prostate cancer, found 1 area outside that had it. Extensive radiation and lupron injections for 18 months. PSA has been steady at 0.014 for the whole time until today read 0.038. See Dr. next week, very worried. Thanks for video.
PSMA “PET” scan uses a radioactive dye to “light up” prostate cancer (head to toe). I had it recently and confirmed CA was confined to prostate. Bad news it showed an unrelated AROTIC ANEURYSM in my ascending (major) aorta. Actually that was good news because it was a ticking time bomb that could have killed me within a year or too. Had aortic repair/replacement surgery 5 weeks ago and am good to go. I start proton beam therapy he prostate CA in 2 weeks. So getting prostate CA literally saved my life (cardiac wise).

Wow I live in Knoxville. I was diagnosed late 2019 and had the robotic surgery at University of Tennessee Medical Center Feb 2020. Last PSA is demonstrating an increase and more than likely need the same treatment as you in the future. If I had know you were in the area I would have bought you lunch. Can I ask you a question , after surgery in 2020 did you have zero PSA ? you say you're PSA is increasing and you are likely going to need radiation treatment like Mark , have you had PSMA cat scan and are you being told treatment should be started sooner rather than later ?
@@mikescott6519 Great questions Mike. Post surgery psa was 0.02. The past year psa seems to have stabilized at 0.10. No treatment is presently planned. Surveillance is the plan to determine if psa continues to increase or remains stable. My understanding is the PSMA CT scan generally will not detect under a psa of 0.2. So it is a wait and see approach. Medical advances in prostate cancer diagnoses and treatments has been amazing the past three years.

I treated my cancer in 2017 with Proton Theropy in Hampton Va. No side effects or pain. ED: Can not help as that has been gone a while. Now in 2024 it is back. Proton people want nothing to do with me. Did not know all the after treatment rules. Seeing Dr. Freeze soon.🦇
Hope everything turned out Mark cuz I do watch all your videos but if I was you I'd probably would have done ... Nothing. I would have waited a little longer it's such a slow grower. Just saying Mark Hope everything is well Right now it's 2024 August. I'm about to take PHOTON radiation and I am going to REFUSE ADT treatments. The proton facility is ONLY 35 minutes away so it was a very very difficult decision. Between both facilities.

Hi Mark Following your ct/pt PSMA scan. Did you then have a bone scan or MRI chest scan to confirm that there is prostate cancer in the bone.As I have recently been diagnosed with Gleason 9 prostate cancer had a ct/pt PSMA scan showed a bright spot on the rib I then had a bone scan which was clear.I’m now having this week MRI chest scan to further confirm if there is any prostate cancer on the bone.
I have also just started 18 months of Hormone treatment.

Oh just remembered before choosing SBRT radiation treatment did they do a DNA cheek swab to establish your toxicity level and your tolerance to SBRT vs lower dose more treatments This is done to minimise after effects.

I am interested in this also. My husband was just diagnosed with pc, gleeson score 6, psa 9.45 (or close). He was 100% ready to do the radical surgery after the diagnoses but 4 days later thinking about just doing observance. This scares me to death. His father passed at age 56 from prostate cancer. But his mother lived to a few days shy of her 93 birthday. He is an otherwise healthy man.

Considering Proton Radiation to treat my Prostate cancer instread of surgery. Only thing afraid of is the Lupron therapy side effects (hot flashes) , which takes away your testosterone which the prostate cancer feeds on. The Lupron weakens and shrinks the prostate cancer prior to Proton radiation.
Hey Mark, what do they do to appropriately protect your soft tissue behind the rib being irradiated? Is lung possibly falling into the radiation field a concern? I'd appreciate your input on this. Thank you for your post.

Mark, the PSMA Pet Scan is certainly a game changer and you’re going to zap the now-identified PSMA-avid PC spot on your ribs.
In my case (RP scheduled in three weeks) my doc used it as part of staging. While I’m not sure if it is sensitive enough to completely rule out any extra-capsular extension (ECE) that might lead to positive margins, that is the dream. It does apparently rule in/out other initial staging concerns such as distant spread, SVI, or lymph node involvement.
My surgeon hasn’t raised the idea of salvage/adjuvant radiation following RP (immediately or down the road) I suppose that depends on post -RP pathology.
You are so right regarding medical advances, etc.

Hello Mark. I’ve recently been diagnosed with a Gleason 7 (3+4). I still can’t believe it as I have no symptoms. I’ve been asked to choose between surgery and radiotherapy. You have had surgery, and now doing radiotherapy. Knowing what you know now, would you have chosen radiotherapy instead of surgery back in 2015 ? Wish you all the best.
Surgery should always go first, because you can not go the other way around.
Proton radiation therapy was initially introduced mid 1990's at Loma Linda Chiledren's Hospital to treat brain cancer in children. Proton radiation is much different than conventional radiation. Please review online background comparing the several radiation treatments. Normally, proton radiation is done with small localized prostate cancer. Good luck..
JR, yes, you are correct. I am fortunate to have one of Loma Linda's specialists working on my treatment. While proton radiation is an exceptional form of radiation for spot-treating cancer, they have since developed a method to create a "cloud" of radiation in a broad area, such as the pelvic region and lymph nodes, to maximize coverage. Because of the Bernoulli effect of proton radiation (where the strength of the dose intensifies at the target location), this "cloud" will be at full strength, and the surrounding bones, muscles and tissue receive a considerably lower dose.

I stayed local, I'm 57 years old living in Rochester and I had my Prostate removed 1 1/2 yrs ago at U of R Strong . The cancer got outside of the prostate a little they told me and it was an aggressive form Gleason score of 4+4 (8) PSA was 8. In my case removal was the best option because it's harder to remove tissues if warranted after you've had radiation the doctors said. Turned out after prostate removal my PSA went from 8 to .08 (they'd like it around .02 or lower). PSA started climbing over the months .08, .12, .14, .19, .27, .32 and .44. I had CT scans, MRI, nuclear scan test, 2 pet scans during the PSA increasing levels. I did 38 sessions of radiation (wasn't too bad) and hormone therapy that consists of 3 shots total over 6 months (side effects I have so far hot flashes, no sex drive, itchy patches, swelling of my right foot). I just did a PSA blood draw today and it was .02! Whahoo! You may be lucky and only have to do the seed type radiation... Either way, hang in there. Your mental state has a big effect on your healing. I'm religious so my faith and family was my biggest help along with my sense of humor. I get my last hormone (lupron) shot in 2 days! Did your Psma scan show some new oligomer spots after these courses of proton beam radiation? You got radiated again one year later after this treatment with 40 days radiated !
Hi Mark, I had my prostate removed in Aug 2019 with a 9 Gleason score that was downgraded to 8 post surgery. 6 months later PSA was rising so 38 radiation treatments. Fast forward to Friday my PSA was .03. I did a follow up independent lab test and it yielded .22. My oncologist wants to wait 3 months and get another reading before discussing treatment options. What made you decide on Proton therapy, it seems like a no brainer but there are so many dang options available. What was your decision journey like?
Ken, Proton therapy is less damaging to the areas around the prostate bed, so is a preferred method of treatment. However, many insurance companies will only approve IMRT, which is the more traditional photon radiation (still very effective).
I wish you the best brother. I'm on day 25 of 39 for salvage IMRT having failed surgery two years ago. This disease is a bitch. The mainstream portrayal of it is completely inaccurate.
What does proton treatment cost assuming no insurance?
It's horrifically expensive, and insurance companies will often deny it, and suggest other type of radiation. But, you doctor can coax them into a approval, if they try hard enough.

Hi Mark from Canada. I received my biopsy results yesterday Jan 19, 2023 and the results came back as mid grade prostate cancer. I have another meeting with my specialist in two weeks to discuss next steps! Did you have any pain in your ribs prior to the possible re-currence? I only ask as I have had some pain in my lower rib area for a long time. I was recently sent for a Nuclear Medicine bone scan which didn't show anything in the rib area, so I'm just hoping that they didn't miss reading anything.

Hi Mark. I just had my robotic radical prostatectomy yesterday. But late last year I was watching your videos already when I learned I had this cancer. I am sorry to hear about the recurrence but am comforted by how you handle the situation. I asked my surgeon before I had my surgery as to what will be the next move if my PSA started increasing again, and he told me it will be hormonal therapy or anti-androgen treatment of that sort. I threw the question to him, why not radiation? He said NO. Did your oncologist discuss with you anything about hormonal treatment? I will include you in my prayers for healing.

How does one get that type of cancer? Is it hereditary?
Most men get it if they live long enough. But there is some hereditary risk of getting more aggressive forms.
So you sit on a possible reoccurrence a possible reoccurrence but not 100% reoccurrence OK man well I don’t know that I will just go and have proton therapy on a possible reoccurrence
Everyone is different and the types of treatment they choose to get or not should be respected. Mental health is a big part of cancer. Some women decide to remove the entire breast instead of a lumpectomy...personal choice.

Nothing wrong with proton therapy but, 1. It isn't new 2. It hasn't been proven to be superior to photon therapy either in terms of efficacy or lessened toxicity. In fact, some studies have shown IMRT to have less toxicity. The outcomes are the same. Proton therapy was one of those therapies that looked awesome on paper but real world clinical evidence has not supported that initial hypothesis that it would be a superior form of radiation.
Thank you so much for this comment of yours I really needed it right now! Because I am debating on photon or proton for my prostate cancer. I chose photon. The proton facility is only 40 minutes away versus 20 minutes. I just would rather like the convenience of being close. The winter months are coming in Florida is notorious for traffic. So I'd rather be closer to the treatment facilities. And I liked the oncologist and nurses and whole atmosphere a little better at the photon place in Boca Raton Florida. Very tough decision. But your comment really encourages me that I did choose correctly. And I certainly did very much research.
Mark did you get the ... "space oar"inserted? If so locally or general. And if you did how uncomfortable was it.
@@ricknowak4582 SpaceOAR is for primary treatment when one has a prostate. He is in the salvage phase so no. (He had surgery to remove his prostate as his first line treatment).
Come on Mark , when the Doctors removed your cancerous prostate they also left cancer cells in your body that obviously spread to another part of your body. . This is a common occurrence when dealing with cancer surgery. Just ask any oncologist it’s as simple as two plus two. Your recommending surgery yet all surgery has done was complicate your cancer diagnosis.
David, I have a lot of compassion for anyone who has to go through this process. The anger should be directed at the medical care received, and not at the proven medical science. This is why I tell everyone who is about to get treatment for prostate cancer to do their research and speak to multiple doctors for a consensus.
