SYNERGY is autofocus lens. As a Professor of Math, with research in Geometry, I did not expect to get a continuous focus from small print to my Colorado mountains. SYNERGY is a miracle invention!! I did one eye on March 22, 2023, and plan to do another, again with Synergy, on May 3, 2023.
@@walterski8377 Before right eye cataract surgery, my right eye was 20/60 and left eye 20/25. A month after first surgery, on April 26, 2023, my vision became 20/20 in BOTH eyes! I told my doctor that I am ready to pay another $2600 to get Synergy in my left eye. He told me "You don't need it." He actually refused to operate because mysteriously unoperated eye became 20/20 as well! Just now, 6 months later, the vision remains 20/20 in both eyes, and the doctor took me off Lumigan (glaucoma drops), and left only timolol. Greeting from Istanbul/Turkey! I must say I am a big fan of your videos. I had PanOptix on my left eye due to a cataract 2.5 yrs ago. Now it's time for the right (dominant) one again due to a cataract. My Dr. was opting in for Synergy. I asked him strictly to have Vivity instead because I want no more or I should say fewer halos/glares etc. and better intermediate with higher contrast quality. The surgery is planned for next week. I am 53 yrs old, still very active in sports (lifting, basketball, running), 6'2 with long arms, and very excited to have Vivity. Thank you Dr. Shannon for your all-educating videos which help globally for your kind information )))) You touch many people without knowing...
Thank you for sharing. The patient's description is so familiar. I had Synergy in both eyes December 2021. TOTAL nightmare so far. The worst part is that it was an elective surgery - $12,000. Now all I want is my contacts and glasses back. MISERABLE.
For presbyopia there is no better than Synergy on the market, perhaps PanOptix will see better from a distance, but PanOptix is not better for near vision than Synergy. I have Synergy and my big problem was presbyopia. PanOptix would surely see better from afar, but not very close. and the distance vision of Synergy does not make me need distance glasses. (it is my experience with Synergy) thanks..!
I had synergy lens implant in both eyes in the summer of 2021. I could not tolerate the lack of distance vision. My distance vision was blurry and even in the daytime nothing was clear and I could not see well enough to feel safe while driving. I determined I was not only experiencing halos at night but also during the daytime. In Nov and Dec 2021 I had iol replacement to Panoptix. I am happy with Panoptix. Since my surgery on my right eye in Dec 2021 I have experienced a retinal tear for which I am now recovering. I lost seven productive months of working, had to go to part time, stopped driving. I had to turn the brightness up on my computer monitor but had too much eye strain. I spent a lot of time in bed with my eyes closed. I will be so happy to have this difficult time behind me. I am 63 yrs old with no preexisting eye conditions except I’m near sighted. I believe sufficient warnings were not available from Johnson and Johnson and the Synergy lenses are a negatively life altering product.
I started watching your videos about two months before I decided to visit an eye surgeon in Indianapolis Indiana and see if I qualified for the interocular lenses. A friend of mine also recently had panoptix lenses implanted in both eyes and had a very positive experience. After visiting the eye clinic in Indianapolis they strongly recommended synergy over panoptix. I originally decided to take the surgeons suggestion and go with synergy. I change my mind after I you decided to no longer offer synergy to your patience. About a week ago I had the panoptix inserted into my left eye and now I plan on having the other eye done in about three weeks. I am really happy with the panoptix and I’m really happy I watched your videos. I can read the The smallest print on the Eye Chart which I have on my phone. My vision has greatly improved.I’m going back to the doctor in a couple days and see officially what my vision is and I will let you know. Additionally I have not put on a pair of glasses since my surgery.
I had Synergy lens implanted in my left, Symfony in my right in December 2021. I'm totally with your patient in this video regarding the difficulties i'm having with the Synergy and trying to figure out how to tell my doctor what i'm experiencing. I'm not certain with the Symfony, but i'm very sure i'm not happy with my expectations now with the Synergy lens. Her description of the leaves in the trees, is exactly my experience. It's just not far distance either for me, at least.
I had the Synergy toric lenses implanted 9 days ago. Just like the lady stated, immediately after (next day) vision was great. I was impressed. I could read the fire station sign about 500 metres away and close up was amazing. Intermediate was also great, even though a bit of fuzziness. Then the following day vision quality reduced a bit. Three days post-op exam showed I had 20/20 vision but it was already getting fuzzy/blurry. Now 9 days after and vision has kept deteriorating now to the point everything except reading up close, is blurry. It is very stressful. I am taking my eye drops as prescribed (antibiotic, anti-inflammatory and lubricating). No lifting. Nothing strenuous. I spend time looking at distances hoping it will help my brain sort out the neural adaptation to the lens. I don't get it. Day after great vision and nine days after very bad vision. I am doing a UA-cam journal of my experience and hope and pray this resolves. I am due to head back overseas end of July and to think it will stay like this (is as bad as I think it can possibly get) would be traumatic. I would need strong prescription glasses to give me back mid to far distance vision. I probably won't be back to get any corrective surgery for many months to a year.
It's been 9 months since Tecnis Synergy implants. Yesterday I had YAG for posterior capsule opacification and capsule bag wrinkling in both eyes. It has been 24 hours and my vision is no better. I am on anti-inflammatory drops and told it can take up to 48 hours for vision to improve. I still have issues with bright daylight where everything outside is very blurry. Sunglasses everywhere, but they only improve things a little bit with visual clarity. I have read that once YAG is done capsule lens exchange becomes more difficult as there is now exposure to the aqueous vitreous inside the eye. My eye health is otherwise normal. I feel like the lady interviewed on this video. I hope things improve. In one month I get a follow up review and they will discuss if LASIK is needed. But the ophthalmologist yesterday expects I will probably obtain close to 20/20 vision and likely won't need LASIK (to correct residual astigmatism). I am not confident it is just some astigmatism causing all these blurriness problems for me. And extreme blurriness on bright daylight. Night time vision improves but still blurry. Just reading up close is the only clear vision.
I will have cataract surgery two weeks from now. I choose Synergy because my ophthalmologist said Synergy is definitely better than Panoptix. He said Panoptix is made by pushing Acrylic into a mold, which will become foggy over time. But Synergy will not become foggy because it is made by cutting Acrylic.
That is a real concern and it is called glistening. Alcon is planning to replace Acrysof PanOptix with Clareon PanOptix that hopes to address the time induced glistening. It has been FDA approved since 12/2020 but not available.
@@biranchibarad4327 I had both eyes done with Panoptix. My near and intermediate vision is very good. My distance is good but could be clearer. It's only been a month for my left and 10 days for my right. I recommend them if you have the patience to work through the eye drops and the neural adaptation needed for these lenses to work properly.
Well, now I'm concerned. I had a Synergy lens implanted in my left eye on 1/20 and am due to have my right eye done this week. So far, my left eye's near and intermediate vision are improving (20/25), but not yet crystal clear. This woman's description of fuzzy edges is spot on. I don't know whether my vision will continue to improve (in both resolution and clarity) over the next few weeks or if how I'm seeing now out of my left eye is as good as it'll get without further tweaking with Lasik. I'd hate to do the Lasik, achieve 20/20, but still not have the razor-sharp clarity I require. I should point out that I'm a pilot and need for each eye to test at 20/20 individually. The razor clarity is my own standard and what I've been accustomed to achieving with toric contacts over the last 40+ years prior to cataracts driving the move to IOLs. I'm also an edge case as a high-myope and my ophthalmologist told me that my choice of lenses was very limited. In fact, the Synergy was the only one that she said would suit my anatomical requirements. She initially suggested that we implant a Symfony in my right (dominant) eye, but I'm very concerned about that. For one thing, I'm extremely attentive to detail and am likely a terrible candidate for any sort of neuro-adaptation, as any difference between the two eyes would be constantly compared. It would bother me all the time like a splinter that I couldn't excise. Also, I don't want any sort of "mini-monovision" as each of my eyes need to test at 20/20 distant individually. I feel as though I need to go through with the Synergy implantation this week on my right eye and just hope for the best. To be clear, I'm not unhappy with my left eye (the near vision is better (and closer) than I ever achieved with contacts), but am hoping that it will continue to improve. I know that you can't comment on my situation in detail from afar, but thought that I'd leave this here for others to read and, at the very least, mull.
Thank you for sharing! I remember your post since a while ago. And wish you fast recovery and adaptation to the lens. I read elsewhere, actually maybe on the older post of yours I mentioned above, a commet of a very nice professor who was very generous and kind to share her experience. She had an initially very good outcome of the Symphony-Synergy combination. I will look her comment up, and reply to you from there if it's on your post, and you can ask her yourself about her experience with this combination.
@@anassyria5176 Thank you - please do. I don't recall seeing her response. The issue I have with dissimilar lens combinations is that they all seem to be predicated on the net binocular result, which is not what I'm after at all. I need to have each eye considered entirely separately, as that's how they'll be evaluated (this is an FAA thing for a 1st Class medical). If each eye is corrected to 20/20 individually, I'm confident that the binocular vision will be just fine. Moreover, I need more than an objective correction to 20/20. It's not enough to be able to make out the letters on the chart - they need to be razor sharp, if only for my own comfort.
Im having cataract surgery this week now I'm having second thoughts any advice as to what lens what lensI should ask the surgeon for crystal clear sharpness for distance? I'm having laser for distance and also have an epiretinal issue in my left eye.
@@jayblue5310 If your priority is sharpness and crystal clewr vision, then go for the monofocal IOL. If you decide you want a nonofocal, please do check out the new RayOne EMV IOL and ask your doctor about it. It has clear distance vision, and the plus is that it maintains some of the intermediate vision due to its smooth defocus curve when compared to the standard older monofocal IOL designs. It's similar to the Eyhance, but said to be clearer in the intermediate range.
You are very detail-oriented and thus perhaps better suited for a monofocal lens targeted for distance. That’s assuming that the need for reading glasses is not a non-starter for you. Remember what Shannon always says: “there is no perfect lens.” Whatever you do, after you’ve had both cataracts done, don’t let them do a YAG laser “touch up” on you until you know that you basically like the lenses. It makes an implant exchange a lot more difficult and risky. But it’s still doable… I think I have videos of it on my channel.
I had Synergy implants done just over a year ago. Objectively I see amazing with 20/20 both eyes. Subjectively I have a hard time seeing details in clutter. Finding things in the utensil drawer is difficult and taking my PC apart is almost out of the question now. Like her I do experience some discomfort and feel as if I see the outer edges of the lense. Overall I'm very happy. Vision is much better than when I began to develop cataracts and I can funtion day to day and work on the computer without glasses.
That is really interesting. I have the exact same thing as far as seeing details in clutter. Great way to put it. I can be looking for something on a cluttered workbench, and not see it when it is right in front of me. It's like I don't recognize what I'm looking at. I got mine in August 2021. One positive note is that seeing up close is amazing.
@@helifan1961 it's definitely a thing. I used to work on computer hardware as part of my job a few years ago and had no issues. Now with every piece i have to examine each part really carefully to to see which piece fits where and often I can't find the right socket it wire. Maybe it's neuralogical overload or something.
@@jquebedeauxjr Sorry to hear that. Mine is as good as I can ever remember, but my distant vision isn't ideal. I got glasses (-.75/-.75, -.25/-.75) which help with distance clarity when I really need it. I don't think the opthamologist really nailed my prescription so I wonder if the necessary pre-work was done (surgery induced astigmatism calculations, A-constant consideration, etc.)
@@jquebedeauxjr My near vision is almost as good as I remember up to about inches. Any closer and things get blurry fast. Reading these comments it sounds like my surgeon at Mitchell Eye Calgary did an exceptional job.
I have panoptix lenses in. Had surgery in June 2022 then had rt eye surgery again hoping for improvement and comfort. Not the case. I would pay double to have never had the panoptix eye surgery. So many issues. But I guess I shouldn't complain I am not blind just miserable and in a pressure strain type pain,24/7. I would give anything to have comfort and decent vision. 😢
I think the tri-focal implant is the very best....so far. $15,000 US is a no-brainer. Worth every penny, and lasts your whole life. Beg, borrow, or steal.
I'd rather put in a lens that gives you A- results 99.9% of the time than put in a lens that gives you an A+ result 95% of the time and an F 5% of the time.
I think you are such a wonderful dr because you will do anything needed to achieve the vision your patient’s are happy with. I can tell from watching your videos that all you care about is your patient...no ego involved at all. My Synergy lenses were implanted 6 months ago...my right eye is not perfect 20/25...but my left eye fluctuates between 20/50 + 20/80. I now have a slight astigmatism in my right eye and a severe irregular astigmatism in my left eye. And now severe floaters in my left eye. I can read but nothing is totally sharp. Distance is not good either but having the left eye being so different from the right eye is the main discomfort. I recently found out that the lens in my left eye is slightly off center...which is surprising because I was told that Synergy lens stay centered better than other lenses. My night vision is not good probably because my overall distance vision is lacking. The halos at night are not as bad as your patient mentioned. And I don’t feel the lenses in my eyes...so I guess that’s some good news for me. I will be having PRK done to my left eye in 2 weeks...hopefully that will solve the problem. But it worries me to hear that you no longer will use Synergy lenses. That makes me worry that these are not good lenses especially that you mentioned the lens had become cloudy in one of her eyes. I hope that never happens to me. Thsnk you for sharing these videos...they are always very helpful.
THIS lady describes MINE as it is in the GOOD eye.. (bad one has retinal reattachment and have to wait 8 weeks to see how it works.. ).. BUT she sounds like me- but mine is a bit worse!!

@ Dr. Wong - I have had Synergy lens in left eye for 2 months and I'm experiencing exactly the same simptoms as the patients in this video. My doctor wants to perform a YAG Posterior Capsulotomy and a LASIK to correct the far vision blurriness. My question is if after YAG + LASIK my vision is still not satisfactory, can lens exchange be done? I understand Dr. Wong cannot give specific medical advice without exhamining the subject but I'm looking for a generic answer like can a IOLX be performed after YAG or should be avoided. Thank you
Thanks for this video Doctor. I had cataract surgeries in December 2021 and got Alcon Vivity lens implanted in both eyes. My near and intermediate vision has improved significantly, but distant vision is still little blurry. Do you think it will improve with time and I just need to be patient?
Hope your distance vision gets clear soon! Maybe your doctor aimed to give you better intermediate and near vision on the expense of some of the distance vision?
Thanks Anas, I got Alcon Vivity lens implanted in both eyes, Vivity is an Extended Depth of Focus lens (kind of multifocal lens) that gives you a very good distant and intermediate vision and good near vision. But in my case, the results are opposite. I met my surgeon today and he said that it takes couple of months for the vision to improve, I should wait for 3 to 6 months.

Shannon do you think refractive surgery can be considered therapeutic and not just stetic surgery? Many people develop intolerance to contacts and can't live a normal live. Doing exercise with high myope glasses is not comfortable and affect quality of life of many. What are your thoughts about it?

I have Symfony in one eye and i wanted multifocal (Synergy) in another in the future. Will it be possible that i will have such problems with Synergy while i dont have problems with Symfony?
Appreciate the video, doc. I was about to get a Synergy lens on 11/22/21, but based on the video you released that morning stating that the PanOptix slightly edged it out, I changed my mind at the VERY LAST MOMENT (right before being anesthetized). I *was* considering possibly going with the Synergy for the other eye, but I think I'll stick with bilateral PanOptix (or perhaps an EDOF w/ fewer dysphotapsias).
@@walterski8377 I went with PanOptix for both eyes and am very happy with the results. I have a small amount of residual refractive error in one eye, so that one can see a bit closer with a bit of a compromise in far vision. The other eye is 20/20 at distance. Between the two I have great vision both near and far.
Are natural lenses removed during cataract surgery ? I got lenses and im not satisfied and im thinking to remove it and try alternative ways but without natural lenses it will not work I gues ? I got multifocal lenses, it helped me a lot but I dont see clear at close distance. Is it possible to revert it now ?
I have Panoptix in left, Synergy in dominant right eye. My question to posters is: 1. Did anyone go with this combo? 2. How is night vision, driving? 3. How is your near distance like reading on Smartphone, Tablet... I have BAD halos, Starburst, Rings in BOTH eyes. Can't say one worse than other. It's been 5 months now. I hate driving at night. I had a synergy implanted in my left eye 16 days ago, I wish I would have seen this video prior. I am struggling with my vision being very hazy 16 days post surgery.
Dear Dr Shannon, thank you for your videos on this topic. I underwent a Synergy lense implant in my left (non-dominant) eye just over a monrh ago. The day after the surgery my vision from the operated eye was abismal but has improved over time. Last week I had my 1 month check and things have improved a lot. I have 20/20 vision and a combined visus of 160. Nevertheless, even though,, I can read even small letters without glasses, I have exactly the same discomfort as the lady in the video describes (except for the fastidious halos). Letters seem to have a shadow around them (this is especially visible when looking at white letters on black background with only the left eye). I occasionally see rhe sickle formed refraction of the lense at rhe prerifery of my left eye, and I feel something like a temporal pressure on the operated eye. Some halos are visible, but they don't bother me. From the atudiea I have read in the topic it is xlear that Synergy lenses take longer to get used to. In your experience, should I be more patient or is further improvement after 1 month unlikely? Thank you in advance.
I’m pleased with synergy lenses, vision is fine at all distances, especially near vision which is important for me. They’re not perfect, but there’s no perfect replacement lenses

I have PANOPTIX -- had it for 3 monts now and STILL can't read street signs more than 40-50 ft. away-- which at highway speed- that's getting a bit too close. It started improving a bit when I started doing a spoon of MCT oil in my coffee daily--- I noticed about 40% improvement first day... but eventhat depends on how I feel. I'f Im' tired- can't see.. or stressed, can't see- I have to be very relaxed and had JUST put the drops in (artificial tears). and then I can see SOMEWHAT better- but not good!!! It's like the lady in the video is describing but WORSE-- WITH the PaNOPTIX lens..
Did your eyesight improve? I had Panoptix lenses in both eyes four months ago and have been hazy since day 1. The doc treated me for inflammation saying that was the reason but still I can’t see properly and it’s so depressing.
I had the synergy put in my left eye. I have had the tag done to them. They are still not right. I can feel them. I now have dry eye as well and on restasis.,Vision still not clear mid or far. Can I exchange the lens to the pantonix?
Good morning sir I have iridofundal coloboma ... I had cataract surgery 3 month before after the surgery I m experiencing positive dysphotosia (glare,big-big halos,star bust , lightning streaks) and negative dysphotosia (dark crecent shadow on my left side) and dilopia(double text vission) and also had PVD (unignoreable giant floater that can be seen in dim light .... And causing dynamic glare fluctuations..... I am truly depressed I only have this eye with vission other eye don't have vission..... Pls help me sir .... I know the only solution for this problems is IOL exchange along with Floater vitrectomy.... I m not able to my simple day to day work from this problems... I m very cleaver and excellent in acedemics sir .. I was persuing my engineering in CS branch from IIT Madras ... Pls help me to grow and fly again like before .... I had cataract surgery in Dec 2021 and had the Tecnis Synergy implanted in my left eye. In my case I can see the distance and near fine but intermediate range is not very clear. I am waiting for a couple of months to see if the intermediate also clears up.
How is your intermidate vision now? I am 37 yo and have cataract in my left eye only. How is your night time experience? Please share. Thank you in advance
After more than a year of using Tecnis Synergy, I can say that the intermediate vision is not sharp. It's a little out of focus. Maybe it's because the lens stretches the light but that does not provide a sharp focus. Overall visual acuity is reduced a bit owing to distribution of light - sharpness and contrast is lower as compared to my right eye which still has the natural lens. At night I do see the rings. The rings and halos will not go away, your mind has to get used to them and filter them out mentally.
FWIW I had Panoptix implants in both eyes over the summer of 2021 and both have been a disappointment. The left eye is fair with distance vision but not sharp on well defined, intermediate and near vision are not well focused and night vision is very compromised by large halos around light sources. The right eye is worse in all respects and prescription glasses only provide minor near vision improvement. On my next visit to the doctor I will request the replacement of the right lens with a standard distance lens. Thank you doctor for the helpful information you have provided.
Have you looked into replacing it with a Vivity lens? Dr. Wong has mentioned those a few times as a good alternative to the Panoptics and the Synergy/Symfony. Check his other videos for details.
Sorry to hear that! If going for monofocal, please check out the new RayOne EMV IOL which is a monfocal and has the good aspects and sharpness of a monofocal at distance but still maintains some of the intermediate vision (some patinets even gain some near vision). I'm looking into having it in both eyes with a mini-monovision approach. Unfortunately there are no independent accounts available, although a doctor I visited recommended the lens.
I wonder if Dr. Wong has had a patient who was unhappy with the Panoptix lens and then replaced it with a Synergy, with good results? In other words, vice versa of this video in regards to the two lenses.
@@ShannonWongMD Thanks for confirming that. I'm doing my due diligence before having my second eye done. My first IOL was a Synergy toric 18 months ago on my non-dominant eye, and I've been less than happy with the results. I will discuss options with several eye surgeons here in Quito Ecuador before making a decision. I of course am not soliciting advice, just want to thank you for being candid about some patients' results.
Can you explant 3 years after original surgery? No YAG. What she describes about leaves now no longer defined is what I experience with Symfony. Even facial recognition is difficult.
I have Vivity in my in right and left eye and I have the same issues with your 2 patients. My right eye is always hazy compare to my left eye. ALL visions are blurry, unfocused. I am angry!
Yes. I use the eyhance from time-to-time. In my opinion, the vivity delivers a better range of vision than the eyhance. So, I usually prefer and recommend the vivity over the eyhance.
I had Tecnis implant 7/2013 and I hate it! Never had my other eye done. I cant hold off much longer and have to make a decision soon. Thinking of going with standard lense cause I dont want hazy vision in that eye I too. Had posterior capsule zapped too didnt help.
Actually for these and other reaons you mentioned, I was looking into the RayOne EMV. It is basically a monofocal IOL that has extended depth of focus. Which maintains some of the intermediate vision when targeting the eyes for distance, It's said by several doctors that it's very forgiving, which means the vision doesn't deteriorate due to some amount of error/inaccuracy when the lens is implanted. You can check it out and see if it suits you!
@@anassyria5176 I’ve been looking at the Ray One EMV also. The marketing material makes it sound great. Duh! That’s what marketing is for. But I have not heard anything from patients or independent cataract surgeons. I have a neighbor who recently had two PanOptix IOL lenses implanted and she is thrilled, despite some nighttime halos and starbursts. She said she just got used to that and is very happy. I’m going for my first exam in ten days and will probably go with what the doctor suggests. He is very qualified and I’ll discuss these issues.
@@martinsnitow544 Good observation! I find it quite bothersom to search for independent patient testimonies just to find none across the web. That being said, this Rayner lens was recommended to me by a doctor before I knew about it, and he sounds pretty confident with it's outcomes. Although, after listening to several patinets, it's safer not to limit our sources to only one ophthalmologist. But I agree, eventually, it's wise to pay good attention to a qualified, trusted doctor, not only because they have a superior idea of the results by their patients, but also because all doctors are not the same, and one doctor might be able to hit the right spot with a specific lens due to her/his experience with it and its compatibility with his methods and protocols of power calculations. As my birthday present, my ophthalmologist said my cataracts are not significant and I don’t need surgery now. My corrected vision is close to 20/20. I recall Dr. Wong saying that no iol can beat the original lens if it is working. So I’ll be back in a year or two to read about the latest and greatest. 😎

That's not what all these people are talking about - WHO ADVERTISES the implantation of a piece of plastic into the eye. All their tongue-twisting is a matter of 25th degree of importance or \ HERE WE GO AGAIN ...\.... ....They would need to explain what happens to the tissue of the eye if a PIECE OF PLASTIC is inserted into it and that the body’s reaction can be completely DIFFERENT to this PRACTICALLY splinter NOT IN THE TISSUE OF THE BUTTOCKS, but in the TISSUE OF THE EYE. .... that even to a SPLINTER in the buttock, different organisms react differently - in some cases the splinter is integrated into the body without any special complications, AND IN OTHER CASES PROBLEMS ARISE - leading to complete rejection of the splinter, due to swelling, inflammation, suppuration, etc.. ....AND WHAT, the reaction of the eye tissue to a PLASTIC SPLINTER IN THE EYE cannot be predicted, much less controlled by ANY SEPTIC AND ANTISEPTICS - that no one KNOWS the ETIOLOGY of the body's reactions and can only refer to "diabetes - age - gender - nutrition" etc.. ....BUT WHO WILL TALK ABOUT THIS IF MONEY IS AT Stake...
So here is my experience...for me, computer vision and smartphone was super critical. I got Panoptix in left eye, Synergy in Right. Right eye is 20/20, left 20/25 after 3, 4 months. Panoptix is fuzzy. Synergy is laser sharp. Night driving is hellish...rings, halos, starbursts in both eyes, bit worse with Panoptix. Not sure if anyone went for a Panoptix, Synergy combo? I was told I should try Vivity for less Halos...but might sacrifice computer vision...after surgery I got .5 diopter astigmatism...tried glasses, 0 different at night... I really don't know what to say...I guess losing near vision quality with Vivity will drive me nuts...may be I just hang on to what I got...
We do in our clinic like 1000 multifocal lenses per year. If you would be our Patient, we would explant the panoptix or synergy and would implant a ZLB00 in 1 eye and ZKB00 in the other eye. Less halos and still Gold computer vision.
I can share my experiene, I got Vivity EDOF lens implanted in both eyes (post cataract), computer vision is very good, but distant vision is a bit blurry. I see halos and glares at night, but they don't bother me much.
since the orginal eye lens is flexible-- why are they not replacing them wtih a SOFT bag of silicon gel?? (like a fake boob).. that will FLEX with the eye muscles.. ???

SYNERGY is autofocus lens. As a Professor of Math, with research in Geometry, I did not expect to get a continuous focus from small print to my Colorado mountains. SYNERGY is a miracle invention!! I did one eye on March 22, 2023, and plan to do another, again with Synergy, on May 3, 2023.

@@walterski8377 Before right eye cataract surgery, my right eye was 20/60 and left eye 20/25. A month after first surgery, on April 26, 2023, my vision became 20/20 in BOTH eyes! I told my doctor that I am ready to pay another $2600 to get Synergy in my left eye. He told me "You don't need it." He actually refused to operate because mysteriously unoperated eye became 20/20 as well! Just now, 6 months later, the vision remains 20/20 in both eyes, and the doctor took me off Lumigan (glaucoma drops), and left only timolol. I wish you all the very best for the approaching December 2023 Holidays and a Happy, Healthy, 20/20 New Year 2024!

Now it's time for the right (dominant) one again due to a cataract. My Dr. was opting in for Synergy. I asked him strictly to have Vivity instead because I want no more or I should say fewer halos/glares etc. and better intermediate with higher contrast quality. The surgery is planned for next week. I am 53 yrs old, still very active in sports (lifting, basketball, running), 6'2 with long arms, and very excited to have Vivity. Thank you for sharing. The patient's description is so familiar. I had Synergy in both eyes December 2021. TOTAL nightmare so far. The worst part is that it was an elective surgery - $12,000. Now all I want is my contacts and glasses back. MISERABLE.
For presbyopia there is no better than Synergy on the market, perhaps PanOptix will see better from a distance, but PanOptix is not better for near vision than Synergy. I have Synergy and my big problem was presbyopia. PanOptix would surely see better from afar, but not very close. and the distance vision of Synergy does not make me need distance glasses.

I had synergy lens implant in both eyes in the summer of 2021. I could not tolerate the lack of distance vision. My distance vision was blurry and even in the daytime nothing was clear and I could not see well enough to feel safe while driving. I determined I was not only experiencing halos at night but also during the daytime. In Nov and Dec 2021 I had iol replacement to Panoptix. I am happy with Panoptix. Since my surgery on my right eye in Dec 2021 I have experienced a retinal tear for which I am now recovering. I lost seven productive months of working, had to go to part time, stopped driving. I had to turn the brightness up on my computer monitor but had too much eye strain. I spent a lot of time in bed with my eyes closed. I will be so happy to have this difficult time behind me. I am 63 yrs old with no preexisting eye conditions except I’m near sighted. I believe sufficient warnings were not available from Johnson and Johnson and the Synergy lenses are a negatively life altering product.
Your experience is shared by many other patients with the synergy lens.

I started watching your videos about two months before I decided to visit an eye surgeon in Indianapolis Indiana and see if I qualified for the interocular lenses. A friend of mine also recently had panoptix lenses implanted in both eyes and had a very positive experience. After visiting the eye clinic in Indianapolis they strongly recommended synergy over panoptix. I originally decided to take the surgeons suggestion and go with synergy. I change my mind after I you decided to no longer offer synergy to your patience. About a week ago I had the panoptix inserted into my left eye and now I plan on having the other eye done in about three weeks. I am really happy with the panoptix and I’m really happy I watched your videos. I can read the The smallest print on the Eye Chart which I have on my phone. My vision has greatly improved.I’m going back to the doctor in a couple days and see officially what my vision is and I will let you know. Additionally I have not put on a pair of glasses since my surgery.
I had Synergy lens implanted in my left, Symfony in my right in December 2021. I'm totally with your patient in this video regarding the difficulties i'm having with the Synergy and trying to figure out how to tell my doctor what i'm experiencing. I'm not certain with the Symfony, but i'm very sure i'm not happy with my expectations now with the Synergy lens. Her description of the leaves in the trees, is exactly my experience. It's just not far distance either for me, at least.
I had the Synergy toric lenses implanted 9 days ago. Just like the lady stated, immediately after (next day) vision was great. I was impressed. I could read the fire station sign about 500 metres away and close up was amazing. Intermediate was also great, even though a bit of fuzziness. Then the following day vision quality reduced a bit. Three days post-op exam showed I had 20/20 vision but it was already getting fuzzy/blurry. Now 9 days after and vision has kept deteriorating now to the point everything except reading up close, is blurry. It is very stressful. I am taking my eye drops as prescribed (antibiotic, anti-inflammatory and lubricating). No lifting. Nothing strenuous.
I spend time looking at distances hoping it will help my brain sort out the neural adaptation to the lens. I don't get it. Day after great vision and nine days after very bad vision. I am doing a UA-cam journal of my experience and hope and pray this resolves. I am due to head back overseas end of July and to think it will stay like this (is as bad as I think it can possibly get) would be traumatic. I would need strong prescription glasses to give me back mid to far distance vision. I probably won't be back to get any corrective surgery for many months to a year.
It's been 9 months since Tecnis Synergy implants. Yesterday I had YAG for posterior capsule opacification and capsule bag wrinkling in both eyes. It has been 24 hours and my vision is no better. I am on anti-inflammatory drops and told it can take up to 48 hours for vision to improve. I still have issues with bright daylight where everything outside is very blurry. Sunglasses everywhere, but they only improve things a little bit with visual clarity. I have read that once YAG is done capsule lens exchange becomes more difficult as there is now exposure to the aqueous vitreous inside the eye. My eye health is otherwise normal. I feel like the lady interviewed on this video. I hope things improve. In one month I get a follow up review and they will discuss if LASIK is needed. But the ophthalmologist yesterday expects I will probably obtain close to 20/20 vision and likely won't need LASIK (to correct residual astigmatism). I am not confident it is just some astigmatism causing all these blurriness problems for me. And extreme blurriness on bright daylight. Night time vision improves but still blurry. Just reading up close is the only clear vision.

I will have cataract surgery two weeks from now. I choose Synergy because my ophthalmologist said Synergy is definitely better than Panoptix. He said Panoptix is made by pushing Acrylic into a mold, which will become foggy over time. But Synergy will not become foggy because it is made by cutting Acrylic.
That is a real concern and it is called glistening. Alcon is planning to replace Acrysof PanOptix with Clareon PanOptix that hopes to address the time induced glistening. It has been FDA approved since 12/2020 but not available.
I am going to implant panoptix. Suggest me whether should I go for it or not ?
@@biranchibarad4327 I had both eyes done with Panoptix. My near and intermediate vision is very good. My distance is good but could be clearer. It's only been a month for my left and 10 days for my right. I recommend them if you have the patience to work through the eye drops and the neural adaptation needed for these lenses to work properly.
Well, now I'm concerned. I had a Synergy lens implanted in my left eye on 1/20 and am due to have my right eye done this week. So far, my left eye's near and intermediate vision are improving (20/25), but not yet crystal clear. This woman's description of fuzzy edges is spot on. I don't know whether my vision will continue to improve (in both resolution and clarity) over the next few weeks or if how I'm seeing now out of my left eye is as good as it'll get without further tweaking with Lasik. I'd hate to do the Lasik, achieve 20/20, but still not have the razor-sharp clarity I require.
I should point out that I'm a pilot and need for each eye to test at 20/20 individually. The razor clarity is my own standard and what I've been accustomed to achieving with toric contacts over the last 40+ years prior to cataracts driving the move to IOLs. I'm also an edge case as a high-myope and my ophthalmologist told me that my choice of lenses was very limited. In fact, the Synergy was the only one that she said would suit my anatomical requirements.
She initially suggested that we implant a Symfony in my right (dominant) eye, but I'm very concerned about that. For one thing, I'm extremely attentive to detail and am likely a terrible candidate for any sort of neuro-adaptation, as any difference between the two eyes would be constantly compared. It would bother me all the time like a splinter that I couldn't excise. Also, I don't want any sort of "mini-monovision" as each of my eyes need to test at 20/20 distant individually.
I feel as though I need to go through with the Synergy implantation this week on my right eye and just hope for the best. To be clear, I'm not unhappy with my left eye (the near vision is better (and closer) than I ever achieved with contacts), but am hoping that it will continue to improve.
I know that you can't comment on my situation in detail from afar, but thought that I'd leave this here for others to read and, at the very least, mull.

I read elsewhere, actually maybe on the older post of yours I mentioned above, a commet of a very nice professor who was very generous and kind to share her experience. She had an initially very good outcome of the Symphony-Synergy combination. I will look her comment up, and reply to you from there if it's on your post, and you can ask her yourself about her experience with this combination.
@@anassyria5176 Thank you - please do. I don't recall seeing her response. The issue I have with dissimilar lens combinations is that they all seem to be predicated on the net binocular result, which is not what I'm after at all. I need to have each eye considered entirely separately, as that's how they'll be evaluated (this is an FAA thing for a 1st Class medical). If each eye is corrected to 20/20 individually, I'm confident that the binocular vision will be just fine.
Moreover, I need more than an objective correction to 20/20. It's not enough to be able to make out the letters on the chart - they need to be razor sharp, if only for my own comfort.

@@jayblue5310 If your priority is sharpness and crystal clewr vision, then go for the monofocal IOL. If you decide you want a nonofocal, please do check out the new RayOne EMV IOL and ask your doctor about it. It has clear distance vision, and the plus is that it maintains some of the intermediate vision due to its smooth defocus curve when compared to the standard older monofocal IOL designs. It's similar to the Eyhance, but said to be clearer in the intermediate range.
You are very detail-oriented and thus perhaps better suited for a monofocal lens targeted for distance. That’s assuming that the need for reading glasses is not a non-starter for you. Remember what Shannon always says: “there is no perfect lens.” Whatever you do, after you’ve had both cataracts done, don’t let them do a YAG laser “touch up” on you until you know that you basically like the lenses. It makes an implant exchange a lot more difficult and risky. But it’s still doable… I think I have videos of it on my channel.
I had Synergy implants done just over a year ago. Objectively I see amazing with 20/20 both eyes. Subjectively I have a hard time seeing details in clutter. Finding things in the utensil drawer is difficult and taking my PC apart is almost out of the question now. Like her I do experience some discomfort and feel as if I see the outer edges of the lense. Overall I'm very happy. Vision is much better than when I began to develop cataracts and I can funtion day to day and work on the computer without glasses.
That is really interesting. I have the exact same thing as far as seeing details in clutter. Great way to put it. I can be looking for something on a cluttered workbench, and not see it when it is right in front of me. It's like I don't recognize what I'm looking at.
I got mine in August 2021. One positive note is that seeing up close is amazing.
@@helifan1961 it's definitely a thing. I used to work on computer hardware as part of my job a few years ago and had no issues. Now with every piece i have to examine each part really carefully to to see which piece fits where and often I can't find the right socket it wire. Maybe it's neuralogical overload or something.

@@jquebedeauxjr Sorry to hear that. Mine is as good as I can ever remember, but my distant vision isn't ideal. I got glasses (-.75/-.75, -.25/-.75) which help with distance clarity when I really need it. I don't think the opthamologist really nailed my prescription so I wonder if the necessary pre-work was done (surgery induced astigmatism calculations, A-constant consideration, etc.)
@@jquebedeauxjr My near vision is almost as good as I remember up to about inches. Any closer and things get blurry fast. Reading these comments it sounds like my surgeon at Mitchell Eye Calgary did an exceptional job.
I have panoptix lenses in. Had surgery in June 2022 then had rt eye surgery again hoping for improvement and comfort. Not the case. I would pay double to have never had the panoptix eye surgery. So many issues. But I guess I shouldn't complain I am not blind just miserable and in a pressure strain type pain,24/7. I would give anything to have comfort and decent vision. 😢

I think the tri-focal implant is the very best....so far. $15,000 US is a no-brainer. Worth every penny, and lasts your whole life. Beg, borrow, or steal.

I'd rather put in a lens that gives you A- results 99.9% of the time than put in a lens that gives you an A+ result 95% of the time and an F 5% of the time.
I think you are such a wonderful dr because you will do anything needed to achieve the vision your patient’s are happy with. I can tell from watching your videos that all you care about is your patient...no ego involved at all. My Synergy lenses were implanted 6 months ago...my right eye is not perfect 20/25...but my left eye fluctuates between 20/50 + 20/80. I now have a slight astigmatism in my right eye and a severe irregular astigmatism in my left eye. And now severe floaters in my left eye. I can read but nothing is totally sharp. Distance is not good either but having the left eye being so different from the right eye is the main discomfort. I recently found out that the lens in my left eye is slightly off center...which is surprising because I was told that Synergy lens stay centered better than other lenses. My night vision is not good probably because my overall distance vision is lacking. The halos at night are not as bad as your patient mentioned. And I don’t feel the lenses in my eyes...so I guess that’s some good news for me. I will be having PRK done to my left eye in 2 weeks...hopefully that will solve the problem. But it worries me to hear that you no longer will use Synergy lenses. That makes me worry that these are not good lenses especially that you mentioned the lens had become cloudy in one of her eyes. I hope that never happens to me. Thsnk you for sharing these videos...they are always very helpful.

My doctor wants to perform a YAG Posterior Capsulotomy and a LASIK to correct the far vision blurriness.
My question is if after YAG + LASIK my vision is still not satisfactory, can lens exchange be done?
I understand Dr. Wong cannot give specific medical advice without exhamining the subject but I'm looking for a generic answer like can a IOLX be performed after YAG or should be avoided. Thank you

Thanks Anas, I got Alcon Vivity lens implanted in both eyes, Vivity is an Extended Depth of Focus lens (kind of multifocal lens) that gives you a very good distant and intermediate vision and good near vision. But in my case, the results are opposite. I met my surgeon today and he said that it takes couple of months for the vision to improve, I should wait for 3 to 6 months.

Wow, thanks for the wisdom. I've been struggling with the whole PanOptix vs Synergy decision. My experience with a relatively small number of patients has been similar to your's.
Shannon do you think refractive surgery can be considered therapeutic and not just stetic surgery? Many people develop intolerance to contacts and can't live a normal live. Doing exercise with high myope glasses is not comfortable and affect quality of life of many.
What are your thoughts about it?

Appreciate the video, doc. I was about to get a Synergy lens on 11/22/21, but based on the video you released that morning stating that the PanOptix slightly edged it out, I changed my mind at the VERY LAST MOMENT (right before being anesthetized). I *was* considering possibly going with the Synergy for the other eye, but I think I'll stick with bilateral PanOptix (or perhaps an EDOF w/ fewer dysphotapsias).

@@walterski8377 I went with PanOptix for both eyes and am very happy with the results. I have a small amount of residual refractive error in one eye, so that one can see a bit closer with a bit of a compromise in far vision. The other eye is 20/20 at distance. Between the two I have great vision both near and far.

I underwent a Synergy lense implant in my left (non-dominant) eye just over a monrh ago.
The day after the surgery my vision from the operated eye was abismal but has improved over time.
Last week I had my 1 month check and things have improved a lot.
I have 20/20 vision and a combined visus of 160.
Nevertheless, even though,, I can read even small letters without glasses, I have exactly the same discomfort as the lady in the video describes (except for the fastidious halos).
Letters seem to have a shadow around them (this is especially visible when looking at white letters on black background with only the left eye).
I occasionally see rhe sickle formed refraction of the lense at rhe prerifery of my left eye, and I feel something like a temporal pressure on the operated eye.
Some halos are visible, but they don't bother me.
From the atudiea I have read in the topic it is xlear that Synergy lenses take longer to get used to.
In your experience, should I be more patient or is further improvement after 1 month unlikely?

I’m pleased with synergy lenses, vision is fine at all distances, especially near vision which is important for me. They’re not perfect, but there’s no perfect replacement lenses
90% of my synergy patients are happy with their vision. 10% are not.

I have PANOPTIX -- had it for 3 monts now and STILL can't read street signs more than 40-50 ft. away-- which at highway speed- that's getting a bit too close. It started improving a bit when I started doing a spoon of MCT oil in my coffee daily--- I noticed about 40% improvement first day... but eventhat depends on how I feel. I'f Im' tired- can't see.. or stressed, can't see- I have to be very relaxed and had JUST put the drops in (artificial tears). and then I can see SOMEWHAT better- but not good!!! It's like the lady in the video is describing but WORSE-- WITH the PaNOPTIX lens..
Did your eyesight improve? I had Panoptix lenses in both eyes four months ago and have been hazy since day 1. The doc treated me for inflammation saying that was the reason but still I can’t see properly and it’s so depressing.
I had the synergy put in my left eye. I have had the tag done to them. They are still not right. I can feel them. I now have dry eye as well and on restasis.,Vision still not clear mid or far. Can I exchange the lens to the pantonix?

I have iridofundal coloboma ... I had cataract surgery 3 month before after the surgery I m experiencing positive dysphotosia (glare,big-big halos,star bust , lightning streaks) and negative dysphotosia (dark crecent shadow on my left side) and dilopia(double text vission) and also had PVD (unignoreable giant floater that can be seen in dim light .... I had cataract surgery in Dec 2021 and had the Tecnis Synergy implanted in my left eye. In my case I can see the distance and near fine but intermediate range is not very clear. I am waiting for a couple of months to see if the intermediate also clears up.
How is your intermidate vision now? I am 37 yo and have cataract in my left eye only. How is your night time experience? Please share. Thank you in advance
After more than a year of using Tecnis Synergy, I can say that the intermediate vision is not sharp. It's a little out of focus. Maybe it's because the lens stretches the light but that does not provide a sharp focus. Overall visual acuity is reduced a bit owing to distribution of light - sharpness and contrast is lower as compared to my right eye which still has the natural lens. At night I do see the rings. The rings and halos will not go away, your mind has to get used to them and filter them out mentally.
FWIW I had Panoptix implants in both eyes over the summer of 2021 and both have been a disappointment. The left eye is fair with distance vision but not sharp on well defined, intermediate and near vision are not well focused and night vision is very compromised by large halos around light sources. The right eye is worse in all respects and prescription glasses only provide minor near vision improvement. On my next visit to the doctor I will request the replacement of the right lens with a standard distance lens. Thank you doctor for the helpful information you have provided.
Have you looked into replacing it with a Vivity lens? Dr. Wong has mentioned those a few times as a good alternative to the Panoptics and the Synergy/Symfony. Check his other videos for details.

If going for monofocal, please check out the new RayOne EMV IOL which is a monfocal and has the good aspects and sharpness of a monofocal at distance but still maintains some of the intermediate vision (some patinets even gain some near vision). I'm looking into having it in both eyes with a mini-monovision approach. Unfortunately there are no independent accounts available, although a doctor I visited recommended the lens.
I wonder if Dr. Wong has had a patient who was unhappy with the Panoptix lens and then replaced it with a Synergy, with good results? In other words, vice versa of this video in regards to the two lenses.
I have not removed a Panoptix and replaced it with a synergy. We stopped using the synergy in 2021.
@@ShannonWongMD Thanks for confirming that. I'm doing my due diligence before having my second eye done. My first IOL was a Synergy toric 18 months ago on my non-dominant eye, and I've been less than happy with the results. I will discuss options with several eye surgeons here in Quito Ecuador before making a decision. I of course am not soliciting advice, just want to thank you for being candid about some patients' results.
Can you explant 3 years after original surgery? No YAG. What she describes about leaves now no longer defined is what I experience with Symfony. Even facial recognition is difficult.

I have Vivity in my in right and left eye and I have the same issues with your 2 patients. My right eye is always hazy compare to my left eye. ALL visions are blurry, unfocused. I am angry!

Yes. I use the eyhance from time-to-time. In my opinion, the vivity delivers a better range of vision than the eyhance. So, I usually prefer and recommend the vivity over the eyhance.

I had Tecnis implant 7/2013 and I hate it! Never had my other eye done. I cant hold off much longer and have to make a decision soon. Thinking of going with standard lense cause I dont want hazy vision in that eye I too. Had posterior capsule zapped too didnt help.
Actually for these and other reaons you mentioned, I was looking into the RayOne EMV. It is basically a monofocal IOL that has extended depth of focus. Which maintains some of the intermediate vision when targeting the eyes for distance, It's said by several doctors that it's very forgiving, which means the vision doesn't deteriorate due to some amount of error/inaccuracy when the lens is implanted. You can check it out and see if it suits you!
@@anassyria5176 I’ve been looking at the Ray One EMV also. The marketing material makes it sound great. Duh! That’s what marketing is for. But I have not heard anything from patients or independent cataract surgeons.
I have a neighbor who recently had two PanOptix IOL lenses implanted and she is thrilled, despite some nighttime halos and starbursts. She said she just got used to that and is very happy.
I’m going for my first exam in ten days and will probably go with what the doctor suggests. He is very qualified and I’ll discuss these issues.

Good observation! I find it quite bothersom to search for independent patient testimonies just to find none across the web.
That being said, this Rayner lens was recommended to me by a doctor before I knew about it, and he sounds pretty confident with it's outcomes. Although, after listening to several patinets, it's safer not to limit our sources to only one ophthalmologist.
But I agree, eventually, it's wise to pay good attention to a qualified, trusted doctor, not only because they have a superior idea of the results by their patients, but also because all doctors are not the same, and one doctor might be able to hit the right spot with a specific lens due to her/his experience with it and its compatibility with his methods and protocols of power calculations. As my birthday present, my ophthalmologist said my cataracts are not significant and I don’t need surgery now. My corrected vision is close to 20/20. I recall Dr. Wong saying that no iol can beat the original lens if it is working. So I’ll be back in a year or two to read about the latest and greatest. 😎

Mixing one implant in one eye and a different type of implant in the second eye is commonly done with generally very good results.

That's not what all these people are talking about - WHO ADVERTISES the implantation of a piece of plastic into the eye. All their tongue-twisting is a matter of 25th degree of importance or \ HERE WE GO AGAIN ...\....
....They would need to explain what happens to the tissue of the eye if a PIECE OF PLASTIC is inserted into it and that the body’s reaction can be completely DIFFERENT to this PRACTICALLY splinter NOT IN THE TISSUE OF THE BUTTOCKS, but in the TISSUE OF THE EYE.
.... that even to a SPLINTER in the buttock, different organisms react differently - in some cases the splinter is integrated into the body without any special complications, AND IN OTHER CASES PROBLEMS ARISE - leading to complete rejection of the splinter, due to swelling, inflammation, suppuration, etc..
....AND WHAT, the reaction of the eye tissue to a PLASTIC SPLINTER IN THE EYE cannot be predicted, much less controlled by ANY SEPTIC AND ANTISEPTICS - that no one KNOWS the ETIOLOGY of the body's reactions and can only refer to "diabetes - age - gender - nutrition" etc..
....BUT WHO WILL TALK ABOUT THIS IF MONEY IS AT Stake...
So here is my experience...for me, computer vision and smartphone was super critical. I got Panoptix in left eye, Synergy in Right. Right eye is 20/20, left 20/25 after 3, 4 months. Panoptix is fuzzy. Synergy is laser sharp. Night driving is hellish...rings, halos, starbursts in both eyes, bit worse with Panoptix.
Not sure if anyone went for a Panoptix, Synergy combo? I was told I should try Vivity for less Halos...but might sacrifice computer vision...after surgery I got .5 diopter astigmatism...tried glasses, 0 different at night...
I really don't know what to say...I guess losing near vision quality with Vivity will drive me nuts...may be I just hang on to what I got...
We do in our clinic like 1000 multifocal lenses per year. If you would be our Patient, we would explant the panoptix or synergy and would implant a ZLB00 in 1 eye and ZKB00 in the other eye. Less halos and still Gold computer vision.
I can share my experiene, I got Vivity EDOF lens implanted in both eyes (post cataract), computer vision is very good, but distant vision is a bit blurry. I see halos and glares at night, but they don't bother me much.

These are bifocal lenses from J&J. They work pretty good, if you mix the Addition. 1 eye with +2,0 and the other eye with +3,0.

since the orginal eye lens is flexible-- why are they not replacing them wtih a SOFT bag of silicon gel?? (like a fake boob).. that will FLEX with the eye muscles.. ???

@@myobmyob2215 Accommodating lenses is a developing technology, and many, MANY years away from maturity. Rest assured, like fuel injection in cars, it WILL come to fruition, just not yet
