You are very motivated, and I understand the feeling of not completing your longest run with your first marathon just 6 weeks away. You have done an amazing job up to now.
I have a feeling I know the foot pain you are experiencing. I am not a professional, I can only speak based on my experience and what you described resonated with the foot pain I had when training for my first marathon in 2023.
Both my feet are flat. I can run with neutral or stability shoes with mild overpronation. Short-distance running does not result in any pain/discomfort.
This pain only started when I increased my mileage rapidly during my marathon train (from 16km to 24km over 3 long runs). My posture, and gait "collapsed" as I became tired. I was no longer able to support my proper running gait/posture. The tendon and muscle started to become strained and over time/distance, I started to feel the foot pain with every step.
I later found out that the pain on the inside (medial) of the foot was the result of overuse of the posterior tibialis tendon connected to the posterior tibial muscle resulting in tendonitis. The road camber provides support for the arch, relieving the strain (pain) on the tendon, if you run on the other side of the road, the camber will worsen the strain - this was the same thing I experienced.
I strongly advise that you stop running and start promoting healing and flexibility with RICE. Swimming and cycling should be fine, that's what I did. The physio should be able to massage the tibial muscle to release any knots/tightness. The cause is likely the rapid increase in mileage from 26 to 34 and the muscles and tendons were too weak to sustain the load resulting in overuse.
I overcame this injury through rest, strengthening the posterior tibial muscle and managing my load post-recovery. Wearing neutral (with support) running shoes helps too.
There are 2 other possible causes, but I highly doubt they are what you are experiencing.
    a) a navicular bone stress fracture (pain around that same area)
    b) an accessory navicular syndrome
Hope you recover soon to continue your training for the Manchester marathon.

Thank you so much for taking the time to write such a long message. Sounds like what you've had is what's causing me pain on my foot. It has been a lot better since last Sunday and even during my Half Marathon yesterday it felt okay. Ended up with a completely different new injury though and it's so frustrating. I've been trying to gradually increase the distance. I did run a 30K between the 26 and 34, but I guess my body just isn't used to those kinds of distances and maybe I'm just not strong enough when I start to feel tired. Like you said, your posture and everything changes when you start to feel fatigued and I think I'm just not strong enough to hold myself up properly on longer runs.
Do you think neutral shoes with stability insoles would help? It's so annoying my feet aren't the same on both sides, which makes identifying issues quite hard.
I've been off my feet trying to recover, but I've developed a new injury which means I'm going to be off my feet again, but I might see if I can manage to use an exercise bike or cross trainer to keep up with my fitness and endurance.
Despite wanting to be strong to get through all of this, physically I can definitely feel the limitations and how much harder I potentially need to work to succeed.
Again, thank you for your input. I've made notes to look into everything a bit more and maybe have a plan of action for the future.
Are you training for anything at the moment?

@@YuennyLam No worries. The aerobic system develops faster than the muscles, bones and tendons. They are always playing catchup to the aerobic system. It just takes time and strengthening work.
Insoles may change the structure of the shoes and it can go either way. I have done that too with my neutral shoes but ended up removing them. They usually don't fit well and do not address the root cause. Strength and conditioning are key.
For now, managing your volume, having sufficient rest and icing the area is my best advice. Muscle soreness/tightness is also a sign of fatigue that results in tendonitis.
There were a lot of lessons learnt, about my body, from my first marathon. I am now training for the Sydney Marathon on Sep 24. I am getting wiser, stronger and hopefully faster 🙂. Good luck with your marathon.

Good luck with your training as well. Sydney sounds lovely. I'd love to visit some time. There's a chance Sydney will be added as another Marathon Major, so in that case I will have to visit 😄

Yuenny, try not to be too hard on yourself. I do understand why you are, though. You set a goal for yourself and you want to prove to yourself that you can meet it. Just know that the percentage of runners who get no injuries, whether they be minor or more serious, is microscopic. Even Olympic level runners get injuries from time to time. They just happen.
Whether you see it or not (sometimes it can be hard for people to see their situation as accurately as people looking in can see it), you are making great progress. However, it's the periods of time when people are making a lot of progress when injuries are most likely to occure. But even when you get an injury, it will pass, and unless it's something serious, you'll heal back stronger than you were before you got it. It's a natural thing the body does to try to prevent further injuries. The only time you'll hear of people having repeated injuries in the same location is when they didn't give it enough time to heal, or when it's a more serious type of injury. Foot and ankle sprains are common and almost all runners experience them at some point.
If you don't meet your goal in the amount of time you were aiming for, there's, very honestly, nothing wrong with that. That goal was set without taking into account the potential for injuries, and when they happen they are not your fault. They are just something that sometimes happens. You will still reach your main goal of being a marathon runner, which is more amazing than you might think.
Think about this; only 0.01% of people in the world can run marathons. That's a real statistic and it's pretty crazy to think about. This means that when you reach the point of being able to run one, you'll have better physical endurance than 99.9% of people in the world. That is no small feat, Yuenny. What you're doing is a huge.
Side note: Since you're a slower runner, have you ever tried using shoes that are specifically geared towards easier paced running? I've personally used many different kinds of shoes and one pair that I've really been enjoying are the Asics Gel Nimbus 25. They are one of Asics highest rated shoes and they're geared for runs of a slow to medium pace. This creates a really nice feel when your foot hits the ground since slower paced runners strike differently than faster paced runners. The Numbus 25's also have a lot of cushion which will protect your feet more, over long distances. And even though they're considered a max cushion shoe, they still offer a good ride. They are currently Asics best selling shoe (the 26's are out now but not much different than the 25's which you can get at a discount) and I think, from how you run and what you need, they might be a really good fit for you. I'm not sure what running a marathon in them would be like but as far as daily training goes, they're great shoes.

I would love to be in that 0.01% and I know I will get there at some point in my life 🤞 It is hard to accept we get injured sometimes though when we feel like we're doing all the right things to prevent it, but I understand it is inevitable considering how much we workout.
I've tried the ASICS Gel Nimbus in the past. I've not actually run in them but ordered them to try. Once we wear them outside we can't return them, so I tried them at home but it felt like it was exaggerating my slight overpronation on my right foot. And that was when I was only walking in them and jogging up and down the corridor. I felt like it might cause issues on my foot if I can feel the overpronation without even running. I've heard a lot of good things about the Gel Nimbus though and maybe I should try them again. Think I tried the 24s.
I will continue to work hard and do my best. Thank you for the lovely long message 😊

I would advise Pysio before make any more long-run plans. you may have 6 weeks to go but would be better to take time off the pounding and do some cardio work non-concussive, like a cross-trainer for 7 -10 days to allow to heal, the more you run on it the worse it will get and if the body is making compensation to the opposite side then can create more issues to deal with. We dont have to run to keep the marathon training going when we get an injury, just find an alternative. YOU can do this 🥰 it might be an idea to go docs and try get in for an Xray as possible could have a stress fracture.

The danger is that if we try to push through them, we might make them worse or even cause secondary injuries.
