      @@profLidiaRudnicka I do have a lot of regrowth! My 28 month appointment is coming up really soon. My scalp has been around 90% regrowth for over a year and my brows and lashes are ok-could be thicker but they are there.

    I live in the UK and have alopecia universalis, and unfortunately none of the jak drugs have been approved on this side of the pond for treatment of any form of alopecia. The decision was made by the powers that be back in June 23. It's left me feeling angry and dissolutioned and not knowing where to turn next.
    For those of you over in the United States, I wish you all the best with this exciting new treatment and really hope the these drugs work 100% for you ❤

    Hi Dr.Rudnica
    My son 14 years old started seeing improvement from taking Baricitinib . We see black specks all over his head, few on eyebrows.
    He has alopecia universalis
    My question is , how long does he need to be on it? One year will it be enough? How long do people usually be on it?
    Do you lower the dosage before stopping it completely? I would appreciate it a lot if you will answer my questions

    Can Barcitinib work for telogen effluvium which is caused by an autoimmune disease?

      Baricitinib is approved for alopecia areata, atopic dermatitis, rheumatoid arthritis and COVID-19 (in certain clinical situations). In other autoimmune diseases, the doctors are considering off-label on individual basis.

    Thank you for your research-based discussion. I follow your and some other's discussion on this topic, but nowhere I see a very important point to be discussed. Which is, after full regrowth, if someone stops JAK inhibitor, will the small patch come back or the regrowth is sustainable?

      If the withdrawal of the treatment is too early, then the re is a very high probability of relapse within few weeks. Long treatment is likely to give more durable results

      I have alopecia areata only on my beard for the last 5 years. I took all possible treatment available. Hair comes back but falls again after the treatment. I used JAK inhb. for 5months, but had to stop because German doctors are very skeptical about the side effects. I am also slightly diabetic from the time I have alopecia. I have no other blood issue at all. Not sure if the diabetics is the reason for this. Do you have any opinion on this?

    Hi Prof. My new derm said all treatments don’t affect my FFA at all(I’m on Isotretinoin,Dutasteride,Cyclosporine,Prednisone and Plaquenil). He wants me to start Clexane injections once a week(he said there are few studies that show great results on LP). What do you think? He said if it doesn’t work we’ll try Xeljanz. What do you think on Xeljanz for FFA? Big thank you as always!

      Your diagnosis is sure? There are no scientific data for tofacitinib and FFA. If nothing else worked, maybe I would consider

      @@profLidiaRudnicka Yes he’s done 3 biopsies-one from my scalp,one from my arm and one from my right thigh. They all showed LPP. I’m losing eyelashes,eyebrows,leg hair,arm hair, I lost already few inches of my frontal hair line and underarm hair😢😢 What about Otezla? Did you use it in LPP/FFA? I’m on LPP facebook group and they’re saying it is a miracle drug lowering inflammation and even capable of regrowing some of the lost hair

      In regard to indications the two drugs are for two different diseases. Finasteride is for androgenetic alopecia and Olumiant (baricitinib) is for alopecia areata.

    Hello. Do you think that these JAK-inhibitors are suitable to treat regular androgenic alopecia? I saw a video of some studies that were done on androgenic alopecia and the JAK inhibitors works on that as well according to those studies. What is your opinion on this?

      No, according to current medical knowledge there is no reason to believe that they could be effective in androgenetic alopecia.

    Hello Dr.Rudnicka, I just read New York Times article about Olumient being FDA approved. I have AU for about 2 years, after childbirth. And had AT in 2018, after which the hair regrew without treatment (may be only Rogaine). Basically I believe it’s stress related alopecia. But now it’s AU and the hair doesn’t want to regrow very much, but I got my facial hair back, and a little hope on my head, but still it’s 2 years almost. My question is - does my case can be the reason for prescribing me Olumient? I’m desperate and planning to order other jak inhibitor from other country may be… plus cannot find dermatologist specialising in alopecia under my insurance (Chicago suburbs, Chicago area)…

    I am a patient of OCD and taking psychotic medicine last 6 years. My main concern is my hair fall started when I was 14. Now I am 31 years old. Totally bald. Probably Norwood 7. But the problem is I have losing hair from lower and side portion of the head. Maybe retrograde alopecia. And also losing beard.almost 70% of my beard density has gone and losing my beard line. I saw many dermatologist but nothing works. I just want to know is it treatable?

      Is it alopecia areata? If so, then there at least few options of treatment with possible efficacy. I would prefer a non-steroid treatment in someone who has a psychotic condition. JAK inhibitors are just one of the options

    Hi, in the uk, jak inhibitors are not approved for hair loss treatments and I think its only available for arthritis etc.
    Can I visit a doctor in the USA and obtain say 12 months treatment plan of jak inhibitors for me to use back in the uk , if so any suggestions for the west coast ?

      It is difficult for me to suggest. In the European Union baricitinib was approved in 2022 and ritlecitinib is expected to be approved soon. Maybe waiting for a UK approval is the best strategy?

    How affective are Jak inhibitors at treating alopecia barbae I believe it’s called of beard alopecia? My dermatologist gave me a prescription for it but shortly after using it I got Covid which I never had before and it scared me. I haven’t used it since but I’m sure I didn’t use it enough time to see results.

      There are no scientific data on isolated alopecia areata of the beard. We can only extrapolate data from results in scalp hair loss.

    hello doctor i seriously need your suggestion,im victim.of alopecia and its been a month since im taking baricitinib,i havent find any hair follicle to grow but i develop small pores all over my body .what does these small pores mean.is it sign that hair follicles will start to grow through these pores and please let me know how long it takes for hairy follicle to appear on skin

      Hair regrowth with JAK inhibitors may be sometimes as late as after 6 months of treatment

    as someone that has suffered from alopecia areata to now alopecia totalis for 9 years, i’ve done every treatment possible. so far, everything has worked only temporarily including PRP (with both blood, and chemiclas) cupping, homeopathy, aurvedic, steroids, vitamins, creams, oils, solutions. everything has side effects. everything works but for me it was temporary. one of india’s best dermatologist has advised me to take elocon and tofajak, (tofacitinib) i have definitely seen some results, such as thin hair growth around my scalp, but it relapses once my skin gets flakey and if i’m out of the dosage. would you recommend increasing the dosage? or using a different jak? i hope to find a cure for alopecia soon! x willing to try jak’s ruxolitnib, but why it is it so expensive 😩 it’s as expensive as my human hair wig

      It is not recommended to increase the dose above the doctor’s advice, in particular in case of tofacitinibie. Full dose Treatment is usually long

    I’ve had alopecia since i was around 4yrs old started as AA and as i got older it turned into AT. I believe my AT started in my early 20’s give or take. I’m 33 now and still with AT. Just asking for your educational answer but do u think if i were to take any of these JK drugs would may hair growth % be low since I’ve had alopecia for so long?

      @Alfredo and@@MeMe-ht2hd every alopecia is different and nobody knows. In my opinion, the chance for cosmetically acceptable regrowth decreases over the years/decades. In any case, the chances are never zero. Regarding a history of 13 years, I had some patients good with regrowth (on classic treatment).

    Jestem kobietą i mam 26 lat oraz pcos. Ze wględu na pcos mam wysoki testeron (2x górna granica normy). Zdiagnozowano u mnie łysienie androgenowe. Włosy wychodzą garściami od 3 lat, dziennie 300 sztuk. Od 8 miesiecy stosuję finasteryd 5 mg, spironol 25 mg, minoxidil doustny w dawce 0.001 (1mg). Po 4.5 mięsiaca była poprawa w trichoskopii, aczkolwiek leczenie w żadnym stopniu nie zmniejszyło wypadania, tracę objętość włosów, boję się je dotykać. Wypada mi dużo baby hair, takich 5-7 cm, a włosy mam za łopatki. Czy to dobry pomysł, aby zastąpić finasteryd dutasterydem i włączyć Alopexy albo zmodyfikować to leczenie? Wypadanie takiej ilości jest dla mnie bardzo uciążliwe.

      Wszystkie leki, które Pani wymieniła są stosowane w łysieniu androgenowym kobiet poza wskazaniami. Zawsze lekarz - razem z Panią - będzie podejmował decyzję o wyborze leczenia.
      W przypadku finasterydu i dutasterydu szczególnie trzeba pamiętać o skutecznej antykoncepcji i czasie jej stosowania po leczeniu.

    I have total alopecia, I'm on baracitinib for 7 months now and had complete regrowth but 2 months ago my hair started falling again and I lost all of it (I did not stop the medication). What can cause this and do you recommend trying another type of inhibtor in my case?

      Yes it is possible. This should be discussed with your doctor, but yes it is definetly worth trying (if available)

    My dermatologist suggested Tofacitinib 2*5mg.
    My queries are:
    1. For how long I have to take this medicine.
    2. How it is going to impact the immune system?
    3. If i stop this medicine after 5 6 months, what will happen? Is there a rebound. If yes, how can I avoid it?

      There are no known significant adverse effects beyond the time of taking the drug

    thank you for this video. it is very insightful. i have been on olumiant (4mg) for 1.5 months now and i am still losing a lot of hair. however i am also seeing regrowth. is it normal to be shedding on olumiant? is tofacitinib better than olumiant? can i switch my jak inhibitor?

      I would wait for few (4-6) months to have an opinion about the efficacy in your case. In some people it starts working after 4 months or more. Regrowth is a good sign.
      Olumiant (baricitinib) is FDA approved for alopecia areata and tofacitinib is not.
      There are no studies which compare these two drugs

    I was taking tofacitinib from 1.5 years. After full regrow my doctor told me to stop the medication. Now after 6 weeks I am loosing a lot of hairs and again got bald patches. What should I do as my doctor is not understanding.

      I hope that meanwhile your hair regrew. There are many possible reasons for discontinuation of a mediacation.
      The treatment of severe alopecia takes usually few years, but it may be significantly shorter in some cases.

      This has to be decided individually. I would not use it as first line treatment, but in general tofacitinib should be taken at a dose not higher than 2x5 mg

      There are very few data, but yes, we may expect some efficacy in active, progressing LPP. They may theoretically decrease the speed of progression in some patients.

    Do you have to take this your whole life or can you stop taking it once your hair regrows. What is the point in this cure if it needs 1 year to fully grow back but relapses after 8 weeks

      The course of the disease is very variable and unpredictable. In many of my patients, the hair grew back and there was no relapse. But a pessimistic scenario is also possible, unfortunately

    Hi , there are jaks from different brand for example Cipla , dr . Reddys in India , these are very cheap compared to Lilly’s which is expensive , so u think I can substitute other brands in place of Lilly’s ?

      Needs to be prescribed by medical professional and the patient needs to remain under their care.
      I wouldn’t advise just buying off the internet. Also the drug is extremely expensive, in the uk at cost price Novartis sells to the NHS for upto £3000!!!

    Hi professor I thank for your brilliant information. I have alopecia totalis for 22 years now ,11 years ago my dr. Prescribed for me prednisone that is gradually decreased from 5 tabs to 1 tab I got a few hair back but I stopped it .the few hair I got is still alive now but now I having small patch on my beard on my chin I am taking minoxidil 5% and Dr. Also Prescribed for me methotrexate 7.5 MG a week with folic acid but I didn't take it. So plz professor help 😫 should I take it and get better I need advice plz ,,I lost my scalp hair but am not going to lose my beard plz help .I have hair all my body except my scalp .help me for the sake of God 10q

      I am sorry to hear about your problem and that it is impossible to help with individual problems via UA-cam. Methotrexate is one of the drugs used in alopecia areata and it has a good safety profile. I can not predict whether it will be effective or not in your situation. Minoxidil alone (in monotherapy) is unlikely to be effective in alopecia areata.

    Mam i have had an alopecia areata since 5 yrs ,then my doc prescribed me many medicine like sazoo 500 at first ,then my hair patches got cleared but relapses happen again n again ,finally my doc prescribed me tofacitinib for 3 months and my patches recovered totally ,but these days again I saw a small patch at temple side ...i m worried now ...after 5 months ,again relapse ,plz do suggest me what to do now

      I have alopecia totally universal from 14 years .I use ollumiont 7 month without any good result nothing happened .I used after that three month zjelganz without good result .now I use litfullo from three month .I am truying to use rinvoq at the same time with litfullo .is that normal .I take alsoo oral minoxidil 9 month 2.5 mg. Please i need your advice

      I recommend talking to your dermatologist. Approved JAK inhibitors (for AA) are not yet available, but there are some off-label options
