There's actually a lot of explanation if you watch all the trailers. And pay close close attention to everything.

There's 3 other trailers which I highly recommend watching and reacting to in order. Also this is Kojima, his first game since Metal Gear. His games are very cryptic and artsy.

it is a mystery, there are multiple theories but i guess we have to wait until the game comes out or there is more detail coming out in the future

There is no backstory. Everyone's just as confused as you. Kojima just said he was working on a project and this was the project. Although we don't know anything this is a hell of a game and a day 1 pickup

There's nothing to explain, Hideki Kojime is known for his amazing work, but ever since death stranding was announced, people have been wondering just what exactly it's about, since no information or leaks have been shared about the game besides E3 trailers. Everyone is just memeing though when they say he was smoking pot while making this game. Lmao

From what I understand thus far, the main character, Sam, is a delivery man (a blue-collar worker of sorts). The game is survival-based. The rain (called "Timefall") actually ages the person/skin it touches, which is why you have to seek shelter. The bug the girl ate is supposed to keep you young. When his body turned red, I believe it was actually because there was one of the invisible creatures nearby (he got goosebumps, aka his senses went on alert), not because of the rain.
Whenever the character dies, they go to this upside-down water world. I'm not too sure what exactly happens when you return to the land of the living, since looking up this game on Wikipedia it says that the things you did before and any damage/explosions caused are still there, but then another video mentioned that a huge crater gets left behind when you die.
And I still don't know what is up with that baby.

What we know so far...
- Sam is a porter, it's his job to deliver cargo from one city/community to another (that's why we saw lots of walking in this trailer, because it's Sam/our main job in this game), he work for Bridges (company?) & on the other scenes he's wearing 'Corpse Disposal Team' suit (orange).
- You can see Sam bring spare boots in one of the scene, which indicate that your boots can worn out from all those walking in this game (you can see Sam's feet get hurt).
- From the all trailers that we saw up until today, we can assume that Sam can use various vehicles in his journey but maybe there are some places or locations that vehicle can't be used.
- We also can use weapons to defend ourself not against invisible entities (in 3rd trailer bullets not have effect on them), and according to developer weapons only for last option (pacifist recommended).
- 'Timefall' rain is one of the dangerous element in this game, if you get contact with that rain, it speed up time... a drop make your skin wrinkles, get exposed and you can get old rapidly (watch the 3rd trailer where Sam's colleague get stuck under flipped car).
- Cryptobiote, small creature that looks like large Tardigrades (which the short-haired woman ate) have effect to prevents/reverse 'Timefall'. In real life Tardigrades can enter cryptobiosis (an ametabolic state of life entered by an organism in response to adverse environmental conditions such as desiccation, freezing, and oxygen deficiency) to prolong their life or protect themself from extreme conditions/environment.
- It seems in this game you can't die (game over), when you get caught/eaten you'll enter purgatory (underwater like world), and revive but with permanent consequences that affects your gameplay.
- The short hair woman that helping Sam almost certainly a fellow porter (different company?), you can read on her suit 'Fragile Express - Handle with love-'.
- The Baby is special in this world, they can detect invisible entities if they were connected to the device thet attached on the shoulder (5 fingers light thingy?). In many asians country, there's a belief that Baby can see/sense invisible entities like ghost, spirit or jinn.
- Those invisible entities can't see us or can't hear us (device's sound on Sam's shoulder seems not drawing their attention) but maybe they can sense "life" from our breath.
- As for their conversation about 'getting eaten, voidout, getting come back & crater', you can watch the 3rd trailer where there's a big tall creature swallowing Sam's collague after his attempt to suicide failed & that creature explode creating a giant crater after Sam revive.
- The last woman on trailer is the woman in the picture that Sam holding on to but a younger version of her.
The woman at the end is Lindsay Wagner, she played Bionic Woman in the 1970s, and Kojima sought her personally to put her in the game as her younger self.

Something I don't see many people comment about is that when the "thing" comes walking near Norman Reedus, the footprints are actually just slightly bigger human hands.
Now, what intrigued me the most about it is that, although it clearly stepped where the photo was, *THE PHOTO REMAINED UNTOUCHED* . It just started floating above the water as if a puddle suddenly poured out from the ground.
I figure that there could be different planes that overlap like sheets of paper, and while the things can't touch what is in the layer the photo is in, the protagonist can. It could mean that Reedus' character is "stranded" in that plane and trying to work his way back. But this is Kojima, and he already hinted a lot of stuff in interviews, such as "it's a game about conneting us all together" that we can be sure mean something. So even if you think a theory is too crazy, it's probably not enough.

lindsay wagner played the bionic woman in the 1970s and kojima is a giant fan of hers.
When his body turned to cold he knows that the monsters are close but he can't see them.
I even had to look some things up, but from the look of it, wherever the "things" go along with the rain, it makes anything caught in it speed up in time. For example, plants growing and then withering in the span of seconds. Hence why Daryl has to wear that suit.
The baby might give him resistance to the rain by allowing the rain to age the baby rather than him and those thing that are floating I have no idea what they could possibly be. They honestly look like lost souls looking for a way back by replacing others with themsleves or even apart of some bigger threat or interconnected system because they knew almost all at once that he was there.
I watched every trailers and still can’t figure what is all about!
They cry from an allergy or something. She saw Norman cry and realized he had some condition that either let them see the alien beings, or sense them. She can see them, maybe the baby can sense and see them + much more. Maybe they clone the babies or he was transporting one of the babies and realized he could use its condition to his advantage.
All I can say is that the most confusing thing is trying to figure out how Hideo Kojima's mind operates. I couldn't figure out any of his trailers to this same game at all because he can twist any story line around in a game. I... gave up. Lol. :)

For those who are confused by this trailer; which is literally everyone, Kojima is known for creating games with trailers that give very limited and sometimes misdirected information so that your opinion on the game isn't made through the trailer but by your interaction throughout the actual gameplay. None of his trailers are supposed to make sense and they are supposed to keep you guessing. Hideo Kojima is well known for this and thats why his games are among the greatest that have been created. People will always look forward to his trailers so they can keep gathering up more and more information about a game.
P.S. This wasn't the first trailer for Death Stranding so there is also other ones but it also gives different limited information.
Norman reedus is trying to deliver the baby in the capsul somewhere or something cuz its seems like its the last bit of humankind and the e3 2017 trailer at the end you see a giant dark figure that made a explosion and seem to make him go back to the begging and start over idk but there is no game before this, this is kojimas first game ever made but we will have to wait and see when the game comes out.

there are actually a lot of things we know, they just don't seem to connect right now:
- the rain is called timefall (it ages people where it touches)
- Norman Reedus has some kind of ability in this game, that allows him to travel between life and death
- Death Stranding does mean something from another world came
my guess is following:
two dimensions collided, where the one invaded our world... Norman Reedus doesn't have the ability to "not die", he just comes back as the baby (like a clone maybe), that's why he forgets everything, at the end he asks his wife (photo) "who are you?"
And just so you know Kojima has areason why he doesn't tell us, he doesn't want this game to be like MGS V where most things where spoilert beforehand, i think he does a great job kepping people interested and speculating, i wanted to see real gameplay, but i guess the game is just not ready yet

Just a quick thing: the Timefall is the rain and it causes time to accelerate on whatever it touches like she said in the trailer. There’s a lot more to explain but I don’t even fully grasp it, you’re better off watching YongYea’s videos on the game (if you have time/care enough)

Ain't no backstory but that's part of the fun, just coming up with your own theories. Hideo Kojima is known for making every reveal a riddle to decipher. This is his first game since he left Konami but he did this kind of stuff with the Metal Gear franchize too.
From what I can tell it seems that they act as carriers through the world. The creatures are invisible and anything they touch is fast forwarded through time so if a person is touch they age to death. The female in the scene seems to be of a higher class. In the start he said he was extinction level and she asked If he could see them and he said no he can only sense them. My guess is that the woman cried because she's a higher class and can actually see the creatures and it was probably pretty horrific to look at. Just my two cents tho

Ok ok ok hear me out! My conclusion is this. The story MIGHT be about a delivery guy (main character) who got an important package to deliver (The BABY!) and hes being hunted by Death (in another trailer they showed a guy who controlled all of those floating beings) and my get is that the baby is their last *hope* (but idk what hope though because i havent gotten that far) but tha game looks amazing tho

We don't know the backstory but it would help to know that Hideo Kojima is the creator of this game, he's the brain behind the Metal Gear series and is the man who man japan weird

The main point of the game is connection. There s a Japanese proverb roughly about how man uses the stick and the rope. As game players you use the stick to fight or defend. Kojima s idea is for players to use the rope to connect players in one story. Beyond that, the best theory video is from UA-camr YongYea. Check it out

There was a bomb the created the world that the trailer takes place and that bomb is what created BTs. BTs are the creatures that we are present through the trailers, but on thing to note is that they are beings that are greater than our knowledge, we don’t know how they work but all we know is that they are connected. In the game Reedus has a baby that allows him to see the BTs as a solid object. Like he said he can’t see them but he can sense them. But once he connects to the other side, I’m talking about the baby, he can see the BTs. The baby allows him to see the BTs, the demon people are the people that worship the BTs and can connect to the other side when ever they please,the people who don’t have a baby, they have the light thing that can sense BTs, but since the character has the baby, he can sense and see them, and the floating people I’m guessing is what will end all life, like they wore saying on another trailer, there was a bomb that created the bing bang, a bomb the created war or something around those lines, another bomb that created new life and that’s what created the BTs and the last bomb is what will end all life... and that bomb is called death stranding.
It's Kojima... we're just gonna have to wait for the game.

OK let me explain to those people who didn't understand. When he's walking, you see him carrying stuff right, yeah, so that means nothing to me. But when he pulls out the baby in that container what that tells me is that Hideo Kojima was on something when he pitched it to Sony... and the shadowy figures, well it's clearly a cry out of the Metal Gear Survive game characters wanting to be in a Kojima production.

It seems he's on another planet dimension or something and these are aliens? Rainfall speeds up times which is why the plants grow then die immediately. Norman reedus character and maybe a few others have a immunity to some of this stuff. Kojima did a interview where he talked a lil bit about the game mechanics and the science or science fiction elements in it.

Fans of eccentric and ambitious Metal Gear Solidcreator Hideo Kojima went into this event wondering what, if anything, they'd see of his new game Death Stranding. Would it just be another cryptic cinematic trailer? Would something about the story be revealed? Was this game really just an elaborate ruse in which Kojima released a bizarre trailer every year with no actual game to be released? (Probably not that one.)
Those fans got a real deep dive in the form of a long gameplay demonstration. But it introduced just as many questions as it answered.
The demo showed the game's apparent protagonist (played by Norman Reedus) traversing vast landscapes in some kind of protective suit, with a baby in stasis on his back. The landscape looked desolate and hostile, but not entirely alien. In a cave, he was stalked by an invisible monster that seemed to have hands for feet, judging from its ominous footprints in the mud.
He then met a woman-a new character we haven't seen before-and they commenced with some cryptic dialogue that didn't reveal anything helpful about the plot. Again: plenty of new questions.
A later sequence showed Reedus' character walking through a shadowy landscape with a sensor dish emerging from his backpack. The dish seemed to reveal shadowy figures floating in the air; they looked like adult humans, but some of them were attached to umbilical cords. Several of them surrounded him and dragged him into the mud in a horrific way.
The trailer ended with the new female character eating a strange slug. I'm not even going to try to speculate about what any of this meant, but the visuals were unique and gripping
Death Stranding is an action game set in an open world environment, which also includes multiplayer functions.[2][3] Kojima compared the genre to how his earlier game Metal Gear- now considered as a stealth game - was called an action game during its release because the stealth genre was not considered to exist at the time.[4][5]
According to Kojima, one of the key aspects of the announcement trailer was the idea of a connection between life and death.[4] He explained one of the main themes through a short story by Kōbō Abe; The first tool created by humans was a stick, meant as protection by putting a distance between oneself and "bad things", and that the second was a rope, used to secure things one finds important. Kojima compared the main "tools" in action games - punching, shooting and kicking - to sticks, and that in Death Stranding, he wants people to communicate through the game equivalent of ropes.[4] While the title refers to the phenomenon of cetacean stranding, "strand" is an alternative meaning for shore, and also stands for strings and connections.[6][7]
When the main character, named Sam, dies, the player is sent to another world, referred to as an upside-down world submerged in water. Kojima suggests that when the player returns to the world of the living, anything they did or any damage that an explosion or the likes causes when the player dies remains persistent in the world and does not go away. Another element talked about is the existence of a type of rain, called "Timefall", with the ability to age or deteriorate whatever it hits. Another gameplay element talked about is the player's ability to interact with the environment and wander outside of the character's body, as well as recovering items when they die.[8]
