    I have an 8 cm fibroid. I had a C section last year with my 1st child due to my fibroid preventing her from turning into the birth canal. My OB said the delivery was very difficult and said I must remove this fibroid if I plan on having another child. He said I can do UFE, myomectomy or a hysterectomy. He said if I UFE, all blood flow to my uterus would stop meaning I won't be able to convince after. Is this accurate ?

    Dr please share disadvantages of UFE. With my limited knowledge, I understand that with UFE there's utery or whatever it's called that is closed so that it doesn't feed blood to the fibroids so they can shrink. So please share more light on the disadvantages of UFE as there's an utery closed as I believe that utery doesn't only feed blood to the fibroids section but to other organs too

    I live in Connecticut and everyone including Dr's are telling me that I should get a myomectomy if I want to have children, the ufe procedure is the one I'd rather do and I need a Dr. With your confidence to perform this on me because I'm getting discouraged and I don't want any problems with getting pregnant in the future.. I'm 36 with tons of fibroids big and small. Please give me some advice and reassurance 🙏 😔

      Without seeing a pelvic MRI and going through a consultation, it's impossible to give specific advice. However, the fact that you have "tons of fibroids" suggests that UFE may be a better option. With lots of fibroids the gynecologist cannot remove all of them and often leaves many behind. These fibroids will grow after surgery and within 5 years over half of these patients will need another procedure (11% recurrence rate per year for myomectomy). UFE should knock out all of the fibroids and therefore the recurrence rate is much lower. Good luck with whatever you decide.

    HI DOCTOR AFTER HAVING UFE SURGERY FOR ANDOMETRIOSIS. CAN I HAVE A SUCCESSFUL PREGNANCY.     I've had 3 myomectomies. I don't want another one. They're back and I'm symptomatic. I've been bleeding nonstop for 4 months. I also got on birth control, but i feel like they fibroids grew bigger. I've seen couple of doctors, they're recommending another myomectomy. I'm 40, i don't have children, my dream is to have some. I would like to make an appointment with you. I live in Texas, but i don't mind flying out

      In my opinion, there is a never a reason to do more than 1 myomectomy. The fibroids are not "back", they never left. Those fibroids were the ones your gynecologist didn't/couldn't remove. Please do not do another myomectomy no matter what. Fertility will be difficult due to the three previous fibroid surgeries that you endured. I would be happy to see you remotely in consultation.     If one wants to maintain fertility and avoid major surgeries at all cost.
    (If the fibroid is above their belly button) Can the person take the GnRH treatment to shrink the fibroids then immediately do the UFE?
    Also, for one who doesnt qualify for UFE yet wants to maintain fertility and avoid major surgery should she opt for the acessa procedure, the mri guided focused ultrasound or just risk myomectomy.

      Acessa is laparoscopic surgery with no data on fertility and not recommended. Also, a lot of insurances don't cover it. Insurance also doesn't cover MRI focused ultrasound and it only treats up to 3 small-mid size fibroids or less. Vast majority of women are not good candidates for MRI focused even if you've lots of money to pay for it. You likely do qualify for UFE even if you want fertility. Births after UFE are typically full-term and vaginal whereas if you have myomectomy you will get a c-section. I would get an opinion from an experienced Interventional Radiologist who performs a lot of UFE. I'll bet you're a candidate. Learn more at ATLii.com. Good luck.

    I need this ASap!.. Where are you located? I have Andomiosis and I haven't stopped bleeding in 8 months I will need to make appt at Stanford hospital but I already had 2 MRI done there and they were forcing me to have a hysterectomy which I don't want. Now they are having me wait for more tests. They want to take out another piece out of my uterous but that's not scheduled until April. I've already had a blood transfusion because my level was at 4 point whatever that is. 🥺 These past 5 days have gotten worse the bleeding got heavier with clots. I can't go to work I cant even go on my daily walks. It's terrible. I am 50 old and never had kids but would like to have one. Please give me some advice. I am on high doses of contreceptives and Ibuprofen.

    Hi! Dr. Just wanted to know that After ufe is there any chance of ovary disorder?? My Gynecology told me that don't do this procedure.

      Your Gynecologist is over reacting. There are some women who don't menstruate after UFE. This hasn't occurred in anyone in our experience under 40 years of age. If you are 40-45 yrs of age the risk is 1-2%. For a lot of women the chance of undergoing a myomectomy and waking up with a hysterectomy is often 5-10x higher.

    I need to find a doctor in Miami that accepts Medicaid that does UFE!! I don’t want a myomectomy. And I want more children... do you accept Staywell?? I’ll fly to u if I have to. I’m desperate at this point 🥺

      I am one of the few specialists that accepts Medicaid. However, it is state-specific, meaning it is limited to your state of residence (i.e Florida). Try Dr. Srini Tummala in Miami. He is a friend and former colleague. Good luck.

    What is the difference between a UFE procedure and a Radio-frequency endometrial ablation procedure? I have Fibroids and would like the procedure to remove my Fibroids.

    Hey doctor! I am 40 years old. I have multiple fibroids. I had a myomectomy done about 7 years ago. And the fibroids grew back and just a year ago I had the UFE is it still possible for me to have children ?

      Yes, it is. However, being forty there is obviously less fertility and having had a previous myomectomy (and the associated cutting on the uterus) also diminishes fertility. Good luck.

    Wow. My OBGYN did not know ehat a UFE was and told me i cant get pregnant after a UFE and i can only get an open myemectomy or a hysterectomy to solve my multiple large fibroids (11cm and 7cm and maybe a few more idk). They did a CT scan, not an MRI as well as an ultrasound. I think i need a second opinion. Do you have any recommendations of good OBGYNs in Indiana that can help?

    Can UFE be used for large fundal myomas? 19cm?

      Yes, but it is not as simple as that. Without seeing imaging and knowing the patient's history, I would not be able to say if UFE would be the best option. Sometimes with these very large fibroids performing UFE will allow a better and safer surgical option. For example, this size fibroid would cause the uterus to be well above the belly button. The surgical approach for this size uterus is often a vertical incision from her breastbone all the way down to her pubic hair line. UFE might shrink the fibroids and her uterus to a level at or below the belly button to allow a horizontal incision (i.e. like a C-section) which is better for the patient. The fibroids/uterus are also easier to manage if they've been embolized/devascularized.

    I had UFE and my fibroids came back. I know I multiple fibroids with the back and side pain. I was told pregnancy is not an option. If your fibroids return after this procedure, does it lesson the chance of pregnancy

    Why do you recommend the myomectomy for the younger in age? I have heard the older the woman is the more likely a UFE can trigger menopause. Is that true?

      In 25 years of performing UFE in well over 9,000 women I have never seen anyone under age 40 go in to menopause after a UFE. Even 40-45 yrs it is 1-2%. Every woman contemplating myomectomy has to sign a medical release stating that they are going in for a myomectomy but sometimes wake up with hysterectomy. That happens more than 1-2% Also, with myomectomy the recurrence rate (11% per year) is much higher than with UFE.

    Hello Dr Lipman I am set to have a UFE soon I have 3 large fibroids , but I was told that the chances of getting pregnant are slim after the prodecure. I am 45 yrs old and very healthy , wondering if a myometomy is the better opinion or a UFE.I would like to at least have 1 more child naturally without Fertility treatments. is this possible?

      Can’t say exactly which procedure is preferable. The fertility rates are comparable but where UFE shines is recurrence rate is significantly lower. Also UFE nonsurgical and outpatient, therefore safer, less expensive, and less invasive. Also, to undergo myomectomy you’ll need to sign a medical release form attesting to the fact that you realize you may go in for myomectomy but may wake up with hysterectomy. That percentage that wake up with hystectomy much higher in forty year olds than twenty-thirty year olds. Be aware of this. Good luck. Learn more at ATLii.com

    How long is the wait time before trying conceive after UFE?

      Patients are instructed not to have intercourse for 2 weeks after UFE. Between 2 weeks post-UFE and 3 months post UFE (when we see every patient in the office in follow-up), we instruct them that they can resume intercourse but don't try to get pregnant. Any additional recommendations on when to try to get pregnant following the 3 month f/u depend on what things look like on the MRI exam. For most patients, they can try any time after the 3 month f/u.

    My posterior myometrium shows a large myometrial fibroid measuring 98 x 90 mm and both ovaries are obacured by large myometrial fibroid. I am 35, and will turn 36 in October. I am also yet to be married and want to preserve my uterus and fertility. There is no heavy bleeding. My period days are normal but I do get clots sometimes and extreme cramps for a day or two. Will UFE be best for me or laproscopic/myomectomy? I am inclined towards UFE though. Kindly do advise.

      Hard to say for sure. If your symptoms are not significant enough and this is driven more by fertility, myomectomy may be the better option. If your symptoms are significant and you also happen to be interested in fertility, UFE may be an option . I would suggest getting a pelvic MRI and going over it (and your symptoms) with an Interventional Radiologist who is experienced in UFE. Good luck. Learn more at ATLii.com

      @@Atlii the only major complication m facing right now is the size of this fibroid pressing on my distal ureter, and causing swelling in my kidney. With that reason alone will UFE be the right option given that it takes 3 months for the fibroid to shrink as I read while my kidney might need immediate relief to avoid severe fullness of backed up urine? I am to see a doctor soon but would appreciate your view so I can make an informed decision. I require relief for my kidney and urine back up issue with UFE procedure. Will this help me?

    OMG! I was just told pregnancy wasn't recommended after a UFE. My doc wants me to have an open myomectomy for a 3cm fibroid (grape-size) that is in the wall of my uterus. It's kinda pushing on the cavity. But, I think I should get a 2nd opinion for something so small. She said even after that, I would still be in danger and would need a C-section cause of fear of the incisions opening back up. I was like, what the heck lady?!!! LOL

      UFE is an option for women interested in fertility. Our births post UFE are typically full-term and vaginal, whereas as you found out, if you go the myomectomy route and get pregnant, delivery will be by c-section. 3cm fibroid not very big. If you were to do a myomectomy, a laparoscopic approach would be better than open and perhaps the Gyn is not an expert in these less invasive surgical options.

      I was in a similar situation… my fibroid is also pushing into the uterine cavity but mine is 6cm (not 3) & causes a lot of pain & cramping. Did you have yours removed yet?

      Not per se. However, the location of the fibroid does affect fertility. The submucosal (and large intramural) ones are the types that can affect fertility. However, the UFE patients we see that have children often have these. The MRI (and a thorough history) are necessary to counsel patients effectively on whether myomectomy or UFE would be a better option.

    Myomectomy is not the answer, because fibroids will still be there after procedure! I have two myomectomy procedure and still suffer pain and heavy, bleeding!

      Measures 8.1 x 3.3 x 5.9cm. Anteverted.
      Intramural fibroid of size 24 x 13mm is noted in the fundus on right side.
      Subserosal fibroid of size 7.0 x 6.1 x 5.0cms is noted at the fundus and extending along the
      left lateral wall.
      OBSERVATION:
      Uterus measures 8 x 4.3 x 3.9 cm. Endometrial thickness measures 7 mm.
      Evidence of a well defined predominantly T2 hypointense lesion measuring 5.9 x 5.7 x 7.4

      Well defined intramural lesion of size 13 x 14 mm seen at the fundus which is mildly
      hyperintense on T2 and T1 showing restricted diffusion.
      Right ovary measures 2.3 x 1.3 cm.
      Left ovary measures 3 x 1.7 cm with a well defined T2 hyperintense cyst measuring 15 x 13 mm
      within - likely a functional cyst.
      Cervix and vagina appear unremarkble.
      Minimal free fluid in the POD.
      Urinary bladder is normally distended. No abnormal wall thickening seen.
      IMPRESSION:
      Previous reports available: Yes - USG
      Well defined predominantly T2 hypointense lesion arising from the left lateral wall of
      the uterus at the fundus - likely a subserosal fibroid.
      Well defined intramural lesion at the fundus which is mildly hyperintense on T2 and
      T1 showing restricted diffusion - might represent an atypical fibroid.
      Suggested clinical, histopathological correlation.

    I’m 36 years old and had a UFE. I had two doctors told it was a bad idea and I would have a slim chance of getting pregnant because the procedure weakens the uterus

      What actually weakens the uterus is a surgical myomectomy. It is the reason why women that undergo myomectomy and happen to get pregnant will need to deliver by c-section (i.e. they're worried about uterine rupture due to uterine weakness). Our patients that have gotten pregnant after UFE are typically delivered vaginally.

      Yasmeen what has been your personal experience after the UFE? I'm battling a large one and all but two doctors have tried to convince me to get a hysterectomy. That doesn't work for me because I have no children. 39 may be too old to them but it's not too old to me.

      @@rasheada578 well after the ufe my period has been less painful and less bleeding. However I’m still not pregnant. I’ve been trying for a year. The fibroids are still there but shrunken and dried out. I have an appointment in 2 weeks with a black gyn doctor. Maybe I can get more insight on my situation as far conceiving. I suggest you try getting a second opinion. A hysterectomy is not the only option.

    I'm 42 and I have a hormonal imbalance, a body with the proclivity to produce fibroids, and have had major uterine and fibroid surgery. Because of this I'm seeking to have a UAE procedure done. However I was wondering, would it be possible for someone with my particular issues to concieve?

      Possible. However, you have clear “hurdles” including previous fibroid surgery, multiple fibroids, and 42 yrs of age.

    I've heard that UFE is not possible for very large fibroids.
    I have several, ranging from 9cm to 24cm. Most are intramural. One is subserosal.
    Can you say whether UFE might still work for me and the likelihood of preserving my fertility?

      If your fibroid (and not your uterus) is 24cm, UFE probably not your best option.

    Dr , Iam 39 years old newly married ..from India..I have fibroid 7.3 cm in fundal , 4.5 cm anterior and 3cm posterior wall respectively (intramural fibroid) not symptomatic only leg pain.Can I have UFE ?...can I have chance to have baby after UFE?
