    so every time MMA is high that means B12 deficiency? i mean high MMA = low B12? i think not. theres an enzyme that converts the MMA into succinylCoA, that could be deficient and that could be the reason for high MMA levels. I understand that the presentation is pointing entirely to megaloblastic anemic patient and thats why it should be understood that MMA has to be high because of low B12. But then pernicious anemia is the most common cause of b12 deficiency too. so just like pernicious anemia isn't "necessarily" the cause of b12 def anemia; high MMA doesn't "necessarily" mean b12 def anemia. On the other hand, pernicious anemia always = b12 def anemia while high MMA does not always = b12 anemia. NEVERTHELESS, b12 labs could show normal in these conditions? thats is GREART to know. i DID NOT know that. so Thank you, as always.

    Ngl I was also thinking of concommitant autoimmune diseases because of AMS (“acting differently”), but that seems like more than a 4th order answer so I’d probably just pass on it
    Also as for not picking b12 or b9, MMA would rule in b12 and rule out b9 simultaneously

    Disagree. Checking for B12 level *first makes more
    sense actually , if it’s down end of story (if equivocal then MMA is warranted ). Sparing your patient from *unnecessary work-ups*😊 6:26

    In a nutshell :
    Vitamin B12 deficiency results in SECONDARY folate deficiency , on the contrary folate deficiency does NOT result in secondary vitamin B12 deficiency !

      md sohail no, cause mma is increased with neurological symptoms plus MCV>100. =b12 def.
      In b6/biotin defi, propionyl Coa will be increased.

      B6 is pyridoxine, not biotin. Biotin is B7. The story about propionyl coA is correct.
