Should it be inserted with cuff inflated or not ?
That would depend on the model of the SGA (RTFM: read the manufacturers instructions!) and personal/institutional preferences. I prefer the cuff inflated as it comes out of the box, but especially for the first generation SGAs there were widely differing manufacturer instructions, ranging from completely evacuated with or w/o bent tip, a bit deflated, up to "as supplied".
