    There is also the case where a person can have a chronic tissue based problem (cervical stenosis and osteophytes, inflammation and scar tissues) that can’t be treated successfully surgically and these are a trigger for central sensitization flare up. This is a bottom up situation and In this case opioids work to calm and prevent the trigger source pain thus helping to keep the central system calmed down, keeping a lid on all the mind body top down symptoms. Sadly with the overreaction and irrational response to the “opioid crisis” the top down sufferers have lost the one tool that works consistently to reduce their suffering. Cannabinoids require a lot of experimentation to find the right one that works without the high or sedation. They may not be the best choice in bottom up Central Sensitization. The bottom line is these patients are complex and require an initial investment of time that most primary care practitioners want nothing to do with. So we are misunderstood and left to flounder and suffer.

      You sound very similar to me and pretty wise , how do you get along ? What has helped you , 14 years and I’m on my last leg ! I had a 3 level 360 fusion , only made everything worse

    What happens when a person is suffering "Multiple Injuries" while 1 injury really requires surgery but the other injury has already been centralized? WHY would Gabapentin make spinal pain worse?

      There is a horrible drug don't ever take gabapentin. That's what they give people who are on state aid and stuff because it's cheap. Lyrica was the follow- on medicine when Gabapentin went off patent. Lyrica is a much better the drug not only effective for what you need but less side effects as well. I didn't say none but I will say very little try to get it if you can get insurance to pay for it. They're forcing us to do the very thing they said they didn't want us to do which is share our medication which each others. I could see we're going into a depression is so bad that there would be Underground centers where you could go and exchange medication if by medication that you need. If you haven't figured it out by now they're trying to kill us and doing a good job. Who is they they is the New World Order and they will do anything to see that Satan is set up in power over everything on Earth. Jesus will come back and kick him in the balls.

    I love this information--but it's missing a KEY component. People may be born with a propensity to develop sensitization, but it's the environment--traumatic experiences, attachment issues, threatening interpersonal experiences, chronic inflammation due to poor health behaviors--that actually shape the nervous system's reactivity as a protective coping mechanism. For individuals seeking treatment, there are very effective therapy modalities that reduce pain and sensitization that shouldn't be skipped over or disregarded.

    Is Central Pain, neuropathic pain?

      Central as in "central nervous system" so definitely neuropathic.

      It does "come" from the nerves, but it's thought to be more from function than actual nerve injury, such as in what is usually called neuropathic pain.
