I think what would be considered a concern, is abusing substances that interfere with your mental health and wellbeing to the extent that you can harm others.

I need guidance on accurately filling out the Green Card Lottery application and preparing for the visa interview due to my complex family situation.I have two biological children from a previous cohabitation. Both have my name on their birth certificates.I legally married another woman but divorced after she attempted to harm me. She had an adopted child who also has my name on his birth certificate.I am currently married with three children, all bearing my name on their birth certificates.How should I list my children in the application, and what documentation will I need for the interview?

You need to list all your children regardless of whether or not they will be coming to the US with you. List names according to their legal documents and have copies of their birth certificates handy. Any child that is not biological (i. e adopted), you need to state as such. Your DS-260 form should have the same details as what you provided on the Green Card Lottery application.
