    As a CNA instructor I have never allowed students to instruct patients to hold onto their shoulders. Experienced physical therapists reinforce with us during their clinical training that this has often resulted in injury to the CNA. Should the patient lose their balance they can easily cause injury to the CNA's shoulders or neck. Additionally, this is not promoting independence as the patient is being asked to steady themselves by holding onto the CNA. The patient should be instructed to help themselves by pushing up from the bed or chair with their hands while the CNA assists them to stand using the gait belt.

      Thank you, and I appreciate your comment and understand your point. However, other healthcare professionals indicate that a patient moving as a unit, rather than having trailing shoulders and upper body, is preferable for body mechanics and reduces muscle strain and back injuries from uncoordinated lifting. The amount of force required to lift a person with their hands resting on your shoulders is considerably less than the amount required if they are pushing up on the bed or chair (try it yourself and see...I was amazed at the difference too!). Having them rest their hands on your shoulders, NOT grab around the neck, keeps them moving as a unit with you during the lift. This is an acceptable method for CNA testing through Prometric, as of the most recent update. Of course, the individual facilities will determine which method they prefer for their facility and this will be outlined in the policy and procedure manual for all healthcare workers.     I watched this playlist and even though not all of these are still on the test the ones that are the base line is the same and it helped so much I just took my skill portion of my state test and passed today :) thank you so much without this I dont think I would have passed.

      No the pay isn't top-level, but there are other rewards. The knowledge that you are able to help people when they need it. The smile from a family member when they know their loved one is in good hands. The pride of a job well done. Move up the ladder in nursing to LPN or RN and your wages will increase, but so will your responsibilities. Nothing worth doing in life is easy. Good luck!

    I see comments about low pay. Research and investigate the salary being offered, as well as the benefits short and long-term before you interview and/or accept the position. Decent pay is essential, but what about the quality of care you provide to the resident? It shouldn't be about the pay, but the interaction with the resident, their family and your coworkers. Are you a reliable worker and team player? Do you have the minimum of a BLS? Those are minimum certifications and requirements most companies are seeking. However, I did not have any certification, but took the class that was offered, received hands-on training on the floor, passed my class, and later received my state certification. Now my salary has increased and I'm even a Lead. You have to have the mentality to crawl before you walk, with patience and determination.

    For our class room, washing our hands is simply using hand sanitizer, will this be the case in the actual testing location as well?

      No. For the exam, you must PHYSICALLY wash your hands (using at least 20 seconds of friction before moving to your nails) at the beginning and end of the first skill. You will be graded on handwashing. Failure to actually wash your hands will result in a failing grade on the exam. After the first skill, you may be told to simulate handwashing. Do not simulate until you are instructed to do so by the evaluator. Best of luck!

    I was taught to place one of my legs in between the clients for stability when helping them stand, not in front. Is that acceptable??

      If you put your leg in between the client's legs, there is nothing to prevent their feet from sliding forward. In addition, no caregiver should ever "straddle" a patient's leg (one leg in between their legs and one leg to the side of their body) - this may give the wrong impression and is not good body mechanics. The body mechanics demonstrated in this video are appropriate and help ensure safety of both the caregiver and the patient. Thank you for your comment!

    hey Pat, l want to make sure l understand this, in the book it says to ambulate a resident ten steps, now is it five steps going and five steps coming back or it is actually ten steps going? l am confused please help!

      Remember the patient has to walk for the return trip to the chair...so 5 steps in one direction + 5 steps back = 10 steps. Coming or going, it's still going to count as steps. Hope this helps.

    great video , I'm wondering regarding Red Cross Stat exams , do we have to use the transfer belt with a cane or a walker ? or just a gait belt ?

      You would go by the instructions on the care plan for the exam. The care plan will indicate if a gait belt must be used. As a CNA, you will receive directions from the care plan or directly from the RN. You are not allowed to make decisions regarding patient care, so the test will have a care plan that indicates the task that needs to be done and information you need to perform that skill. Hope this helps!

      I am not familiar with MA's clinical skills scenarios, but the candidate booklet for Massachusetts does not list Ambulate with a Gait Belt as one of the testable skills. They do list Transfer, but not Ambulate. There is limited information available for testing standards for this state, so I am unable to be more specific. Good luck!

    Do we not suppose to identify the patient with the patient armband? And how about raise the bed for your body mechanic and lower the bed before leaving the room for the patient safety?

      For the Florida exam, it is not required to check ID bands during the test because not all patients in all settings will have ID bands (ALFs, some nursing homes and home care will not use them). These videos are demonstrating current Florida requirements. You will simply address the patient by name. Just like in a doctor's office, when they address you by name - if they call you the wrong name, you would tell them! For the exam, the patient will self-identify. In a clinical setting, if they cannot self-identify, the appropriate method of identification would be indicated in the care plan. Please see our video on the care plan for more information. Raising the entire bed to make this skill more comfortable for you is not REQUIRED for the exam. You can do so if you wish, however. This is not a graded point on the exam. If you do decide to raise the bed to a comfortable height, you MUST remember to lower it at the end of the skill. If you do not put the bed in its LOWEST position, it is an automatic failure because if the patient tries to get out of bed, the floor will be much farther away than expected, causing them to fall!

    Shouldn't we check patients I.d bracelet? Will we lose mark if we forget?

      For the exam. the resident will not have an ID bracelet. This is because not all settings have ID bracelets (nursing homes, home care, assisted living, long-term facilities). Since they are not present in every setting, they are not used in the exam. You must identify the patient BY NAME prior to performing care. This is a requirement, to ensure you are working on the right patient (very important in healthcare). Failing to identify your patient could result in a failing grade (because of the potential consequences to the patient in a real setting). Hope this helps!

      Yes you are absolutely right you should always check a patient’s wristband to make sure you’re doing the skill on the right person, if not you will fail. I don’t know why they don’t show this in these videos I literally just made a comment about this. That is a big step they are skipping over

      @@4yourcna What state are you referring to as far as not checking an ID bracelet because that’s wrong for the state of Florida you have to I’ve been a CNA over 10 years, and my family owns a business that trains people to become nursing assistants in Florida We are also a state testing site.

    I am just not understanding- The CNA knocks two times, waits for the patient to say Hello and then the CNA comes in. How is that respecting the patient's privacy? Why not ask if it is okay for me to come in or is it safe for me to come in or may I come in? Upon hearing the patient say Yes or come in, the CNA comes in to introduce themselves. If the patient says nothing at all, then the CNA should poke his/her head in to check on the patient just in case the patient is in the bathroom, asleep, or had hurt themselves or unresponsive.

      For Prometric testing, this person is already sitting in a chair at the side of the bed, so there is no reason to check bed wheels or sit the client up since neither apply. Every state is different. Please refer to the clinical skills checklist for your state. Thank you for your comment!

    I don’t know what state this is for but in the state of Florida if when you’re introducing yourself to your patient and you do not check their wristband to make sure you are doing the skill on the right person you will fail. I don’t know why they never show this in these videos but people please keep that in mind. I’ve been a CNA for over 10 years.

      Thank you for your comment. We are also in Florida and these videos are produced according to Prometric's Clinical Skills Checklist in Florida. Please note on the Clinical Skills Checklist (available on Prometric's website under Candidate Resources), item #1 for every demonstrable skill states, Does the candidate, "Greet resident, address by name and introduce self?" Nowhere in this checkpoint does it require verifying the ID band. In fact, it specifies that the tester must address the resident by name, which is shown in all skills videos. When producing these videos, we always refer back to the Clinical Skills Checklist to guide performance, so that the videos show EXACTLY what is REQUIRED for the state exam, but these videos should always accompany classroom training by an RN who would explain that in some settings (like a hospital, rehab or surgery center), patients may also need to be verified through the use of an ID band, according to the policies and procedures of that facility. These videos show how to perform the skill according to testing checkpoints, but should not be considered a substitute for training, as the watcher is not getting the benefit of classroom discussion, skills foundation lectures or acceptable variations that might occur in the workplace (all of which would be covered in classroom training by an RN). Training that does not cover this content, or is not provided by an RN, will be incomplete and often contains improper information that may lead to misconceptions. But, to be clear to anyone else reading the comments, checking an ID band is not a required step during the exam, according to Prometric's published checklist. Students should be encouraged to print this checklist and refer to it as they are watching any video skills instruction to make sure that all steps listed on the checklist are performed as described.       For the exam, the patient for this skill will be sitting in a chair and your care plan will instruct you to return them to the chair after ambulation. Always follow the care plan.       You have to think about the return trip. Normally, when a care plan instructs you to ambulate in a clinical setting, it will give you a specific destination - such as to the nurse's station, to the bathroom, to the bedside chair and back. But in the testing center, they cannot write a destination on the care plan because the care plans are universal across all testing sites in Florida, so the "bathroom" may be a whole lot further in one testing center than another and not all testing centers are arranged so that a chair can be placed 5 or 6 steps away from the bed. So, for the test, they give you steps. Trust me, they are NOT counting steps here - that is not what the test is about. They are indicating that they want you to walk far enough that they can evaluate your technique and this can be accomplished in 10 steps. But they also need to see you lift a patient to standing and help them sit, which means you have to turn around to return to the chair. So the steps are just a means to evaluate your performance; 5 steps out and 5 steps back is enough to do that. If you take 6, 7 or 8 steps before turning around, it will be fine (they just don't want you walking for 5 minutes because they don't need to see that much and it wastes their time!) - it's the technique here that counts. I hope this helps.

    in this skill test I didn't check for non skid sneakers and I didn't close the curtain is that reason to fail?!! I also did like 10 steps to and fro😥

      It's a lame reason for them to fail you and they could easily overlook it if they wanted to. But yeah they chose to fail you instead because they're already getting paid and they dont care about us.

    I will be challenging the board next month and I want to know if all actors/resident will be sitting up in chair with shoes on or would the have to get the actor/ resident be in bed with no shoes on ?

      For the exam (if testing with Prometric), this is the exact scenario for testing (patient sitting in chair with shoes on). Other testing agencies may differ with how they set the scene. But if you are in AL, AR, CT, DE, FL, HI, ID, LA, MI, NM, NY, OK or WY, this is the scenario you will be using! Hope this helps!     I have a dumb question. I'm seeing this with all of your videos. She knocks( doesn't wash her hands upon entering) then tells the patient the procedure. Then AFTER that she washes her hands. Wasn't she supposed to wash her hands BEFORE entering the the room AND after she explaines the procedure to the resident because she's preparing to ambulate. I believe there is supposed to be three Washing of hands here. The before entering the room. The before ambulation and after ambulation... I Live in Michigan and am preparing to take my Prometric exam. I don't want to fail because I didn't wash my hands before entering the room 😢 help anyone

      FireStar 311 hi am taking mine in a week. After she introduces herself she will go wash her hands and come back and check the patient identification .After the skill she will go wash the hands again

      You are not required to wash your hands before entering a room, only before and after providing care. Please refer to Prometric's checklist for specific checkpoints and you will see that you are not required to wash your hands before entering the room for any skill. You will knock, enter, address your patient by name, introduce yourself by name and title, describe the task, obtain consent, provide privacy and THEN wash your hands. Always verify specific testing requirements by reviewing the Clinical Skills Checklist for your state- but no state requires washing your hands 3 times for this skill. Best of luck!

      Why? You are in the room and can attend to whatever needs they have. The call light is used by the patient to summon assistance - but assistance is already in the room so putting the call light in their hand at the beginning makes no sense. Now, you ARE required to give them the call light at the end of the skill to ensure that once you leave the room you can be called back if they need you. Best of luck!

      There is nothing wrong with it except that you are showing the evaluator that you have no idea how call lights work and what they are used for. It would be like me telling you to "call me on the phone if you need me" and then you call me on the phone when I am sitting right next to you. The phone isn't needed as an intermediary if I am sitting right there, you can just talk to me. Same thing with the call light. If they press the call light, it doesn't duplicate the CNAs. It doesn't call anyone else. It calls you - and you are already standing right there. So, it doesn't make sense. You can do it if you want, but you will probably get a deficiency for Indirect care because it indicates a lack of awareness of a basic nursing principle. Hope this helps!

      Yes, these videos meet the needs of NY testing since NY uses Prometric. Prometric standardizes all testing states using Prometric testing, so everything you see here is based on Prometric principles. ***A CORRECTION TO THIS VIDEO*** Before beginning this skill, there will be SOMETHING (a chair, a rolling table, something) placed in your walkway. You are required to move that BEFORE you begin the skill (during the opening before you wash your hands). This is not reflected in the video. Important step: When you return the patient to the chair, have them turn around and make sure you ask them if they can feel the chair against the back of their knees BEFORE you ask them to sit.       4yourCNA i also have another question. My instructor told us that if we leave to wash the basin or dispose of anything , we have to lower the bed to the lowest position. Is this true. Or do we only lower the bed to lowest position before ending the skill?

      You want to know if they are safe to walk before you begin walking. It won't do you much good to ask for the first time halfway down the hall and find out that they have been dizzy and risk falling. This is a safety point and very important. You cannot walk someone who is dizzy, weak or unwell. That would be a nurse's job. So yes, this is important. Best of luck!

    This is inaccurate because the residents most likely aren't going to be so cooperate and helpful. You need to assume that they're going to be dead weight and you are the one to be doing all of the work. If I asked my residents to please put their hands on my shoulders I'd get laughed at

      Thank you for your comment, but please remember that every single resident is different. Some residents are weak, some are recovering from surgery, some will be paralyzed on one side, some paralyzed from the waist down. Some residents will have a debilitating muscle disease such as Multiple Sclerosis and others will have suffered a stroke. Some patients may have injuries that require a cast or splint and some may have amputations. Some may even have dementia. But you cannot make a blanket statement that "residents most likely aren't going to be so cooperative and helpful." You cannot ASSUME that all residents are going to be dead weight. That may be the case where you currently work, but that is NOT the case for all patients in all settings. In fact, making any assumptions can lead to injury for you and the patient. You are required to follow the instructions in the care plan, which is developed by an RN after a long, extensive assessment and takes into account each person's patient's specific (individual) needs and develops a plan for the team to follow to keep everyone on the same page. CNAs cannot do assessments. CNAs do not have all the knowledge necessary to make informed decisions to ensure the patient's safety. Because of that, CNAs must follow the care plan. In the beginning of this video, you will see the care plan for the exam. It clearly states that the patient is unable to walk or take steps but can stand with support. That does not mean that ALL patients will have this ability...just THIS patient. This is how the skill should be done for this specific patient. Other patients may require other accommodations and that will be reflected in their care plan. But when helping a patient stand, it is definitely helpful to ask them (or assist them) to put their hands on your shoulders, which keeps their body moving as a unit (promoting proper body mechanics). Keeping their hands at their sides makes their shoulders trail behind as you lift from the center of their body with the gait belt which can lead to injury or instability. This is an important component of this skill and if it is unable to be done in a clinical setting, other accommodations should be made for transfers. Please follow the care plan at all times, for the safety of the patient. Thank you and good luck!

      4yourCNA So what happens when you don't brace yourself to completely be 100% ready for that weight and you end up with a fall. Then what. "It wasn't in the care plan, I didn't know I was supposed to help that much" is an awful explanation on an incident report. What about our non verbal folks who can't say "actually I'm feeling a little bit weak and frail today"
      What I'm getting at here is that it is naive to put the faith and trust in the resident when you as the CNA need to be prepared to assist in total care. We always incourage our residents to do what they can but at no point do you throw risk factors out the door because they're not directly written on the care plan.

      Using this technique, you are able to adjust for the amount of assistance that each patient needs. You can provide full assistance in a pivot transfer (as demonstrated in the video) or minimal assistance and support. Standing at an oblique angle allows you to adjust accordingly and uses the strong muscles of your legs to provide the necessary support. I never said that you shouldn't be able and willing to provide total assistance in the transfer. I never said that you shouldn't be braced and ready for their weight. What I said was in direct response to your comment stating, "You need to assume that they're going to be dead weight and you are the one to be doing all of the work" which is not true. My response to that statement provided examples about how any assumptions can put the resident in jeopardy. It also directed you to the care plan for the exam, which is specifically what this channel is intended for.
      We offer CNA TEST PREPARATION resources and demonstrate skills based on the CNA exam. This is not, nor will it ever be, a complete CNA training program. There are many, many factors regarding every single skill that must be discussed and taught by an RN in a classroom setting that affect the actual performance of any patient care skill. I cannot provide all of that instruction in a non-interactive video-based setting, such as UA-cam. However, I do understand that many students read the comments and learn important principles through this media, in addition to classroom instruction, so I try to correct assumptions and relay the importance of the care plan whenever I see a comment that could mislead a student. That is what I did with the response to your initial comment. As an instructional channel, that is my stated mission. You are applying a general principle to a specific situation, and that is not helpful to learning students.
      It is important to understand the importance of the care plan. It is important to understand the proper body mechanics employed in the skill. It is important to provide the assistance required by the patient to prevent falls. It is important to report any changes in strength or level of ability to the RN. It is important to understand that all patients in all settings will not have the same needs and accommodations in level of assistance will need to be made, based on the needs of THAT patient. Please use the proper body mechanics and be prepared to provide maximum assistance in case of weakness, but do not ASSUME that all patients will need it or you can injure the patient by moving them when they are trying to move themselves. FOR THE EXAM, the patient can stand with assistance but cannot take steps. And that is what is demonstrated in this video. But many of these principles must also be discussed in a classroom setting for complete understanding. I hope this clarified my initial response to your comment.
