Lo is not only their friend, she is in charge of their physical well being. This is a goofy, but very rich and talented kid with prospects of going to the NFL. Lo is there to make sure he's comfortable and to witness that things were done correctly. Also, the Dr. clearly knew the shenanigans that were going to take place during that appt and had to authorize filming. He knows that Shiloh is a jokester.
